A Classification System for the Spread of Polymethyl Methacrylate in Vertebral Bodies Treated with Vertebral Augmentation.

In this study, we develop a classification system for describing polymethyl methacrylate (PMMA) spread in vertebral bodies after kyphoplasty or vertebroplasty for vertebral compression fractures (VCFs) and for assessing whether PMMA spread varies between operators, VCF etiology, or vertebral level. Intraoperative fluoroscopic images of 198 vertebral levels were reviewed in 137 patients (women, 84; men, 53; mean age, 75.8 ± 12.5; and those with a diagnosis of osteoporosis, 63%) treated with kyphoplasty between January 01, 2015 and May 31, 2015 at a single center to create a 5-class descriptive

[Advances on the study of movable artificial vertebral body].

At present, artificial vertebral implants have proven to be effective in the treatment of spinal tumors, infections, fractures and other diseases. However, the fusion artificial vertebral body can cause adjacent intervertebral joint degeneration and loss of original physiological curvature and activity. The movable artificial vertebral body can, to some extent, restore the normal physiological movement and reduce biomechanical changes of the spine, reducing the occurrence of complication. The design of movable artificial vertebral body is to equip movable device when the basis of reliable stab…

Thoracoabdominal aneurysms and changes in adjacent vertebral bodies.

AIM: The aim of this study was to assess if chronic intermittent pressure of a thoracoabdominal aortic aneurysm (TAAA) induces structural changes in vertebral bodies and if eroded vertebral bones can still be found after the extermination of syphilis. METHODS: A retrospective analysis of computed tomography (CT) scans of patients with TAAA was performed. In the anatomical regions were the TAAA was in close contact with the vertebral bodies, the vertebral body alteration was distinguished into 4 categories. Category 0: no changes; 1: discrete changes, minimal asymmetry; 2: obvious asymmetry of…

A kind of specific osteolytic destruction of the vertebral bodies.

This report describes two young patients with osteolytic destruction in two adjacent vertebral bodies along with the intervertebral disc, and reveals its possible mechanism. A lateral radiograph and CT scan displayed a giant osteolytic cavity in the L4 vertebral body. An MRI or CT scan with a two-dimensional reconstruction displayed the same changes in the L4 vertebral body and lower endplate erosion in the L3 vertebral body. A comprehensive preoperative evaluation did not identify a specific cause of vertebral destruction. Both patients underwent anterior lumbar fusion surgery. The lesions we…

Micromechanics of the human vertebral body for forward flexion.

To provide mechanistic insight into the etiology of osteoporotic wedge fractures, we investigated the spatial distribution of tissue at the highest risk of initial failure within the human vertebral body for both forward flexion and uniform compression loading conditions. Micro-CT-based linear elastic finite element analysis was used to virtually load 22 human T9 vertebral bodies in either 5° of forward flexion or uniform compression; we also ran analyses replacing the simulated compliant disc (E=8 MPa) with stiff polymethylmethacrylate (PMMA, E=2500 MPa). As expected, we found that, compared …

Vertebroplasty in the treatment of osteoporotic vertebral body fracture.

During the last few years, vertebroplasty has gained a wide acceptance for the treatment of painful osteoporotic vertebral body fractures and osteolytic changes. However, new guidelines with significant changes in indications and technique were published recently in Europe. Therefore, the aim of this review is to highlight recent changes in indications for vertebroplasty, patient work-up and changes in procedural technique, and to give an overview of patient outcome and possible complications. Therefore, technical details like different types of fluoroscopy, needle placement, pain management d…

A histologic study of fractured human vertebral bodies.

Twenty-seven fractured human vertebral bodies and 24 unfractured human vertebrae from adjacent levels were studied postmortem using histolog … l bodies from individuals without radiographic evidence of osteoporosis. A vascular necrosis of the vertebral body is a common histologic finding in individuals with osteoporosis. Indeed, our histologic observations suggest subclinical fractures and microfractures of the vertebral body may be the underlying pathologic process leading to avascular necrosis in ind …

Volumetric determination of normal and scoliotic vertebral bodies.

