BACKGROUND: Cardiac injuries are rare in pediatric trauma patients and data regarding this type of injury is limited. There is even less data on traumatic great vessel injuries. This study sought to examine and summarize our recent experience at two pediatric trauma centers, which serve a major metropolitan area and large geographic region.
METHODS: This is a retrospective review of pediatric (<18 years) patients who sustained cardiac or great vessel injuries and were managed at a Level 1 or Level 2 pediatric trauma center between January 1, 2010 and June 30, 2020. Demographic and clinical ch

INTRODUCTION: Intentional self-harm (suicide) by firearms is a growing problem in the United States. Currently, there are no large studies that have identified risk factors for patients who die from self-inflicted gunshot wounds. Our objectives are to 1) identify risk factors for patients with the highest morbidity and mortality from self-inflicted gunshot wounds (SIGSWs) at trauma centers 2) present the outcomes of victims of SIGSW by handguns (HG) versus all other specified guns (AOG) and 3) compare the presentations and outcomes of victims with head or face (HF) injuries to other regions of

Aggressive treatment of patients with severe head injury increases the chance for survival and good functional outcome in most cases. To prevent irreversible cerebral lesions, the key point of treatment is the management of intracranial hypertension caused by intracranial hematomas, brain edema and impaired circulation of cerebrospinal fluid (CSF). Therapeutic standards are surgery of traumatic hematoma, osmotherapy and mild hyperventilation for brain edema, and CSF drainage. In highly elevated intracranial pressure (ICP) administration of barbiturates and forced hyperventilation can be consid

BACKGROUND: The rapid and accurate diagnosis of all injuries is critical in trauma surgery. Injuries not diagnosed after the secondary survey are not without serious consequences. Therefore, in an effort to decrease this problem a policy was initiated to perform an ongoing serial exam during the entire course of each patient's involvement with the trauma team at Saint Francis Medical Center.
METHODS: Prospective identification and evaluation of patients admitted to a single trauma service with delayed diagnosis was done from July 1, 1993, to October 31, 1995.
RESULTS: Sixty-eight delayed dia

The hospital records of 307 patients sustaining mandibular fractures between 1980 and 1984 were reviewed. The patient population consisted of 79% males, with precipitating events usually being fist fights (47%) and assaults with a blunt object (18%). The most common fracture involved the body (30%), followed by the angle (21%) and the condyles (19%). Intermaxillary fixation (IMF) was the preferred method of treatment (42%); however, a large number of patients were treated by internal fixation (31%) or external stabilization (11%). Despite the large number with internal fixation procedures, the

BACKGROUND: Level I and Level II trauma centers share similar clinical verification standards, but differences in volume, subspecialty integration, and transfer patterns may affect outcomes for the highest-risk injured patients.
STUDY DESIGN: We performed a retrospective cohort study using the Pennsylvania Trauma Outcomes Study database from 1999-2023. Patients aged ≥16 years treated at established Level I or Level II trauma centers with ISS >9, transfer to a trauma center, or death at a trauma center were included. Patients treated at centers that changed level, Level III/IV centers, burns,

INTRODUCTION: Domestic violence (DV) represents a significant source of suffering, particularly among women as their lifetime risk of DV is 33%, globally. Identifying DV in emergency settings remains a clinical challenge as it is not consistently screened for. The aim of this study was to assess the prevalence of DV in female trauma patients at our trauma center and to describe their injuries, in order to improve targeted care services.
METHODS: A prospective observational study from September 2021 to September 2022 included women (≥18 years) admitted to Sahlgrenska University Hospital's Trau

This is a report of a 22-year experience with penetrating cardiac trauma at a single urban Level I trauma center. We conducted a retrospective chart review supplemented by computerized patient log. Comparisons of mortality between Period 1 (1975-1985; 113 patients) and Period 2 (1986-1996; 79 patients) were by chi2 or Fisher's exact tests. Statistical significance was defined as P < or = 0.05. From 1975 to 1996, 192 patients (mean age, 32 years; 88% male) with penetrating cardiac stab wounds (68%) or gunshot wounds (32%) were treated. The most common initial clinical presentation was cardiac t

Although it is well recognized that other surgical specialties perform various procedures related to trauma care, there is a lack of analyses focusing on the role of plastic surgical management in trauma centers. This retrospective study was designed to investigate the scope of plastic surgery services in acute trauma care, using clinical data obtained from a single, regional, level I trauma center. This study included patients who presented to a single, regional, level I trauma center between March 1, 2016 and February 28, 2018. Patients with acute trauma to the facial soft tissue and skeleton

Background Gunshot wounds (GSW) contribute to poor patient outcomes and increased consumption of healthcare resources. The literature has expanded on the costs of treating patients with firearm injuries. However, further research is needed to understand the implications of GSW on healthcare resource utilization in trauma care. This study aims to compare hospital and post-hospital resource utilization between GSW and non-GSW trauma patients in the United States (US) trauma centers. Methods The National Trauma Data Bank (NTDB) was used to collect data retrospectively (2019-2021) on adult patien

BACKGROUND: Previous data suggest that there may be increased use of invasive interventions for pediatric blunt solid organ injury at nonpediatric trauma centers. In pediatric high-grade (AAST Grades III-V) blunt hepatic injuries (BHI), specifically, previous single-center and state-level studies have demonstrated that presentation to adult trauma centers independently predicts increased use of angioembolization and hemorrhage control laparotomy, but this observation has not been validated using a large, nationwide dataset. This study evaluated the use of invasive interventions (hemorrhage con

INTRODUCTION: Adolescent trauma patients are at increased risk of venous thromboembolism (VTE). It is unclear whether VTE prophylaxis practice patterns differ across trauma center types.
METHODS: The ACS-TQP database was queried for patients aged 12-17 admitted to a pediatric, adult, or mixed level I/II trauma center. VTE prophylaxis was compared between center types. Preplanned subgroup analyses were performed to evaluate guideline adherence.
RESULTS: Of 101,010 patients included, 35 % were treated at a pediatric trauma center (PTC), 43 % at a mixed trauma center (MTC), and 22 % at an ad
