PURPOSE OF REVIEW: The pathophysiology of tremor disorders is reviewed in the context of pharmacologic and surgical treatment.
RECENT FINDINGS: The anatomical origin and pathophysiologic mechanism of tremorogenic oscillation are unknown for all tremor disorders. Except for Parkinson tremor and rhythmic cortical myoclonus, the discovery of drugs for tremor disorders has been largely serendipitous. PURPOSE OF REVIEW: The purpose of this review is to explain a practical clinical approach to the diagnosis and management of tremor.
RECENT FINDINGS: A number of rare but important causes of tremor have been delineated, which means that attention to detail in clinical assessment of patients with tremor is even more important.
SUMMARY: Tremors are best divided into those occurring mainly at rest, mainly on posture, and mainly during action. This basic division leads directly to differential diagnosis and effective treatment.

Essential tremor (ET) is one of the most common movement disorders. Although often considered a monosymptomatic disorder (postural and kinetic tremor), ET has more recently been considered a more heterogeneous syndrome, with motor and nonmotor features. The diagnosis is clinical and pharmacologic and surgical therapies exist. ET is frequently misdiagnosed as Parkinson disease or dystonia. The traditional notion of ET as a benign disorder has been challenged by those who view ET as a slowly progressive neurodegenerative disorder.

The tremor present in a small number of patients who present with features of both Parkinson's disease (PD) and essential tremor (ET) is ill-defined. We therefore studied 8 such patients with 'atypical' tremor and compared them clinically to 11 PD and 10 ET patients in a double-blind, cross-over, placebo-controlled trial. A variety of hyperkinetic movement disorders has been associated with the use of neuroleptics (dopamine receptor blocking drugs), but tardive tremor has not been previously documented. Essential tremor, including the juvenile and senile variations, may be a result of a disorder of the servomechanism that controls physiologic tremor.
