A predictive model to analyze the factors affecting the presence of serious chest injury in the occupants on motor vehicle crashes: Logistic regression approach.

OBJECTIVE: Chest injuries that occur in motor vehicle crashes (MVCs) include rib fractures, pneumothorax, hemothorax, and hemothorax depending on the injury mechanism. Many risk factors are associated with serious chest injuries from MVCs. The Korean In-Depth Accident Study database was analyzed to identify risk factors associated with motor vehicle occupants' serious chest injury. METHODS: Among 3,697 patients who visited the emergency room in regional emergency medical centers after MVCs between 2011 and 2018, we analyzed data from 1,226 patients with chest injuries. Vehicle damage was asse

How we do it: 'tonsil swabs please' - an alternative use in open neck surgery.

BACKGROUND: Thyroid and parathyroid surgery often involves the use of heated instruments for dissection. Whilst these are beneficial, accidental thermal damage to the exposed skin edges can occur, resulting in an unsatisfactory cosmetic outcome. Tonsil swabs can be used in head and neck surgery intra-operatively to control bleeding. This paper describes an alternative use for them in protecting wound edges during the procedure. METHOD: Damp tonsil swabs are sutured onto the wound edges after the initial skin incision. They remain present for the duration of the surgery and are removed at the

[Accidents in children. Epidemiologic studies of a rural and an urban area].

This study concerns all types of accidents which occurred within 1 year in children less than 15 years of age in 2 limited geographic areas: one urban, the other rural. 937 accidents have been registered. The accidental morbidity rate is 90 per 1,000 and the mortality rate 0.2 per 1,000 (close to the national average). Approximately 30% of accidents occur at home, 30% at school, 40% outside. Accidental death is more frequent in males, this difference increasing after the age of 10 years. The cost of the care was rather high, even though most of the accidents have had minor or no consequences.

Hitting the Nail on the Head: A Laparoscopic Retrieval of a Metallic Intrahepatic Foreign Body.

The presence of foreign bodies within the liver is an exceedingly rare event, typically resulting from either direct penetrating trauma to the abdomen or migration from the gastrointestinal tract. This report presents a case of a 21-year-old male, carpenter by occupation, who experienced accidental penetrating thoraco-abdominal trauma, leading to the retention of a metallic nail within the liver. The diagnosis was established based on conventional X-ray and CT scan findings; the nail was successfully retrieved via laparoscopy. This case underscores the significance of prompt diagnosis and inte

Analysis of forensic autopsies of elevator accidents: causes and prevention measures.

Elevator accidents are rare but can cause catastrophic damage, and an analysis of the causes of accidents and preventive measures is necessary. This study analyzed seven deaths from six autopsy cases of accidental elevator deaths that were inspected at a single forensic institution. Four cases occurred during industrial use of the elevator. Two cases occurred during elevator inspection. One case occurred in a home elevator. Three cases were because of human causes and three were owing to mechanical causes of the elevators. All fatalities were caused by severe head or thoracoabdominal injuries

Ophthalmologic findings in abusive head trauma.

A leading cause of death and disability in infancy is abusive head trauma (AHT) and there are common clinical signs that help to establish this diagnosis. Children diagnosed with AHT can have many ophthalmologic findings, including retinal hemorrhages, retinoschisis, subconjunctival hemorrhages, corneal injury, and globe rupture. If any such injuries are suspected, an ophthalmologic consultation, with indirect ophthalmoscopy, should be completed. In addition to a complete physical exam, a thorough history imaging, and lab work, should be obtained to investigate the etiology of ophthalmic patho

A case of "atypical homicidal" cut-throat injury.

Cut-throats can be of homicidal, suicidal or accidental origin. In cases of death from a cut-throat, distinguishing the cause is one of the important functions in crime investigation. The features that differentiate suicidal and homicidal cut-throat injuries are the presence of hesitation cuts, depth of wound, signs of struggle, edges of the wound, etc. In the case of a suicidal cut-throat, it is not uncommon to find hesitation cuts but in a homicidal cut-throat, it is uncommon. We present a case of a homicidal cut-throat injury but with hesitation cuts and tailing over the neck, unlike the cl

Conjunctival pedicle flap in management of open globe injury with corneal tissue loss.

Accidental injury with scissors led to an open globe injury with iris prolapse and corneal tissue loss in the right eye of a 15-year-old girl. Attempts to suture the laceration normally, led to persistent aqueous leak, and tight suturing was leading to unacceptable distortion of the corneal contour. In the absence of donor tissue or tissue glue, a bulbar conjunctival pedicle flap was used to augment sutures placed without undue tension, and watertight closure of the globe was achieved. Postoperatively, the flap retracted, and excellent tectonic, cosmetic and refractive outcomes were achieved.

Burn injuries from radiation.

Accidental exposure to radiation leading to injury and illness occurs notwithstanding safety devices and protocols used for protection. The medical management of radiation casualties is a major concern. Radiation effects are principally thermal, similar to electrical burn injuries, but with some unique systemic expression. The pathological effects of radiation to the skin are known; it is often difficult to assess the level of severity, quickly and with accuracy, because of the delay between exposure and the appearance of lesions and obscured lesions. The severity depends mainly on the nature

[Modification of axillary plexus block with the loss-of-resistance method using a blunt needle].

