The influence of the calcium blocker verapamil (VR) on systemic toxicity resulting from the intravesical instillation of Adriamycin (ADM) and thiotepa (THT) was assessed in mice. Eighty per cent of the animals receiving THT + VR developed a generalized alopecia. Data gathered at necroscopy failed to reveal any trauma to the major organs or the presence of a drug-induced myelosuppression. The effect of verapamil on the pharmacokinetics of digoxin was studied in 49 patients with chronic atrial fibrillation. A dose of 240 mg/day of verapamil was given to the patients who were receiving a stable dose of digoxin. Verapamil is a new anti-anginal and anti-arrhythmic drug whose pharmacological actions can be related to its specific blocking effect on the transmembrane transport of calcium ions. Intravenous verapamil slows the ventricular rate in atrial fibrillation and flutter with an occasional patient reverting to sinus rhythm. Verapamil has been shown to be effective in the management of supraventricular tachycardia (SVT). However, the utility of verapamil may be limited because of adverse effects, specifically hypotension. Several clinical observations have shown that the intravenous administration of calcium is effective in reversing the myocardial depressant effects of verapamil. We report the case of a patient with SVT and a systolic blood pressure of 80 mm Hg in which the administration of calcium chloride prior to that of verapamil may have negated verapamil-induced hypotension.
