Citanest Forte--its use in oral surgery.

A clinical evaluation of Citanest Forte (brand of prilocaine HCL) was carried out on 3,000 oral surgery patients to determine the effectiveness and safety of the drug. Citanest Forte has been found to be a useful local anesthetic, and the results of this study would indicate that it can be effectively and safely substituted for other local anesthetic solutions currently in use for all routine oral surgery procedures.

Endoscopic sinus surgery in sinus-oral pathology.

OBJECTIVE: Oral surgery is compounded by safe and well-known techniques and presents a low rate of complications. When the superior alveolar ridge is approached, surgery may result in oroantral fistula or tooth or implant dislocation in maxillary sinus. Those conditions lead to development of the maxillary sinusitis that, if underestimated, may evolve in orbital cellulitis and cerebritis or cerebral abscess. Our work aimed to compare the surgical techniques suitable for treatment of those complications and define the better surgical strategy. METHODS: Between 2005 and 2010, 55 patients, prese

Propofol-remifentanil TCI sedation for oral surgery.

BACKGROUND AND OBJECTIVES: To investigate the safety and efficacy of a propofol-remifentanil target controlled infusion (TCI) sedation technique for oral surgery. METHODS: This prospective study involved patient and registered nurse (RN) questionnaires to assess the efficacy of sedation during surgery. Outpatients having dental extractions in a private office-based practice with TCI sedation were monitored with a Sleep Apnoea Monitor (SAM) to measure the number of Oxygen Desaturation Events (ODEs), defined as a drop in blood oxygenation to below 94%. RESULTS: Patient- and RN-assessed questio

Oral submucous fibrosis-New dimensions in surgery.

The oral submucous fibrosis (OSMF) has been managed earlier by various medical and surgical modalities but none of these methods is found to give relief to these patients. Hence, a new method has been developed by us. Patients, who reported with pallor of the palate cheek, oropharyn and the tonsils, due to the fibrosis, leading on to mild, moderate to severe trismus, were given a good long-term rehef by this surgical method. This study involved 60 patients between 1979 and 2000 with a female-to-male ratio of 1∶3 with age ranging from 13 to 60 years suffering from the disease. The surgical meth

Postoperative pain control for outpatient oral surgery.

16 healthy patients requiring removal of bilateral symmetrically-impacted mandibular third molars participated in a double-blind randomised crossover trial to test the effectiveness of postoperative pain control using a long-acting anti-inflammatory agent (diflunisal) in combination with a long-acting local anaesthetic agent (bupivacaine). Results were compared to the more traditional method of using an oral analgesic with shorter duration of action (paracetamol with codeine) with lignocaine as the local anaesthetic. Using a visual analogue pain scale, patients reported that significantly redu

Malignant hyperpyrexia: successful management following oral surgery.

Malignant hyperpyrexia (M.H.) is a rare, important and frequently fatal disease provoked by inhalational anaesthetic agents and other drugs. It is manifested most frequently by tachycardia, a rise in core body temperature, muscle stiffness and is associated with profound metabolic effects. It may be transmitted in a predictable familial dominant fashion, but sporadic cases in whom this transference cannot be demonstrated do occur. Susceptibility may be revealed on questioning regarding previous anaesthetic exposure. One such unexpected but successfsully treated case of M.H. is described in det

Cardiovascular effects of cannabinol during oral surgery.

Fifty-five clinical trials were conducted to determine the cardiovascular combined effects of stressful oral surgery and systemic cannabinols. In a first study, delta9-tetrahydrocannabinol (THC) was given IV as a premedicant and compared with diazepam and with a placebo. A classic dose-related tachycardia followed THC injection. The peak heart rate response of patients premedicated with 0.044 mg/kg THC was 24.1% higher (p less than 0.05) than the peak effect for a nonsurgerized control group, suggesting a synergistic effect between THC and surgical stress. A predisposition to syncopal hypotens

Bisphosphonates - Boon or bane for oral and maxillofacial surgery? Anti-resorptive drugs and their potential role in dentistry and oral and maxillofacial surgery.

Antiresorptive drugs such as bisphosphonates and denosumab are vital medications in the therapy of osteoporosis and in the treatment of cancer with osseous metastases. A rare but clinically relevant adverse effect is medication-related osteonecrosis of the jaw (MRONJ). This review outlines epidemiology, clinical features, diagnosis, treatment, and prevention of MRONJ. In addition, antiresorptives show promise in rare bone diseases relevant to oral and maxillofacial surgery, such as aneurysmal bone cysts, central giant cell granuloma, and diffuse sclerosing osteomyelitis. While early evidence i

Comparison of Oral Triclofos and Oral Midazolam as Premedication in Children undergoing Elective Surgery.

BACKGROUND: Children who have experienced previous hospital admission, operation, procedures, and needle pricks are more reactive to subsequent anesthetic procedures. Many sedative agents have been used for the purpose of premedication, but few of them can be given orally, thus avoiding the pricks. Midazolam, being one such choices, can be given orally, intranasally, and parenterally but has unpredictable response. Triclofos, available as sweet syrup, is a phosphorylated derivative of chloral hydrate, has been proven to be effective within 30 min in doses of 25-75 mg/kg. Hence, this study comp

[Innovative methods and developments in oral care. 3D planning in oral and maxillofacial surgery].

With the use of cone beam computed tomography, intraoral scanning and 3D stereophotogrammetry, a virtual 3D head model of a patient can be reconstructed with image fusion. In this way, the malposition, deficiency and other anomalies at the level of bone, dentition and soft tissue can be quantified objectively. The desired position of the dentition, occlusion and soft tissue in the facial profile can be virtually drawn in and used as a guideline for treatment planning. Based on the principle of backward planning, it is possible to determine what repositioning of the jaw is required, where there

[Anesthesiological Concepts in Pediatric Oral and Maxillofacial Surgery].

Anesthesiological care in pediatric oral and maxillofacial (OMF) surgery is challenging due to the proximity of the surgical field to the airway, a high prevalence of craniofacial malformations, and a high frequency of associated syndromal conditions. This review outlines current perioperative strategies for managing complex cases, such as cleft lip and palate repair and craniosynostosis correction. A comprehensive evaluation of clinical practices and current guidelines, including the 2024 ESAIC airway management recommendations and the 2024 German S3-guideline on cleft lip and palate, was con

Sedation Criteria for Intravenous Sedation in Oral Surgery.

Intravenous sedation is a key component of oral and maxillofacial surgery, improving patient comfort and procedural tolerance. Safe administration depends on a structured pre-sedation evaluation that includes comprehensive medical history, airway assessment, ASA classification, BMI evaluation, and screening for obstructive sleep apnea. Systemic conditions such as diabetes, anemia, sickle cell disease, and adrenal insufficiency significantly influence sedation risk and require individualized management. Together, these criteria support effective risk stratification, guide sedation planning, and
