Repetitive transcranial magnetic stimulation in primary sleep disorders.

Repetitive transcranial magnetic stimulation (rTMS) is a widely used non-invasive neuromodulatory technique. When applied in sleep medicine, the main hypothesis explaining its effects concerns the modulation of synaptic plasticity and the strength of connections between the brain areas involved in sleep disorders. Recently, there has been a significant increase in the publication of rTMS studies in primary sleep disorders. A multi-database-based search converges on the evidence that rTMS is safe and feasible in chronic insomnia, obstructive sleep apnea syndrome (OSAS), restless legs syndrome (…

Efficacy and Safety of Intensive Transcranial Magnetic Stimulation.

Repetitive transcranial magnetic stimulation (rTMS) is customarily applied on a daily basis for prolonged periods of time for the treatment of psychiatric diseases. The process is demanding in terms of staff and patient time, and the onset of the effect is slow. Recently, intensive rTMS protocols have been introduced in which stimulation is applied to the same area more than once a day with a higher than standard number of pulses. This article reviews 16 articles to determine the safety and efficacy of such protocols. Intensive rTMS seems to be effective in various mental disorders. It appears…

Transcranial magnetic stimulation in myoclonus of different aetiologies.

Transcranial magnetic stimulation (TMS) may represent a valuable tool for investigating important neurophysiological and pathophysiological aspects of myoclonus. Moreover, repetitive TMS (rTMS) can influence neural activity. In this review we performed a systematic search of all studies using TMS in order to explore cortical excitability/plasticity and rTMS for the treatment of myoclonus due to different aetiologies. We identified and reviewed 40 articles matching the inclusion criteria; 415 patients were included in these studies. The reviewed TMS studies have detected abnormalities in motor …

[Repetitive transcranial magnetic stimulation in neurology and psychiatry].

Transcranial magnetic stimulation (TMS) is a method based on the excitation of neurons in the brain cortex by an alternating magnetic field. Repetitive TMS (rTMS) is a kind of stimulation, in which a series of pulses are generated. rTMS can modulate different cortical area's activity and change their interactions using different protocols. The review shows the application of rTMS in treatment of various neurological (stroke, Parkinson's disease, dystonia, chronic pain syndromes, tinnitus, epilepsy, spasticity syndrome) and psychiatric (depression and anxiety) disorders. Authors collected all t…

Navigated transcranial magnetic stimulation in amyotrophic lateral sclerosis.

INTRODUCTION: Amyotrophic lateral sclerosis (ALS) is a set of disorders associated with preferential degeneration of both upper and lower motor neurons. Navigated transcranial magnetic stimulation (nTMS) is a tool used to perform noninvasive functional brain mapping. We aimed to assess function of upper motor neurons in ALS. METHODS: nTMS was performed on 30 patients with ALS (mean age 54.4 ± 12.1 years) and 24 healthy volunteers (mean age 32.7 ± 13.3 years). RESULTS: The resting motor threshold (MT) was significantly higher in ALS patients compared with controls (P < 0.001). The mean map ar…

Transcranial magnetic stimulation and sleep disorders: pathophysiologic insights.

The neural mechanisms underlying the development of the most common intrinsic sleep disorders are not completely known. Therefore, there is a great need for noninvasive tools which can be used to better understand the pathophysiology of these diseases. Transcranial magnetic stimulation (TMS) offers a method to noninvasively investigate the functional integrity of the motor cortex and its corticospinal projections in neurologic and psychiatric diseases. To date, TMS studies have revealed cortical and corticospinal dysfunction in several sleep disorders, with cortical hyperexcitability being a c…

Repetitive transcranial magnetic stimulation safely administered after seizure.

High-frequency repetitive transcranial magnetic stimulation (rTMS) over left dorsolateral prefrontal cortex has been clearly reported to be effective in the treatment of depression. We report a patient with treatment-resistant depression, without any known risk factors, who developed seizure during the fourth rTMS session. After written informed consent, he received further rTMS sessions under cover of antiepileptic drug valproate and improved without recurrence of seizure.