A new method is presented for stereological evaluation of the volume of the vertebral body in vivo. The height of the vertebral body is measured at three standardised points on an anteroposterior radiograph and at two other points on a lateral one. The area of the body is also measured using a special grid superimposed on a CT scan from the middle part of the vertebra. The volume of the vertebral body is then calculated using Cavalieri's principle for irregular objects: V = delta a x H, where V is the volume of the vertebral body, delta a is the mean cross-section surface area on the CT scan a…

The scanning electron microscopy of compressed vertebral bodies.

Scanning electron microscopy was used in this study to determine the effects of compression at different rates on the structure of trabecular bone in primate vertebral bodies. Rhesus monkey vertebral bodies were subjected to compressive loads at two different strain rates (8.89 x 10(-1) m/sec and 8.89 x 10(-5) m/sec). Each vertebral body was compressed to 50% of its original height. S-shaped bending of trabeculae was present in bodies compressed at the lower strain rate, while fractures were more predominant at the higher strain rate. The percentage of recovered height following compression wa…

Significance of resegmentation in the pathogenesis of vertebral body malformation.

Human embryos and fetuses were investigated histologically to elucidate the possible pathogenesis of congenital malformations of the vertebral body. Special attention was paid to early development of the vertebral column, particularly the often-questioned stage of resegmentation, important as the stage during which the malformations might develop. Evidence of resegmentation was found in 7 embryos ranging from 7 mm C-R length (5 weeks after fertilization) to 13 mm (6 weeks). Moreover, the formation of the definitive vertebral body was considered to be intimately related to the intersegmental ar…

[DOES KYPHOPLASTY AFFECT THE SIZE OF THE VERTEBRAL BODY?].

INTRODUCTION: Thoracolumbar compression fractures are common among the osteoporotic aging population, and a commonly accepted surgical treatment is kyphoplasty. AIMS: Our study aims to demonstrate the change in the known radiologic evaluation methods before and after the surgery. METHODS: A retrospective study, 54 patients with compression A1 fractures were included and evaluated for demographic data. Two observers independently collected data from preoperative, intra-operative, and postoperative lateral and AP radiographs. The collected data included middle vertebral height - MVH/U/D, perce

Outcomes of vertebral body tethering in the lumbar spine.

PURPOSE: The use of vertebral body tethering (VBT) for the treatment of main thoracic deformities in adolescent idiopathic scoliosis patients is becoming increasingly more common, but limited data exist on its safety and efficacy in thoracolumbar deformities. We aimed to evaluate the postoperative outcomes of patients with thoracolumbar (TL) deformities that were treated with VBT. METHODS: We assessed clinical and radiographic data from twenty-eight consecutive patients that were surgically managed with VBT, all of whom exhibited a TL deformity with at least two years (mean: 44.7 ± 14.5 month…

A revised metric for quantifying body shape in vertebrates.

Vertebrates exhibit tremendous diversity in body shape, though quantifying this variation has been challenging. In the past, researchers have used simplified metrics that either describe overall shape but reveal little about its anatomical basis or that characterize only a subset of the morphological features that contribute to shape variation. Here, we present a revised metric of body shape, the vertebrate shape index (VSI), which combines the four primary morphological components that lead to shape diversity in vertebrates: head shape, length of the second major body axis (depth or width), a

Effect of vertebroplasty on the compressive strength of vertebral bodies.

BACKGROUND CONTEXT: Percutaneous vertebroplasty has been used successfully for many years in the treatment of painful compressive vertebral … olymethylmethacrylate (PMMA) with barium (8 g of barium/40 g of PMMA) was injected into every other vertebral body of each spine specimen. After vertebroplasty, all soft tissues were dissected from the spine, and the vertebral bodies were separated and potted for mechanical testing. Testing to failure was performed using a combination of axial compression and an …

[Replacement of the vertebral body with an expansion implant (Synex)].

PURPOSE OF THE STUDY: This paper describes replacement of the vertebral body with the expansion implant Synex. Usually, autologous bone graft is used to replace the vertebral body. In patients with bone cancer or multiple injuries to the spine, cement filling is preferred whereas, in other indicated cases, implants are inserted, of which Harms' titanium cage has been the most common one. However, this needs filling with a large amount of bone tissue and it is often difficult to adjust its size into the space available. Telescopic devices, on the other hand, are easier to implant and their appl…
