Ultrastructure of lung in Legionnaires' disease. Observations of three biopsies done during the Vermont epidemic.

Open lung biopsies from three patients with Legionnaires' disease were examined by light and transmission electron microscopy. The patients had serious underlying disease. All developed a rapidly progressive pneumonia unresponsive to penicillin, oxacillin, and gentamicin. One patient, who received erythromycin, survived. Light microscopy in all three showed severe acute bronchopneumonia. The Legionnaires' disease bacterium was seen in tissue sections and confirmed by direct immunofluorescence. Transmission electron microscopy showed numerous rod-shaped intracellular organisms that were morphol

Neurodevelopmental outcome of extremely low birth weight infants from the Vermont Oxford network: 1998-2003.

BACKGROUND: Physicians and parents face significant uncertainties when making care decisions for extremely low birth weight (ELBW) infants. Many published estimates of death and developmental outcome are from well-funded university programs and may not reflect outcomes of infants from a variety of settings. The best estimates of the probabilities of death and severe disability combine local experience and published data. OBJECTIVE: To describe the neurodevelopmental outcome of ELBW infants from centers of the ELBW Infant Follow-Up Group of the Vermont Oxford Network (VON) and to identify char…

Statewide Trends in Buprenorphine Prescribing in Rural and Nonrural Vermont: Analysis of Population-based Patient Pharmacy Claims.

IMPORTANCE: Opioid-related mortality rates have risen dramatically over the past decade, and office-based opioid treatment using buprenorphine offers hope for combatting this trend. Vermont's policymakers, health care systems, and treatment providers have worked to expand access to treatment throughout the rural state. OBJECTIVE: The objective of the current study was to characterize the trends in the number of buprenorphine prescribers and the number of patients per prescriber in Vermont over the past decade (2010-2020). METHODS: We used Vermont's all-payer claims database to identify patie…

Finding crime in a public health crisis: a comparative analysis of opioid news coverage in West Virginia and Vermont.

BACKGROUND: Under what conditions do newspapers reference crime or law enforcement in their coverage of opioids in the United States? This study examines how crime framing varies across political contexts, time periods, and drug types in newspaper coverage of opioids. METHODS: The study analyzed 2,400 newspaper headlines, including 1,200 from Vermont and 1,200 from West Virginia. For each state, half the headlines were identified using the search term "oxycodone" and half using "fentanyl." Headlines were coded for the presence of crime frames. RESULTS: Bivariate analyses showed that West Vir…

Support for a Weight-Inclusive Curriculum? Exploring Partner Perspectives and Influences on Nutrition Education in Vermont High Schools.

BACKGROUND: The weight-normative approach to nutrition education dominates health education programming across the United States, despite evidence that this paradigm contributes to negative outcomes including weight cycling, bias and stigma, the development of disordered eating behaviors, and weight-based bullying. METHODS: This study investigates perspectives of 10 potential partners with interest in and potential to influence nutrition education. Through qualitative interviews and document analysis, researchers explored support for a weight-inclusive curriculum and factors that influence hi

Patient perceptions of treatment with medication treatment for opioid use disorder (MOUD) in the Vermont hub-and-spoke system.

In 2013, Vermont leaders implemented the "hub-and-spoke" (H & S) system to increase access to medication treatment for opioid use disorder (MOUD). "Hubs" are licensed specialty opioid treatment programs (OTPs) with the authority to dispense buprenorphine/naloxone and methadone. "Spokes" are primary care practices that provide office-based opioid treatment, primarily with buprenorphine/naloxone. This report describes the qualitative component of an evaluation of the H&S system, conducted in 2016. The qualitative data collection assessed patient perspectives about the positive and negative aspec…

Adjustment for case mix in comparisons of cesarean delivery rates: university versus community hospitals in Vermont.

OBJECTIVE: Our objective was to determine whether case mix model adjustment would help to explain differences in cesarean delivery rates between community and university hospitals. We also wished to define a patient population in which the cesarean delivery rate would be more reflective of individual practice patterns than of obstetric or medical risk. STUDY DESIGN: Established risk factors for cesarean delivery were identified by retrospective chart review at two community hospitals (designated A and B) and a university hospital. Each delivery was assigned exclusively to 1 of 6 risk categori

Serological evidence for eastern equine encephalitis virus activity in white-tailed deer, Odocoileus virginianus, in Vermont, 2010.

Serum samples from 489 free-ranging white-tailed deer (Odocoileus virginianus) were screened for antibodies against the Eastern equine encephalitis virus (EEEV) using plaque reduction neutralization tests (PRNTs). EEEV antibodies were detected in 10.2% of serum samples. This is the first evidence that EEEV is present in Vermont. Serum was collected from deer in all 14 counties in the state, and positive EEEV sera were found in 12 (85%) of 14 counties, suggesting statewide EEEV activity in Vermont. Analysis of the spatial distribution of PRNT-positive samples revealed a random distribution of E…

Experiences participating in federal nutrition assistance programs during the early months of the COVID-19 pandemic: an investigation in Vermont.

BACKGROUND: Federal nutrition assistance programs serve as safety nets for many American households, and participation has been linked to increased food security and, in some instances, improved diet quality and mental health outcomes. The COVID-19 pandemic brought new and increased economic, social, and psychological challenges, necessitating inquiry into how nutrition assistance programs are functioning and associated with public health outcomes. METHODS: Using data from a representative statewide survey administered in Vermont (n = 600) between July and September 2020, we examined particip…