Enhanced visual perception with occipital transcranial magnetic stimulation.

Transcranial magnetic stimulation (TMS) over the occipital pole can produce an illusory percept of a light flash (or 'phosphene'), suggesting an excitatory effect. Whereas previous reported effects produced by single-pulse occipital pole TMS are typically disruptive, here we report the first demonstration of a location-specific facilitatory effect on visual perception in humans. Observers performed a spatial cueing orientation discrimination task. An orientation target was presented in one of two peripheral placeholders. A single pulse below the phosphene threshold applied to the occipital pol…

When transcranial magnetic stimulation (TMS) modulates feature integration.

How the brain integrates visual information across time into coherent percepts is an open question. Here, we presented two verniers with opposite offset directions one after the other. A vernier consists of two vertical bars that are horizontally offset. When the two verniers are separated by a blank screen (interstimulus interval, ISI), the two verniers are perceived either as two separate entities or as one vernier with the offset moving from one side to the other depending on the ISI. In both cases, their offsets can be reported independently. Transcranial magnet stimulation (TMS) over the

Applications of transcranial magnetic stimulation in sleep medicine.

Transcranial magnetic stimulation (TMS) is a new method, developed nearly 20 years ago, that allows the study of cortical excitability. The whole brain undergoes profound changes in sleep. Motor evoked potentials (MEPs) have been used to trace the effects of sleep on cortical excitability and to the corticomotoneuron connections. Although in the past some technical aspects limited the application of TMS in sleep, recently we observed a new explosion of interest in this field. The main body of data was gathered on sleep physiology, but its diseases or syndromes were also studied in detail. Many…

Dissociation of neglect subtypes with transcranial magnetic stimulation.

OBJECTIVE: To identify two forms of hemispatial neglect, attentional and intentional, in healthy volunteers using frontal and parietal repetitive transcranial magnetic stimulation (rTMS). METHODS: Ten healthy volunteers performed line bisection tasks while viewing stimuli on closed circuit TV. Direct view of the exploring hand and the target was precluded and the TV monitor guided performance. In the normal condition, the direction of hand movement on the table (workspace) was congruent with that on the monitor, and in the reversed condition, the lateral movement in the workspace occurred in …

Suppressing the surface field during transcranial magnetic stimulation.

Transcranial magnetic stimulation (TMS) is used commonly as both a diagnostic tool and as an alternative to electric shock therapy for the treatment of clinical depression. Among the clinical issues encountered in its use is the mitigation of accompanying pain. The objective becomes one of minimizing the induced surface field while still achieving the target field objective. Three techniques discussed for realizing this end are 1) placing a conducting shield over a portion of the central target region, 2) using supplementary coils of opposite polarity in tandem with the primary field, and 3) o…

Transcranial magnetic stimulation in acute facial nerve injury.

OBJECTIVE/HYPOTHESIS: Available electrodiagnostic tests that are used to evaluate facial nerve injury examine the nerve distal to the stylomastoid foramen; because most facial nerve injuries are within the temporal bone, the tests cannot evaluate the nerve at or across the injury site. The interpretation of these tests depends on the predictability (or unpredictability) of distal degenerative process. Transcranial magnetic stimulation may be able to stimulate the nerve proximal to the injury site. The hypothesis of the present study is that in cases of mild traumatic facial nerve injury where …

Prognostication of Bell's palsy using transcranial magnetic stimulation.

Transcranial magnetic stimulation (TMS) provides a method to noninvasive excitation of the facial nerve in its intracranial segment close to the internal acoustic meatus. Thus, the site of facial nerve activation with TMS is proximal to or within the site of the lesion in Bell's palsy. To evaluate the prognostic capability of TMS in unilateral Bell's palsy we examined 137 patients with this method, and compared the results with electroneuronography (ENoG). Within 0-4 days from the onset of palsy, the patients with elicitable TMS responses recovered better than those in whom TMS responses were …
