Hyponatremia is a common complication among cancer patients. Certain antineoplastic agents have been associated with syndrome of inappropriate secretion of anti-diuretic hormone-induced hyponatremia. The most common agents associated with secretion of anti-diuretic hormone are vinca alkaloids, platinum compounds, and alkylating agents. We report a case of secretion of anti-diuretic hormone associated with tyrosine kinase inhibitors.

The advantages and limitations, theory, and factors in the use of electric current to deliver drugs across the skin are discussed. Application of the technique in the delivery of local anaesthetics, antibiotics/antibacterials, mydriatics, vinca alkaloids, non-electrolytes, biogenic amines, peptides, and other agents is reviewed. It is concluded that the method alone, or in combination with phonophoresis (delivery via ultrasonic perturbation), offers considerable scope in the transdermal delivery of drugs.

The enzymatic basis for quinine 1 biosynthesis was investigated. Transcriptomic data from the producing plant led to the discovery of three enzymes involved in the early and late steps of the pathway. A medium-chain alcohol dehydrogenase (CpDCS) and an esterase (CpDCE) yielded the biosynthetic intermediate dihydrocorynantheal 2 from strictosidine aglycone 3. Additionally, the discovery of an O-methyltransferase specific for 6'-hydroxycinchoninone 4 suggested the final step order to be cinchoninone 16/17 hydroxylation, methylation, and keto-reduction.

We present an elderly patient with metastatic breast cancer and Parkinson's disease (PD) of moderate severity who developed paralytic ileus of protracted course after a low dose of 0.16 mg/m(2) intravenous vincristine. Since colonic myenteric plexus involvement occurs in PD, we suggest that it may predispose to vinca alkaloids-induced autonomic neuropathy. Vincristine should be used with caution in this clinical setup.

Vinorelbine (NVB) is a novel vinca alkaloid FDA approved for use in some advanced carcinomas. However, its role in non-Hodgkin's lymphoma (NHL) is still not well defined. NVB is an antimicrotubule agent, but as yet, it is not known whether it induces apoptosis. Cardiac toxicity following the administration of chemotherapeutic agents is well documented. Vinca alkaloids, as well as high-dose cyclophosphamide, have been associated with myocardial ischemia. The present report describes a case of acute myocardial infarction occurring in a patient with no antecedent cardiac history who received both vincristine and conventional chemotherapeutic doses of cyclophosphamide for the treatment of chronic lymphocytic leukemia. Physicians should possess a heightened awareness of this potentially serious complication.

This article describes the reaction of vindoline with formaldehyde and trimethyl orthoformate to prepare vindolicine, tris-vindolicinyl methane and higher molecular weight homologues. The antitumour effect of vindesine (desacetyl vinvlastine amide sulfate) in disseminated malignant melanoma has been investigated in our oncology service in a phase-II study. Of 25 patients exposed to the drug, 23 were evaluable, and seven of these (30%) were considered objective responders. The observed response of malignant effusions is of particular interest. Side effects related to vindesine are similar to those of the other vinca alkaloids, although alopecia may be more pronounced with the doses used in this study.

For peripheral neuropathy, standards of care are based on established evidence-based practice. Peripheral neuropathy is disease or dysfunction of one or more peripheral nerves (i.e., motor, sensory, or autonomic), resulting in numbness or weakness. Peripheral neuropathy occurs outside of the brain and spinal cord and is caused by cancer, treatment, or both. Chemotherapeutic agents that can cause peripheral neuropathy include epothilones, platinum analogs, taxanes, and vinca alkaloids, as well as immunomodulating drugs and proteasome inhibitors.

Five consecutive patients with progressive extracutaneous stage IV mycosis fungoides (MF) were treated with VM 25, 100 mg/m2 i.v., for at least 3 cycles. All patients had been extensively pretreated and in particular with vinca alkaloids. Two partial responses of 5 and 9+ months duration and 1 minimal response of 5 months duration were obtained. Transient myelosuppression was encountered in all patients. These preliminary results should encourage further exploration of VM 26, even in less advanced MF.

Liver dysfunction in patients with lymphoma presents an important clinical dilemma because it limits the use of anthracyclines and vinca alkaloids for this potentially curable malignancy. Patients are typically treated with corticosteroids, rituximab and the nitrogen mustard mechlorethamine, and then transitioned to definitive therapy after improvement in liver function. Three separate in vitro admixture experiments were accomplished with bleomycin and other common parenteral medications. Bleomycin was assayed by a radioimmunoassay procedure.
