Walking the Benzodiazepine High Wire.

The number of benzodiazepine (BZD) prescriptions has substantially increased over the past decade, leading to a parallel rise in rates of misuse and overdose. These increases have prompted the U.S. Food and Drug Administration to update its "boxed warning" and have caused organizations to revisit prescribing guidelines. Concurrently, strong evidence from clinical trials supports the anxiolytic efficacy of BZDs. Although antidepressants and psychotherapy remain preferred treatments for chronic anxiety, BZDs remain helpful in treating acute or severe and persistent anxiety that does not respond

Persistent toe walking in autism.

The records of 954 ambulatory children presenting for initial evaluation to a university developmental pediatrician were reviewed for the prevalence of persistent toe walking and associated tight heel cords. The incidence of persistent toe walking (20.1%) and tight heel cords (12.0%) were found to be higher in 324 children with an autistic spectrum disorder but lower (10.0%/3.0%) in 30 children with Asperger syndrome. These results confirm the previously reported high incidence of toe walking in children with autism and with language disorders and also raise the possibility of a secondary orth…

Backward walking in Parkinson's disease.

We walk backward on a daily basis, such as when backing away from the kitchen sink or stepping back from a curb as a swiftly moving bus passes. This task may be particularly difficult for individuals with Parkinson's disease (PD) who often fall as a result of moving or being perturbed in the backward direction. The aim of this study was to assess backward walking (BW) in individuals with PD. Both forward walking (FW) and BW were assessed in 78 people with idiopathic PD (H&Y range: 0.5-3) in the ON state, and 74 age- and sex-matched controls. In FW, those with PD had significantly shorter strid…

Cristospinal reflex in circular walking.

This study comprised 22 normal adults, 12 males and 10 females. Electromyograms of the bilateral gastrocnemius muscle were recorded during circular walking under two conditions: no caloric stimulation and immediately after irrigating the right auditory canal with 20 degrees C water for 20 s with the horizontal semicircular canal maintained in vertical position. With no caloric stimulation, gastrocnemius muscle activity on the right side exceeded that on the left during dextrorotation and vice versa during sinistrorotation. Following irrigation of the right auditory canal, the activity was grea…

Mechanical and metabolic consequences of sagittal trunk lean angle in walking - a dynamic walking perspective.

Bipedal walking requires a balance of muscle work and energy losses, with models indicating that powering gait with ankle push-off is more energetically economical than powering with hip joint work. This study investigates how varying trunk lean angle affects joint mechanics and energy expenditure during walking. We hypothesized that leaning forward would increase hip work, reduce ankle push-off and increase energy consumption, and leaning backward would have complementary effects. Healthy young adults walked at 1.3 m s-1 while adjusting their trunk angles from backward 15 deg to forward 60 de…

Electrical Brain Activity during Human Walking with Parametric Variations in Terrain Unevenness and Walking Speed.

Mobile brain imaging with high-density electroencephalography (EEG) can provide insight into the cortical processes involved in complex human walking tasks. While uneven terrain is common in the natural environment and poses challenges to human balance control, there is limited understanding of the supraspinal processes involved with traversing uneven terrain. The primary objective of this study was to quantify electrocortical activity related to parametric variations in terrain unevenness for neurotypical young adults. We used high-density EEG to measure brain activity when thirty-two young a…

Beam walking can detect differences in walking balance proficiency across a range of sensorimotor abilities.

The ability to quantify differences in walking balance proficiency is critical to curbing the rising health and financial costs of falls. Current laboratory-based approaches typically focus on successful recovery of balance while clinical instruments often pose little difficulty for all but the most impaired patients. Rarely do they test motor behaviors of sufficient difficulty to evoke failures in balance control limiting their ability to quantify balance proficiency. Our objective was to test whether a simple beam-walking task could quantify differences in walking balance proficiency across …

To walk or not to walk: insights from a qualitative description study with women suffering from fibromyalgia.

Walking improves health outcomes in fibromyalgia; however, there is low adherence to this practice. The aim of this research was to explore the beliefs of women suffering from fibromyalgia toward walking, and the meaning that they attribute to the behavior of walking as part of their fibromyalgia treatment. This study is a qualitative description research. Forty-six (46) women suffering from fibromyalgia and associated with local fibromyalgia associations located in four different Spanish cities (Elche, Alicante, Madrid, and Talavera de la Reina) participated in focus group discussions in the …

Characteristics of Walking Group Leaders as Compared With Walking Group Members in a Community-Based Study.

BACKGROUND: Walking interventions delivered by lay leaders have been shown to be effective. Knowing the characteristics of individuals who volunteer to be group leaders in walking programs could facilitate more efficient and effective recruitment and training. METHODS: Walking group leaders were recruited into a community-based program and formed walking groups from existing social networks. Leaders and members completed a survey, participated in physical measurements, and wore an accelerometer. Regression models (adjusting for group clustering and covariates) tested psychosocial and behavior…

Psychometric performance of a generic walking scale (Walk-12G) in multiple sclerosis and Parkinson's disease.

Walking difficulties are common in neurological and other disorders, as well as among the elderly. There is a need for reliable and valid instruments for measuring walking difficulties in everyday life since existing gait tests are clinician rated and focus on situation specific capacity. The Walk-12G was adapted from the 12-item multiple sclerosis walking scale as a generic patient-reported rating scale for walking difficulties in everyday life. The aim of this study is to examine the psychometric properties of the Walk-12G in people with multiple sclerosis (MS) and Parkinson's disease (PD). …

Walking capacity evaluated by the 6-minute walk test in spinal and bulbar muscular atrophy.

Spinal and bulbar muscular atrophy (SBMA) is an adult-onset motor neuron disease caused by a CAG repeat expansion in the androgen receptor gene. Because the progression of SBMA is slow, it is plausible to identify biomarkers that monitor disease course for therapeutic development. To verify whether the 6-min walk test (6MWT) is a biomarker of SBMA, we performed the 6MWT in 35 genetically confirmed patients and in 29 age-matched healthy controls. The walk distance covered within 6 min (6MWD) was significantly less in SBMA than it was in controls (323.3 +/- 143.9 m and 637.6 +/- 94.2 m, respecti