A modification of the method of brachial plexus block is described using the "loss of resistance"-test and a needle with an extra short 45 degrees bevel (the Crawford bevel). This report is presented firstly to describe this needle and secondly because it is not necessary to search for paraesthesiae before injecting. This technique has the advantage of a much clearer "loss of resistance" on penetrating the neurovascular bundle. We have treated 219 patients using this technique and the blocks were given by both experienced staff and trainees. The initial failure rate was 3.7%, but a second atte

[Rare forms of ureteral fistulae occurring after obstetrical and gynecological operations].

In missed intraoperative injury to the ureter one can regularly observe retroperitoneal urinary overflow (urinary retroperitonitis). In case of postoperative ignorance of the injury the overflown urine penetrates into the vagina with formation of the ureterovaginal fistula. Less frequent there are ureterocutaneous or ureteroperitoneal, ureterointestinal fistulas after the urine breaking into the operative wound, abdominal cavity or the intestine, respectively. A case of a 37-year-old woman is reported with accidental injury to the left ureter overlooked during and after the operation. The pati

Psychiatric sequelae after traumatic injury: the Pittsburgh Regatta accident.

Accidental injury in a child is sudden, often violent, and emotionally stressful, particularly when it is accompanied by hospitalization and rehabilitation. The following case report examines the presence of post-traumatic stress disorder and other psychiatric illnesses in five children involved in a boating accident during the 1988 Pittsburgh Regatta and considered severity of injury as well as complicating psychosocial stressors in the development of the disorders. The presence of symptoms was not related to the nature or extent of the injury but was instead the by-product of additional fact

Hepatotoxicity.

Chemical hepatic injury may result from accidental or suicidal exposure to toxic agents in the home or at work or from adverse reactions to medicinal agents. Chemical hepatic injury can lead to acute or chronic syndromes. Acute injury may be cytotoxic, cholestatic, or mixed. Cytotoxic injury is characterized by necrosis, steatosis, or both. Cholestatic injury is characterized by arrested bile flow and may be associated with portal inflammation or may occur in a setting in which there is no evidence of inflammation. Chronic hepatic injury includes chronic active hepatitis, steatosis, phospholip

Flexor Compartment Infection Secondary to Bovine Injection Needlestick Injury: A Case Report.

Animal injection-related needlestick injuries are a common occupational hazard for livestock workers and veterinarians. Although often unreported, these injuries can cause significant damage and may require extensive medical and surgical management. This case describes a 69-year-old farmer who accidentally injected his forearm while vaccinating cattle, resulting in a flexor compartment infection. Conservative management with oral and intravenous antibiotics was unsuccessful, and he required multiple surgical debridements for definitive treatment CONCLUSIONS:: Animal injection-related needlesti

Retention or Retrieval: A dilemma in a Case of Pediatric Cervical Spine Penetrating Injury from an Air Gun Pellet.

INTRODUCTION: Airgun or air-rifle injuries are often perceived as minor injuries; however, they can cause significant morbidity, particularly in children. The cervical spine, with its vital neurovascular and airway structures around, is vulnerable to any penetrating injury, even to low-velocity projectiles. CASE REPORT: We report the case of a 9-year-old boy who sustained an accidental posterior cervical airgun injury at home. The child was neurologically intact, and imaging revealed a retained metallic pellet deeply embedded in the posterior aspect of the neck impacted within the edge of the

Impalement injury of anorectum-A rare presentation.

INTRODUCTION: A rectal impalement injury is a rare type of penetrating injury that involves a solid object being forcefully inserted through anal opening. The removal of such injuries should be planned carefully with preparedness to assess and manage associated injuries in the pelvis and peri-anal region. CASE DESCRIPTION: An elderly female, around 65 years old, had a history of accidental penetration of an iron rod into her anal orifice. The patient was hemodynamically stable, and the distal end of the penetrated rod was visible in the anal verge on arrival. On evaluation, we found that the

Discriminating factors in fatal blunt trauma from low level falls and homicide.

PURPOSE: Determination of the manner of death is a major issue in forensic practice. Differentiating the injuries caused by falls from a low height from injuries due to the deliberate application of a blunt object can be difficult. A few studies suggested the use of certain criteria, such as the hat brim line rule, to help in differentiating between falls and blows. Unfortunately they are not consistent. METHODS: All autopsy cases from a 16-year period (1996-2012) were analyzed retrospectively. Three groups were defined: homicide cases (n = 31), sudden natural deaths involving a fall (n = 103

The medico-legal evaluation of injuries from falls in pediatric age groups.

Blunt trauma from accidental falls or intentional jumping from great heights occurs frequently in forensic medicine. The goal of this study was to investigate injuries due to falls in children under 19 years of age. Injuries from falls are the leading cause of visits to emergency departments and to deaths due to injuries. Various methods are used in the classification of falls. In this study, we have classified falls as "high-level" (≥ 5 m), "low-level" (<5 m) and "ground-level". We have retrospectively evaluated 814 boys (61.18%) and 512 girls (38.62%), making up a total of 1326 children (und

Sonographic findings of typical and atypical scrotal trauma.

This review article illustrates sonographic findings in the setting of accidental and nonaccidental scrotal trauma. Although sonographic findings may be irrespective of the type of trauma, the goals of sonographic evaluation are similar in both atypical and typical mechanisms of scrotal injury. Familiarity with findings such as disruption of testicular integrity or vascularity facilitates prompt diagnosis and plays a critical role in clinical management.
