Tuberculous synovitis of the elbow joint.

Tuberculous synovitis in the elbow joint is extremely rare in developed countries. We describe a 68-year-old man who had had a gradually enlarging mass over the volar side of the left proximal forearm near the elbow joint for 4 months. Plain roentgenograms of the diseased elbow showed early osteoarthritic change. Magnetic resonance imaging revealed diffuse synovitis with a large 8 x 8 cm extra-articular synovial cyst. Synovectomy was performed and histopathologic examination of the surgical specimen revealed granulomatous inflammation with caseation, prominent Langhan's giant cells, and sparse…

Haemophilic synovitis: is rifampicin an alternative?

Rifampicin synoviorthesis has been empirically used for the treatment of haemophilic synovitis for some time. This paper reports on the experience of three Latin American centers with this treatment and compares it with radioactive synoviorthesis results. Chemical synoviorthesis with rifampicin is best indicated in younger patients (< 15 years) and small joint (ankles and elbows).

Synovitis secondary to giardiasis in children.

Giardia lamblia, a gastrointestinal protozoan, is one of the most common disease-causing parasites in the world. Giardiasis is primarily encountered in areas with poor sanitation, but it is also seen in more developed countries. A possible sequela of Giardia infections of the bowel is reactive arthritis or synovitis. Few reports of synovitis secondary to giardiasis exist in the literature. Arthropathy secondary to giardiasis is uncommon, but may be underdiagnosed. In this study, Giardia synovitis was diagnosed in two children at a major children's hospital over a 20-year period. Both were boys…

Hemophilic synovitis of the knee and the elbow.

A prospective study from 1974 to 1996 was done to determine optimal treatment for chronic hemophilic synovitis of the knee and synovitis of the elbow. Sixty-five patients with synovitis affecting 65 knee joints and 40 patients who had synovitis of the elbow (44 elbows), despite a 3-month trial of prophylactic substitution therapy, were treated by synovectomy. Radiation synovectomies (Au-198 synoviorthesis) were done on 38 knees, open surgical synovectomy on 18, and nine had an arthroscopic procedure. Radioactive gold synoviorthesis was performed on 29 elbows, and 15 had a resection of the radi…

Recurrences of transient synovitis of the hip.

Thirty six children with transient synovitis of the hip had a total of 80 recurrences, 69 of them personally observed, and 11 described by the mother. No features distinguished the initial attack of those who had a recurrence from that of the 18 children who have not so far had a recurrence. We analysed the total of 126 episodes. In 72 there was evidence of an associated infection from the history, clinical signs, and a raised antistreptolysin O titre or isolation of a pathogen from a throat swab. The prognosis is difficult to assess because the symptoms, signs, and special investigations are …

Arthroscopic debridement of the ankle in synovitis.

Inflammation and hypertrophy of the ankle joint's synovial lining can occur due to various causes. Chronic pain and degenerative changes may be due to synovitis causing clinical manifestations through traction on the joint capsule. The failure of conservative treatment for at least six months indicates arthroscopic debridement, which can provide significant pain relief without the morbidity of extensive surgical exposures. This study was therefore conducted to establish the functional results of arthroscopic debridement of the ankle joint in synovitis. Fifteen patients with chronic ankle pain …

Pigmented villonodular synovitis of the hip.

Pigmented villonodular synovitis (PVNS) is a rare disease that can affect any joint, bursa or tendon sheath. The hip is less frequently affected than the knee, and hence is less discussed in scientific journals.PVNS of the hip mainly occurs in young adults, requiring early diagnosis and adequate treatment to obtain good results. There is no consensus on the management of PVNS of the hip in current literature. We will discuss the options for surgical intervention in hip PVNS using a literature review of clinical, biological, etiological, histological and radiographic aspects of the disease. Foreign body synovitis in the Pacific.

Foreign body synovitis in the Pacific region typically involves a penetrating injury to a joint. The introduced biomaterial produces an inflammatory reaction or innoculates the tissue with bacteria, creating an infection. Each year millions of people visit the Hawaiian Islands to hike, surf, snorkel, and participate in other outdoor activities, creating an array of interactions between people and nature. The two most commonly reported penetrating foreign body joint injuries are sea urchin synovitis and synovitis due to implantation of organic material such as wood splinters or plant thorns. In…

The role of synovitis in osteoarthritis pathogenesis.

Research into the pathophysiology of osteoarthritis (OA) has focused on cartilage and peri-articular bone, but there is increasing recogniti … ological changes observed in the SM in OA generally include features indicative of an inflammatory "synovitis"; specifically they encompass a range of abnormalities, such as synovial lining hyperplasia, infiltration of macrophages and lymphocytes, neoangiogenesis and fibrosis. The pattern of synovial reaction varies with disease duration and associated meta …

Echinococcal synovitis of the knee joint.

Bone lesions are present in 1% to 2% of cases of hydatid disease. Hydatid synovitis can usually be identified due to secondary extension from the adjacent bone, or infrequently after hematogenous spread. We present an extremely rare case of hydatid synovitis without bony involvement. A 74-year-old man with diagnosed hydatid disease was admitted to our department because of left knee swelling. Neither physical examination nor laboratory studies revealed any remarkable findings. Radiographic evaluation of the knee joint was noncontributory. The patient underwent an arthroscopically assisted syno…

Pathology of synovitis and hemophilic arthropathy.

Hemophilia is an X chromosome linked disease characterized by an increased tendency to hemorrhage. Due to recurrent haemarthroses specific c … before synovial inflammation is evident. These studies show that synovitis is involved, but that it is not the only mechanism in the joint damage caused by intra-articular bleeding. These findings do not contradict the current concept of blood-induced cartilage damage in …

Transient synovitis of the hip in children.

Transient synovitis is the most common cause of acute hip pain in children three to 10 years of age. Children with this condition typically present with hip pain for one to three days, accompanied by limping or the refusal to bear weight. Transient synovitis has an uncertain etiology and remains a diagnosis of exclusion. First, septic arthritis must be ruled out, since femoral head destruction, degenerative arthritis and permanent deformity can occur if septic arthritis is not treated promptly. Septic arthritis should be suspected in a patient with severe pain or spasm on hip movement or palpa…

Benign synovitis of the hip in adults.

Hip synovitis plays a part in many rheumatic diseases. In the young adult acute arthritis may be due to reaction to trauma, infection or gout. Although transient synovitis of the hip is a well-known phenomena in children the condition is not well-documented in adults. We present 10 young adults with idiopathic transient synovitis of hip who had attended the Soroka Medical Centre between 1986 and 1990.

Rarity of synovitis in polymyalgia rheumatica.

The joints of 56 patients with polymyalgia rheumatica were examined for evidence of inflammatory synovitis. x Rays, isotope scans, and thermography supplemented clinical examination. Control sternoclavicular joints were examined at necropsy. Peripheral and axial synovitis were uncommon and the results contrast with the findings of several recent studies.
