The purpose of this study was to evaluate the variance of apoptosis in rats in which experimental varicocele was induced and then treated by varicocelectomy. Forty adult male Wistar albino rats were used in this experimental study. Experimental varicocele was created in 30 rats. BACKGROUND: Varicocele is the most common correctable cause of male infertility. It was the subject of recent Association française d'urologie (AFU) Comité d'andrologie et de médecine sexuelle (CAMS) recommendations. Since then, the literature has provided additional information. This review will comprehensively reassess current indications for the treatment of varicocele, and revisit contemporary issues in the light of current advances.

OBJECTIVE: To evaluate the prevalence of varicocele in the left and right spermatic veins in infertile men by several methods of examination.
DESIGN: Prospective study.
SETTING: Andrology unit of a department of obstetrics and gynecology, and interventional radiology unit of the radiology department at a tertiary care facility.

The effect of the treatment of varicocele on symptoms and fertility was followed up in 66 patients for an average of 27 months (range 6-84 months). The patients had been treated either surgically (n = 45) or with embolization (n = 21). Herein we report our experience in the treatment of varicocele by endoscopic surgery. After a 5 mm. transverse incision has been made immediately below the umbilicus, air is insufflated and a trocar is introduced through the incision. We then introduce the optics through the trocar and a similar incision is made 4 or 5 cm. above the internal orifice of the left inguinal duct. This study was conducted to evaluate morphometrically the microscopic components of the testis in cases of varicocele, the leading cause of male infertility, with the aim to identify consistent differences from controls. In our specimens of patients with varicocele, the vascular and interstitial fractions, as well as the Leydig cell count, were significantly increased. Our data support the notion that the interstitium is the testicular compartment most consistently affected and that the damage to the tubules is erratic and probably late.

A study was made of 160 cases of left varicocele treated during 1979-1980; there were 35 patients under sixteen years old. Also 74.3 per cent of patients already had mono- or bilateral testicular hypotrophy. Testicular biopsy revealed 90 per cent had histologic lesions of varying nature and degree. We believe varicocele formation is related to physiologic changes during puberty. These data suggest that early recognition of varicocele and its correction before the appearance of irreversible testicular lesions may prevent male sterility.

The results of selective phlebography of the spermatic vein are reported in 580 infertile men with abnormal sperm morphology and suspected varicoceles. Percutaneous transvenous retrograde sclerotherapy of the internal spermatic vein was performed in 260 of the patients. Follow-up phlebography in 109 patients 3 to 6 months after treatment revealed a persistent varicocele in only 3% of them.
