Diagnostic yield of adding solid food swallows during high-resolution manometry in esophageal motility disorders.

BACKGROUND: High-resolution manometry (HRM) is a key method to evaluate esophageal motility disorders. Current evaluation is usually performed with single water swallows (SWS) that may not challenge esophageal function or reproduce symptoms. Solid food swallows (SFS) could increase the diagnostic yield for clinically relevant disorders. METHODS: Patients with dysphagia referred for esophageal HRM during a 2-year period in a single center were reviewed retrospectively and included if SFS was performed during HRM. Chicago classification v3.0 was used to define esophageal motility disorders. KE

Multiple Rapid Swallows (MRS) Complements Single-Swallow (SS) Analysis for High-Resolution Esophageal Manometry (HREM).

BACKGROUND: Multiple rapid swallows (MRS) during HREM assess post-deglutitive inhibition, LES relaxation, and subsequent esophageal contraction. AIMS: (1) To determine the relationship between MRS and single-swallow (SS) responses and (2) to determine if MRS provides information for symptomatic patients. METHODS: Healthy volunteers (HVs) and patients underwent HREM [30-s landmark, ten 5-mL SS, MRS (5 consecutive 2-mL swallows every 2-3 s)] and were analyzed with ManoView software version 3 with CC version 3.0. RESULTS: In 20 HVs, MRS response consisted of: (1) reduction in GEJ pressure; (2)

Involvement of the epithelial sodium channel in initiation of mechanically evoked swallows in anaesthetized rats.

KEY POINTS: Afferents carried by the superior laryngeal nerve play a primary role in the initiation of laryngeal mechanically evoked swallows in anaesthetized rats. Amiloride and its analogues inhibit swallowing evoked by mechanical stimulation, but not swallowing evoked by chemical and electrical stimulation. The epithelial sodium channel is probably involved in the initiation of laryngeal mechanically evoked swallows. ABSTRACT: The swallowing reflex plays a critical role in airway protection. Because impaired laryngeal mechanosensation is associated with food bolus aspiration, it is importa

Differential response properties of peripherally and cortically evoked swallows by electrical stimulation in anesthetized rats.

We compared onset latency, motor-response patterns, and the effect of electrical stimulation of the cortical masticatory area between peripherally and cortically evoked swallows by electrical stimulation in anesthetized rats. The number of swallows and the motor patterns were determined using electromyographic recordings from the thyrohyoid, digastric, and masseter muscles. The onset latency of the first swallow evoked by electrical stimulation of the cortical swallowing area (Cx) was significantly longer than that evoked by stimulation of the superior laryngeal nerve (SLN). The duration of th

Reproducibility of axial force and manometric recordings in the oesophagus during wet and dry swallows.

BACKGROUND: Manometry is the golden standard to diagnose oesophageal motility disorders but it gives an indirect picture of the peristalsis by measuring radial force only. A novel probe design using electrical impedance recordings enabled axial force and manometry to be recorded simultaneously. Using this method the aims were to study the amplitude and duration of peristaltic contractions, to test the correlation between axial force and manometry, and the reproducibility of the method. METHODS: Ten healthy men were included twice. The probe was positioned 5 cm proximal to the lower oesophagea

Mercury and growth of tree swallows at Acadia National Park, and at Orono, Maine, USA.

In 1997 and 1998 we weighed nestling tree swallows (Tachycineta bicolor) and measured selected body components at two colonies: Acadia National Park on Mt. Desert Island, and at Orono, ME. We used differences in mean growth variables among individual nestlings to evaluate differences between colonies, years, and amount of total mercury (THg) in carcasses and methyl mercury (MeHg) in feathers. We marked nestlings on the day hatched and measured body components every day in 1997 and every other day in 1998 until nestlings fledged. We calculated linear growth rates and asymptotic means as appropr

Reproductive success of barn swallows nesting near a selenium-contaminated lake in east Texas, USA.

Reproductive success and contaminant levels in 1986 and 1987 were compared between Barn Swallows nesting at selenium-contaminated Martin Lake, Texas, USA, and swallows nesting at a reference site. Nests were initiated about the same time or earlier at Martin Lake than at the reference site and clutch size was similar between the two locations. Nest success was significantly higher at Martin Lake than at the reference site and no embryo or chick deformities were documented. Selenium concentrations in 14 of 20 eggs from Martin Lake were above background (> 3 ppm, dry weight); two of 20 eggs cont

The Application of Ultrasound Evaluation of Swallowing to the Analysis of Hyoid Kinematics in Healthy Swallows.

PURPOSE: Using ultrasound as an adjunct tool for swallowing assessment has gained significant momentum in recent years, with research gaps in areas such as speech and language therapist-driven protocol and measurement methods. This study outlines the recording protocol of Ultrasound Evaluation of Swallowing (USES). Additionally, a set of multidimensional measurements capturing the hyoid kinematics in typical swallows was compared with previous studies to evaluate the current protocol and to develop an ultrasound database of healthy swallows to further the clinical implementation of USES. Swallowing dıfficulty and swallowing anxiety in patients with type-II diabetes mellitus.

