Presently, more and more patients are being offered a minimally invasive surgical procedure for the management of chronic duodenal ulcer disease. Although previous authors have described a variety of laparoscopic selective vagotomy operations for this disease, there remains an important role for truncal vagotomy in selected patients. The use of H2 blockers has led to a virtual demise of elective surgery for the treatment of duodenal ulcers. Some ulcers, however, are resistant to medications, and some patients are weary of continuous medication in the long term. Laparoscopic vagotomy is a promising new form of surgical treatment that could offer an alternative to long-term medication and major open abdominal surgery. Prospective well-documented and detailed studies are needed, however, before an objective evaluation of this procedure can be made.

We performed bilateral truncal vagotomy and gastric drainage procedure using standard laparoscopic instruments in five mongrel dogs. The procedure consisted of a transthoracic thoracoscopic bilateral truncal vagotomy and transperitoneal laparoscopic pyloromyotomy. A contact Nd:YAG laser fiber was used. The authors studied the clinical aspects, indices of gastric secretion and basal level blood serum insulin, gastrin and parathormone in 83 patients with ulcer disease recurrence after vagotomy. All patients showed an active acidopeptic factor--sequel of ineffective and incomplete (in some patients) vagotomy as well as inadequate pyloroplasty. This is a report of a prospective randomized study of 3 operations for duodenal ulcer; parietal-cell vagotomy alone (without drainage), selective vagotomy with antrectomy, and truncal vagotomy with antrectomy. Osteomalacia, due to defective absorption of adequate dietary vitamin D, has been found in a 79-year-old woman, 5 years after an anterior selective vagotomy and pyloroplasty. The evolution of the recognition of postgastrectomy osteomalacia is reviewed, and reasons are advanced why similar problems are to be expected following vagotomy and pyloroplasty.

In Leeds and Copenhagen 271 patients were treated electively for duodenal ulcer by parietal cell vagotomy without drainage between 1969 and 1972 inclusive, with no operative deaths. 108 patients have been followed up 2-4 years since operation. Gastric stasis necessitating re-operation occurred in only 2 cases. 32 patients with complicated duodenal ulcer (bleeding, recurrence, perforation) were treated by laparoscopic posterior truncal vagotomy and anterior linear gastric resection in our modification (access to the posterior vagus via the omental bursa) of the method of Gomez-Ferrer/Taylor. Instead of the seromyotomy along the lesser curve we resect a 1 cm broad strip out of the anterior gastric wall. The complications and sequelae of truncal vagotomy have been overemphasized in the surgical literature. With the introduction of minimally invasive techniques, the indications for vagotomy have to be reconsidered. The procedure can be performed either by laparoscopy or by thoracoscopy. The techniques for these approaches are outlined. In an experience of twenty-one vagotomies no major complication occurred, and the immediate results are fair.
