[Childhood vasculitis].

BACKGROUND: The primary vasculitides are rare conditions in childhood. The most common disease subtypes are Schönlein-Henoch purpura and Kawasaki's syndrome, which frequently have a self-limiting course. In the majority of vasculitides, the etiology remains unknown. Environmental exposure, including infections, is suspected to trigger an autoinflammatory response in predisposed individuals. GOAL: The aim of this review is to present the various aspects of childhood vasculitis. MATERIALS AND METHODS: Reviews and special original papers on childhood vasculitis, published classification criteri…

Pediatric vasculitis.

Childhood vasculitis is a challenging and complex group of conditions that are multisystem in nature and often require integrated care from multiple subspecialties, including rheumatology, dermatology, cardiology, nephrology, neurology, and gastroenterology. Vasculitis is defined as the presence of inflammation in the blood vessel wall. The site of vessel involvement, size of the affected vessels, extent of vascular injury, and underlying pathology determine the disease phenotype and severity. This article explores the classification and general features of pediatric vasculitis, as well as the…

[Pulmonary vasculitis].

Following an international consensus conference held in Chapel Hill, North Carolina, USA, vasculitides have been divided based on the size of the vessel involved in the inflammatory process. Thus, the terms large, medium and small vessel vasculitis emerged. Wegener's granulomatosis, microscopic polyangitis and Churg-Strauss syndrome are described as small vessel vasculitides. Pulmonary involvement is a characteristic feature of both Wegener's granulomatosis and Churg-Strauss syndrome, less so in microscopic polyangitis. A pathognomonic feature is alveolar haemorrhage, which encompasses the tri…

[Leukocytoclastic vasculitis].

Leukocytoclastic vasculitis (LcV) is the most common form of cutaneous vasculitis. Often LcV results from deposition of immune complexes in the vascular wall. When IgA is the dominant immunoglobulin in these complexes, systemic involvement is likely (Henoch-Schönlein purpura), being more severe in adults. LcV in which immune complexes are composed of IgG or IgM are more often limited to the skin and may additionally show minor systemic involvement. In other forms of LcV additional pathophysiological factors play a role. LcV can also be a presenting or accompanying symptom of severe systemic AN…

Granulomatous vasculitis.

Granulomatous vasculitis is a subset of systemic necrotizing vasculitis and has granulomatous inflammation as the main histopathologic feature. Etiopathogenesis remains poorly understood, although recent advances suggest an important role for certain pro-inflammatory cytokines, such as tumor necrosis factor-alpha. They are a heterogeneous group of clinical disorders with protean manifestations. Serologic abnormalities are present, and the presence of granular cytoplasmic staining-antineutrophil cytoplasmic antibodies is most important and is particularly useful for the diagnosis of active Wege…

Urticarial vasculitis.

Urticarial vasculitis is a clinico-pathologic entity typified by recurrent episodes of urticaria that have the histopathologic features of leukocytoclastic vasculitis. The cutaneous features may include painful, burning or pruritic skin lesions, the persistence of individual lesions greater than 24 hours, palpable purpura, pronounced central clearing of lesions, and residual hyperpigmentation following resolution. However, because clinical characteristics of urticarial vasculitis may overlap with those of allergic urticaria, confirmation of the diagnosis requires a lesional skin biopsy. This c…

Localized vasculitis.

The mechanisms of vasculitis are poorly understood, but involve immune-mediated destruction of vessel walls. Depending on the syndrome, there is significant variability in the size and types of vessels involved, as well as the nature of the inflammatory infiltrate. In addition, there is a wide variation from 1 patient to another in the extent of involvement throughout the vascular tree. In some forms of vasculitis that are histologically indistinguishable from systemic syndromes, the inflammatory process appears to be isolated, or localized to a single site or organ. In most such cases, especi…

Urticarial vasculitis.

Chronic or recurrent urticarial lesions are common in both primary care and referral medicine. Diagnosis and treatment are usually a challenge for both the patient and the medical practitioner. Most patients are eventually diagnosed with chronic idiopathic urticaria. IgG autoantibody to IgE receptor or IgE itself causes urticarial lesions in 30% of these patients. Only a minority (approximately 10%) of patients with chronic urticarial lesions have urticarial vasculitis. Although some cases are benign, urticarial vasculitis by itself can cause significant morbidity, and it is often a manifestat…

Childhood vasculitis.

Vasculitis can and does occur in childhood. Apart from the relatively common vasculitides (Henoch-Schönlein purpura, Kawasaki disease and in world wide terms Takayasu disease) there are a number of important but comparatively rare disorders affecting children. These include macroscopic and microscopic polyarteritis, cutaneous polyarteritis, Wegener's granulomatosis, Churg-Strauss syndrome, primary angiitis of the central nervous system, hypersensitivity angiitis, hypocomplimentaemic urticarial vasculitis, vasculitis associated with various connective tissue disorders and vasculitis associated …

Renal vasculitis.

The term 'vasculitides' encompasses a group of inflammatory disorders which may affect the kidney by damaging its blood supply. Although the kidney may be affected by many types of systemic vasculitis, renal involvement is particularly frequent in some forms of systemic necrotizing vasculitis which are considered primary, such as Wegener's granulomatosis and microscopic polyarteritis, including its 'renal-limited' variant. Renal vasculitis can occur at any age, but is seen particularly frequently in middle-aged and elderly subjects, in whom clinical presentation and prognosis are significantly…

Granulomatous vasculitis.

Vasculitides are classified by the size of the vessel involved and by the nature of the inflammatory process. Pulmonary granulomatous vasculitis encompasses several entities that are in general characterized by granulomatous inflammation, extensive necrosis, and a variegated cellular infiltrate. Wegener's granulomatosis is a prototype of granulomatous vasculitis and is a disease of unknown etiology that often involves the upper respiratory tract, the lower respiratory tract, and the kidneys. Some of the entities initially classified as pulmonary granulomatous vasculitis have subsequently been …

[Necrotizing vasculitis].

We present a boy who had been under observation for 4 years, suffering since the age of nine from necrotizing vasculitis, which appeared during Proteus sp. sepsis. Complex treatment with antibiotics, large doses of steroids, antithrombotic and peripheral circulation improving drugs, as well as local drugs applied to the skin and oral cavity mucosa, resulted in a complete cure. This illness left some cosmetic skin defects on the face and extremities, a shortening of the tongue and malocclusion.

Cutaneous vasculitis.

Cutaneous vasculitis and vasculopathic processes continue to be difficult to define, diagnose, and treat. Historically, the complexity of these disorders has been compounded by imprecision in terminology and classification and the presence or absence of underlying systemic illness. Approaching the literature with consistent diagnostic constraints and accepted terminology can, it is hoped, eliminate some of the ambiguity. During the past year, several case reports and brief communications regarding cutaneous vasculitis or vasculopathies have appeared, as well as thoughtful basic science reports…