BACKGROUND: Type-II Diabetes Mellitus (TII-DM) is the most common endocrine disorder in people who are over 65 years of age. It leads to a decrease in muscle strength and impaired muscle coordination. Ageing and weakness cause swallowing difficulty (SD). OBJECTIVE: The aim of this study was to evaluate the relationship between duration of disease, SD and swallowing anxiety (SA) in TII-DM patients. STUDY DESIGN: Prospective case-control study. METHODS: A total of 103 elderly individuals (74 females and 29 males) participated in this study. Fifty-two elderly patients (aged 70.27 ± 4.65 years)

The Role of the Cerebellum in Control of Swallow: Evidence of Inspiratory Activity During Swallow.

Anatomical connections are reported between the cerebellum and brainstem nuclei involved in swallow such as the nucleus tractus solitarius, nucleus ambiguus, and Kölliker-fuse nuclei. Despite these connections, a functional role of the cerebellum during swallow has not been elucidated. Therefore, we examined the effects of cerebellectomy on swallow muscle recruitment and swallow-breathing coordination in anesthetized freely breathing cats. Electromyograms were recorded from upper airway, pharyngeal, laryngeal, diaphragm, and chest wall muscles before and after complete cerebellectomy. Removal

Neonatal swallowing assessment using fiberoptic endoscopic evaluation of swallowing (FEES).

OBJECTIVES: Swallowing disorders which lead to aspiration are common in premature infants with a postmenstrual age (PMA) of >36 weeks. Aspiration is often silent and the unique symptom is desaturation during feeding. The aim of this study was 1) to determine the number of prematures with desaturations during feeding due to aspiration, using Fiberoptic Endoscopic Evaluation of Swallowing (FEES); 2) to relate clinical factors and FEES findings to aspiration; and 3) to describe type and efficacy of suggested treatments. METHODS: A retrospective review of 62 ex-premature babies with a median PMA

Diagnosis of spasmodic dysphonia manifested by swallowing difficulty in videofluoroscopic swallowing study.

Spasmodic dysphonia is defined as a focal laryngeal disorder characterized by dystonic spasms of the vocal cord during speech. We described a case of a 22-year-old male patient who presented complaining of idiopathic difficulty swallowing that suddenly developed 6 months ago. The patient also reported pharyngolaryngeal pain, throat discomfort, dyspnea, and voice change. Because laryngoscopy found no specific problems, an electrodiagnostic study and videofluoroscopic swallowing study (VFSS) were performed to find the cause of dysphagia. The VFSS revealed continuous twitch-like involuntary movem

Comparison of liquid swallowing kinematics during tablet swallowing with liquid and liquid-only swallowing in patients with dysphagia: A pilot study.

OBJECTIVE: Difficulty swallowing oral medications is associated with an increased risk of liquid aspiration or penetration in patients with dysphagia. However, the underlying mechanism remains unclear. This study aimed to compare the swallowing kinematics during tablet swallowing with liquid and liquid swallowing alone in patients with dysphagia using a videofluoroscopic swallowing study (VFSS). The presence of a tablet was hypothesized to alter swallowing kinematics and delay the initiation of pharyngeal swallowing. METHODS: In this single-center retrospective study, 40 patients with mild dy

Investigation of the Impact of Preoperative Swallowing Therapy on Swallowing Function in Partial Laryngectomy.

Partial laryngectomy can substantially compromise swallowing safety and efficiency, leading to reduced swallowing-related quality of life in patients with head and neck cancer (HNC). Although postoperative swallowing therapy is effective in managing dysphagia, postoperative pain, fatigue, and anatomical alterations may limit early rehabilitation adherence. This study aimed to investigate whether structured preoperative swallowing therapy enhances postoperative swallowing recovery in patients undergoing partial laryngectomy. Thirty patients with HNC were included. The study group (n = 16) recei

[Swallowing and swallowing disorders at the focus of otorhinolaryngology : From diagnosis to individualized treatment].

Dysphagia refers to painless impairment of the transport of liquid or solid food from the oral cavity to the stomach. If pain accompanies swallowing, this is termed odynophagia, and discomfort when swallowing saliva suggests a globus pharyngis. Swallowing disorders can occur during any one or all phases of bolus transport. The causes are manifold, due to the involvement of central control areas within the brain as well as the peripheral nervous system. The diagnosis consists of a detailed case history as well as screening forms and questionnaires. In Germany, the gold standard diagnostic proce

Neural mechanisms of swallowing and effects of taste and other stimuli on swallow initiation.

Swallowing involves several motor processes such as bolus formation and intraoral transport of a food bolus (oral stage) and a series of visceral events that occur in a relatively fixed timed sequence but are to some degree modifiable (pharyngeal stage or swallow reflex). Reflecting the progressive aging of society, patients with swallowing disorders (i.e., dysphagia) are increasing. Therefore, there is expanding social demand for the development of better rehabilitation treatment of dysphagic patients. To date, many dysphagia diets have been developed and are available commercially to help br
