A microtissue culture method for the assay of low concentrations of diphtheria antitoxin in human sera has been developed, using a monkey kidney cell (VERO) culture technique. Results obtained with sera from nonvaccinated children and with immune sera from children vaccinated with three and four injections of diphtheria pertussis tetanus vaccine were in agreement with antitoxin levels considered necessary to denote immunity to diphtheria. Two cases of bites by the common viper were reviewed. Emphasis was focused on proper medical monitoring in an intensive care unit for a minimum of one day. Observations should include ECG, Hb, leucocytes, electrolytes in both blood and urine, CPK, and coagulation factors. Surgical intervention, heparin and cryotherapy are not recommended. Proper treatment with immobilisation of the bitten extremity, tetanus antitoxin and volume therapy is recommended. In severe cases Zagreb Antitoxin should be used. Patients with cardio-pulmonary problems require special attention.

Blood samples were obtained from school entrants whose primary immunization schedule had consisted of three doses of DT or DTP vaccine and three doses of OPV all given before the age of 8 months. The sera were separated and assayed for diphtheria antitoxin, tetanus antitoxin and antibodies to the three serotypes of poliovirus. The results of the assays showed that the abbreviated three dose schedule induced satisfactory immunity to all five infections until school entry and that a reinforcing dose at 18 months was unnecessary.

We report the case of a 37 years old woman who asked for clinical advice about revaccination with tetanus toxoid. An adverse reaction to the last booster dose of vaccine, 21 years before, was recorded by a clinical doctor on the vaccination card. Fifty-five people were examined for tetanus antitoxin level two months after immunization with a single 0.5 ml dose of concentrated tetanus toxoid vaccine; only 7.1% of patients infected with onchocerciasis became immunized, whereas 44.5% were immunized in the control group.
