Timing of antibiotic prophylaxis in tourniquet surgery.

It is customary to administer prophylactic antibiotics before exsanguination of the limb and inflation of a tourniquet in extremity surgery. To compare the clinical outcome in lower limb operations when prophylactic antibiotics were administered before versus after limb exsanguination and tourniquet inflation, we randomized patients to the administration of prophylactic antibiotics 5 minutes before exsanguination and inflation of the tourniquet (ABT) and administration of prophylactic antibiotics 1 minute after inflation of the tourniquet (AAT). A total of 106 patients completed the study, inc

Tourniquet use in childhood: a harmless procedure?

Tourniquet ischemia is widely used in limb surgery in every age group. In adults, tourniquet-related deep vein thrombosis and pulmonary embolism are recognized complications of tourniquet use. In healthy children, tourniquet-associated alterations of blood clotting physiology are assumed to have no clinical impact. Antithrombotic prophylaxis is, therefore, recommended only in the presence of pertinent risk factors such as extensive surgery, congenital thrombophilia, prolonged immobilization, and indwelling central venous line, however, it is not practiced in obese, otherwise healthy children.

Survey of tourniquet use in podiatric surgery.

Tourniquet use in foot and ankle surgery is common practice; however, the technique varies among foot and ankle surgeons and there are no standard guidelines. To analyze trends in foot and ankle tourniquet use, the authors conducted an e-mail survey. One thousand six hundred sixty-five foot and ankle surgeons were sent a tourniquet-use survey via e-mail, across Canada and the United States. Nineteen percent of the recipients completed and returned the surveys. Eleven (3.4%) rarely or never use a tourniquet and 8 (2.5%) use an Esmarch bandage tourniquet at the ankle. Most use pneumatic ankle cu

Treatment of the toe tourniquet syndrome in infants.

INTRODUCTION: The "toe tourniquet syndrome" is the circumferential strangulation by human hair or fibers of one or more toes in infants, which may induce prolonged ischemic injury and tissue necrosis. Release of the strangulation is mandatory to avoid autoamputation of the digit. We recently encountered several incompletely treated cases and would like to emphasize the effective method of treatment. METHODS: A retrospective analysis of pediatric emergency department records of patients treated for "toe tourniquet syndrome" during 1990 to 2001 was performed. Patients undergo a short, longitudi

Carpal tunnel release without a tourniquet.

Release of the transverse carpal ligament can be performed safely under local anesthesia. The addition of epinephrine to the local anesthetic will cause sufficient vasoconstriction to maintain a dry field without the need of a tourniquet. This allows the procedure to be performed without regard to tourniquet pain. It has been our technique of choice in patients with functioning hemodialysis fistulae. The procedure was performed in 21 wrists to assess its safety. This approach was found to be a safe, effective, and efficient alternative to the standard tourniquet-dependent procedure.

Atraumatic vascular anastomoses using a tourniquet.

Because of the occurrence of stenoses just beyond the distal anastomosis, lower leg exsanguination and the use of a mid-thigh tourniquet was adapted. From July 1, 1988 until June 30, 1990, 75 primary operations and 18 secondary operations on failing bypasses were performed using this technique. The primary operations were to the below-knee popliteal (4), anterior tibial (19), posterior tibial (16), peroneal (26), and inframalleolar arteries (10). The proximal anastomoses were also done while the tourniquet was inflated in 11 cases (three tibio-tibial and eight from the popliteal below the knee

The pneumatic tourniquet: a biomechanical and electrophysiological study.

The pneumatic tourniquet is used routinely in extremity surgery, not only to prevent unnecessary blood loss, but also to increase the safety of the operative procedure. The mechanical part of this study shows that the system can fail in either of two major ways, each causing serious problems. The tourniquet may be inadequate if the pressure delivered to the cuff is not accurately displayed on the gauge or if the pre-set pressure is not maintained for the duration of the operative procedure. In our hospital, three of seven tourniquets (43%) in daily use showed significant errors in this regard.…

[Recommendations and Protocols for the Safe Use of Tourniquets in Upper- and Lower-Extremity Surgeries].

The tourniquet (TNQ) interrupts blood flow to a given anatomical segment and has critical applications in orthopedic surgeries by providing a blood-free operating field. The risks and complications attributed to its use are increased pain, reperfusion injury, edema, deep venous thrombosis, and peripheral nerve injury. The main recommendations for TNQ use and to reduce the occurrence of complications include adequate limb padding; TNQ inflation to pressures of 50 mmHg and 100 mmHg above the perfusion pressure for the upper and lower limbs respectively; avoid TNQ use in children and patients wit

Tourniquets do not increase the total blood loss or re-amputation risk in transtibial amputations.

AIM: To investigate the total blood loss (TBL) and the safety with respect to the re-amputation rate after transtibial amputation (TTA) cond … ssociation with re-amputation was age (OR = 1.07; P = 0.02). CONCLUSION: The results indicate that tourniquets do not cause severe vascular damage with an increased postoperative bleeding or failure rate as the result.

Hair tourniquet syndrome: A retrospective study.

OBJECTIVE: Hair thread tourniquet syndrome (HTS) is a pediatric condition in which human hair or synthetic fiber encircles and strangulates a body appendage causing tissue necrosis. Few epidemiologic studies have been done. Our objective was to better define the demographics, clinical features, and predisposing factors for this condition. MATERIALS AND METHODS: Medical records of all infants up to 1 year old seen in the orthopedic emergency department of Galilee Medical Center were searched for the diagnosis of HTS or its ICD equivalent. RESULTS: Forty-one cases of hair tourniquet syndrome (

Appendicular Tourniquet: A Cause of Intestinal Obstruction.

Intestinal obstruction is one of the common surgical emergencies seen in daily practice. Postoperative adhesions are notorious for being the most common cause for intestinal obstruction. Occasionally, laparotomy findings do come as a surprise to surgeons. Here one such case is discussed. A patient was operated on with suspicion of intestinal obstruction secondary to postoperative adhesions. However, laparotomy revealed the appendix to be inflamed, curled around the terminal ileum and acting as a tourniquet.

Safety and efficacy of a new tourniquet system.

BACKGROUND: In upper limb surgery, the pneumatic tourniquet is an essential tool to provide a clean, bloodless surgical field, improving visualization of anatomical structures and preventing iatrogenic failure. Optimal inflation pressure to accomplish these objects without injuring normal tissue and inducing complications is not yet established. Use of the minimum tourniquet pressure necessary to produce a bloodless surgical field is preferable in order to prevent injury to normal tissue. Various methods have been implemented in an effort to lower effective cuff pressure. The purpose of this s

Toe-tourniquet syndrome: a diagnostic dilemma!

Strangulation of digits, the 'toe-tourniquet' syndrome needs prompt intervention as failure to recognise the condition can lead to ischaemia and loss of the appendage. It is a common condition though relatively under reported. Those who deal with children more frequently are aware of the condition but this is not the case for all medical practitioners and hence the diagnostic dilemma for accidental injury or child abuse arises. We report this case to increase physician awareness of the syndrome, to highlight the importance of early release of the tourniquet and to avoid misinterpretation of th
