Is spinal tuberculosis changing with changing time?

INTRODUCTION: Spinal tuberculosis is a chronic destructive disease with long-term morbidity. Patients are usually young especially from a poor socioeconomic background. Destruction of the intervertebral disk space and the adjacent vertebral bodies is the characteristic lesion. The dorsal spine is the most affected region with multi-level noncontiguous involvement being detected more frequently. Patients usually present with chronic back pain associated with spinal tenderness, paraplegia, spinal deformities, as well as with constitutional symptoms. Magnetic resonance imaging has proved to be mo

Challenges and controversies in the treatment of spinal tuberculosis.

Current guidelines regarding management of spinal TB are mostly extrapolated from trials on pulmonary disease. Since the British Medical Research Council (BMRC) trials in the 1970s, there are not many good quality studies that substantiate best practice guidelines for the management of this entity. Tuberculous infection of the spine behaves much differently from bacterial osteomyelitis and limited data leads to ambiguity in many cases. Although a few studies have been conducted in patients with spinal TB, most were in the era preceding short course chemotherapy and prior to current radiologica

Neurological complications of spinal tuberculosis in children.

Neurological complications of thoracic and lumbar spinal tuberculosis were studied in 32 patients under the age of 16 years. The majority had lesions involving three or more vertebral bodies. Paraplegia occurred in 8 patients and was always associated with bladder and bowel dysfunction. Lesions located at T4/5 were most commonly accompanied by paraplegia. Deterioration of the neurological status was related to the degree of spinal stenosis, whereas the degree of kyphosis was of less importance. Radiculopathy is rare in children with Pott's disease.

Early diagnosis of spinal tuberculosis by MRI.

MRI was performed at three centres in Bombay on 24 patients clinically suspected of tuberculosis of the spine but with normal radiographs. There were 11 males and 13 females and their average age was 24 years (11 to 60). 99mTc bone scans were done in 16 patients before MRI. Eleven patients had the diagnosis confirmed by biopsy and the remainder all responded rapidly to specific antituberculous chemotherapy. On T1-weighted images there was a decrease in signal intensity of the involved bone and soft tissues; on T2-weighted images there was increased signal intensity. Depending on the stage of t

Intracranial and Spinal Tuberculosis: A Rare Entity.

CNS tuberculosis has a broad spectrum of disease patterns and a high risk of complications and mortality. We present a case of a 36-year-old man who was diagnosed with neurotuberculosis with intracranial and spinal tuberculomas, meningitis, and spondylodiscitis. The patient was a known case of sarcoidosis and was being managed on corticosteroids. His presenting complaints were headache, low-grade fever, severe backache, lower limb weakness, and one episode of altered sensorium. The initial diagnosis was based on imaging findings, which were confirmed with positive cerebrospinal fluid (CSF) cul

[Progress on early diagnosis of spinal tuberculosis].

Researching on a large relevant literature of spinal tuberculosis, this paper discussed characteristics of spinal tuberculosis and its early diagnosis method. If spinal tuberculosis can be found and treated earlier, prognosis will be better. The early diagnosis of spinal tuberculosis has become hot spot at present. There are many different diagnostic methods including clinical manifestation, laboratory examination, imaging examination, etc. The newest standpoint showed that immunization has closely correlation with tuberculosis and it will become the focus and developmental direction of future

Kyphosis in spinal tuberculosis - Prevention and correction.

Spinal deformity and paraplegia/quadriplegia are the most common complications of tuberculosis (TB) of spine. TB of dorsal spine almost always produces kyphosis while cervical and lumbar spine shows reversal of lordosis to begin with followed by kyphosis. kyphosis continues to increase in adults when patients are treated nonoperatively or by surgical decompression. In children, kyphosis continues to increase even after healing of the tubercular disease. The residual, healed kyphosis on a long follow-up produces painful costopelvic impingement, reduced vital capacity and eventually respiratory

[Surgical treatment of spinal tuberculosis in aged].

OBJECTIVE: To investigate perioperative features and results of surgical treatment of spinal tuberculosis in aged. METHODS: Review the clinical data of 36 aged with spinal tuberculosis from May 1998 to June 2005 retrospectively. The average age was 70.2 years. The sites of infection included 3 cervical, 9 thoracic, 13 thoracolumbar and 11 lumbar. 28 patients suffered 1 or more complications at least and among of them, there were 18 patients have cerebral or heart vascular disease, 16 patients have diabetes mellitus. Before operation, all patients consulted with internal stuff for the proper t

The treatment of spinal tuberculosis: a retrospective study.

BACKGROUND: There are conflicting guidelines and variations in clinical practice in the management of bone tuberculosis (TB), including spinal TB. A case who received 6 months of treatment in line with current British Thoracic Society (BTS) guidelines, and subsequently relapsed, prompted a survey of treatment and outcomes of spinal and other bone TB. METHODS: A retrospective study examining the clinical features, treatment duration and outcome of patients presenting with spinal and other bone TB to the Leeds Teaching Hospitals National Health Service Trust, between 1998 and 2002. RESULTS: Fo

Behaviour of the kyphotic angle in spinal tuberculosis.

We followed 70 adult patients with spinal tuberculosis for a minimum of 2 years. Forty patients were treated by ambulant multi-drug chemotherapy (group A), and 30 with neurological complications (group B) were treated by antero-lateral decompression and chemotherapy. We studied the angle of spinal kyphosis as calculated on lateral spinal X-ray by the modified Konstam's method. The angle at final follow-up was compared with the pre-treatment angle. The relationship between the amount of initial vertebral loss, the predicted kyphotic angle and the observed kyphotic angle was analysed. Mean initi

[Anterior vertebrectomy and plating for spinal tuberculosis].

OBJECTIVE: To study the effect of anterior vertebrectomy, bone grafting and plating in the treatment of spinal tuberculosis. METHODS: Between February 1998 and August 2000, 12 patients with spinal tuberculosis were operated with anterior vertebrectomy, bone grafting and plating. Evaluation including bone fusion rate, correction of deformity and status of spinal lesion was carried out. RESULTS: In an average of ten months, spinal tuberculosis was completely cured and solid bone fusion had taken place in all 12 patients. The time for bone fusion averaged 5.8 months and the average angle of cor

Anterior instrumentation for the treatment of spinal tuberculosis.

BACKGROUND: Kyphosis and neurological impairment are the major residual problems of spinal tuberculosis after the microorganism has been eradicated with use of appropriate medications. Spinal instrumentation is needed to support anterior strut grafts in patients who have kyphosis that affects more than two levels. Most surgeons use posterior instrumentation. Anterior instrumentation, despite its advantages, has not been widely accepted, partly because of concerns about introducing foreign material into infected tissue. The purpose of the current study was to address those concerns. METHODS: T

[Spinal tuberculosis with vertebral instability: comprehensive approach].

Tuberculosis with simultaneous pulmonary, vertebral, and cerebral involvement is observed episodically in HIV-negative patients, thus requiring evaluation of the entire cerebrospinal axis and a multidisciplinary approach. We report the case of a 54-year-old man who presented with a three-month history of productive cough, constitutional symptoms, dyspnea, and lower back pain. Bilateral pulmonary cavitary lesions, pleural thickening, dorsal spondylitis (T10-T11) with fracture, compression, and edema of these vertebral bodies, paravertebral fluid collections partially compressing the spinal cord
