Children and nocturnal snoring: evaluation of the effects of sleep related respiratory resistive load and daytime functioning.

Twenty-five children, age range 2 to 14 years (mean age = 7), were referred to the Stanford University Sleep Disorders Clinic for various cl … urred during REM sleep. Second degree atrioventricular blocks were also noted. Tonsillectomy and/or adenoidectomy was performed in every case and resulted in a complete disappearance or substantial amelioration of the reported symptoms. Objective evaluation by Multiple Sleep Latency Test and Wilkinson Addition Test confirmed the beneficial effect of surgery.

Adenotonsillar hypertrophy and upper airway obstruction in evolutionary perspective.

Primary care physicians have become increasingly reluctant to refer children for tonsillectomy and adenoidectomy (T and A) during the past years while antimicrobial therapy for pharyngotonsillitis and otitis media has become more common. Consequently, more children retain tonsils and adenoids throughout the childhood years. Airway compromise from adenotonsillar hypertrophy is reported in 11 cases-8 with insidious onset, 3 with acute onset. Typical symptoms occurring during sleep include snoring, snorting, enuresis and obstructive apnea. Daytime symptoms included hyponasal speech, oral respirat…

Severe velopharyngeal stenosis following repeated adenotonsillectomy in a child: a rare diagnostic and surgical challenge.

Severe complications following tonsillectomy are rare in the pediatric population. However, repeated oropharyngeal surgeries can occasionally result in velopharyngeal stenosis. We report the case of a 7-year-old patient from Côte d'Ivoire who underwent two tonsillectomies 5 months apart, along with an adenoidectomy. Subsequently, she developed hypernasal speech, chronic nasal obstruction, dysphagia to solids, and mild respiratory discomfort. Nasal endoscopy revealed a postoperative velopharyngeal stenosis characterized by fibrous narrowing between the soft palate and the posterior pharyngeal w…

Association between mouth breathing and pediatric obstructive sleep apnea: a systematic review.

BACKGROUND: Sleep is critical for children's neurocognitive and behavioural development but is often disrupted by sleep disorders like obstructive sleep apnea (OSA). While polysomnography is the diagnostic gold standard for identifying OSA, its accessibility is limited. Mouth breathing is a key, observable symptom associated with P-OSA, and it can further increase OSA risk by altering craniofacial development. Recognizing mouth breathing can aid in the early diagnosis and treatment of P-OSA. AIM: To conduct a systematic review to evaluate and consolidate the existing evidence on the relations

Is it possible to predict post-adenotonsillectomy hemorrhage in children with preoperative blood tests? Single-center retrospective study.

Introduction: Post-adenotonsillectomy (PAT) bleeding, a life-threatening surgical complication, remains unpredictable despite preoperative b … ve hemorrhage. Method: We conducted an observational retrospective study on 801 children undergoing adenoidectomy, tonsillectomy, or AT over a period of 6 months in our ENT department. Statistical analysis was performed to compare the data. Results: we did not find a statistically significant correlation between preoperative blood markers (coagulation or inflam …

Measuring immediate surgical success in children undergoing adenotonsillectomy.

Adenotonsillectomy is the most common indication for sleep-disordered breathing in children. Measuring pharyngeal closing pressures in anaesthetised children allows identification of severe obstructive sleep apnoea. This technique could help quantify immediate surgical impact and risk stratify postoperative treatment in these patients.

Efficacy and safety of adenotonsillectomy in the management of obstructive sleep apnea syndrome in children with Down syndrome: A systematic review and meta-analysis.

Obstructive sleep apnea (OSA) is commonly observed in children with Down syndrome (DS) and may affect their physical and psychological development. Currently, adenotonsillectomy is the first line treatment option for paediatric patients with OSA. However, surgical outcomes for such patients are not satisfactory. In this study, we analysed the efficacy and safety of adenotonsillectomy in the treatment of children with obstructive sleep apnea and Down syndrome. We systematically searched the PubMed, Web of Science, EMBASE, and the Cochrane databases and pooled data from nine relevant studies inv

Postoperative respiratory complications in children with obstructive sleep apnoea syndrome.

OBJECTIVE: We aimed to prospectively assess the effect of comorbidities on the occurrence of postoperative respiratory complications (PoRCs) after adenotonsillectomy in children with obstructive sleep apnoea syndrome (OSA) and whether otherwise healthy children need a higher level of postoperative monitoring. METHODS: 577 children who had OSA and underwent adenotonsillectomy were enrolled. The effects of demographics, comorbidities and OSA on PoRCs were investigated with logistic regression analysis. RESULTS: The PoRC rate was 4.3%. Postoperative oxygen desaturations were more marked in pati

Parental Knowledge of Obstructive Sleep Apnea Symptoms and Tonsillectomy in Children.

OBJECTIVE: Obstructive sleep apnea (OSA) is present in approximately 2% to 5% of children; however, only 15% of parents are reported to be knowledgeable about OSA in children. Sleep apnea in children can lead to cardiopulmonary disease, abnormal weight gain, failure to thrive, or learning difficulties. The purpose of our study is to assess parental knowledge of pediatric OSA to identify any knowledge gaps. STUDY DESIGN: Survey. SETTING: Three pediatric otolaryngology clinics associated with a tertiary care children's hospital in Buffalo, NY. METHODS: In June of 2021, parents of pediatric (0

Adenoid measurement accuracy: A comparison of lateral skull film, flexible endoscopy, and intraoperative rigid endoscopy (gold standard).

OBJECTIVE: To compare the accuracy of lateral skull film, flexible endoscopy, and intraoperative rigid endoscopy in assessment of adenoid size. METHODS: A cross-sectional study was conducted in 43 pediatric patients undergoing ENT surgery from July 2017 to December 2018. All patients underwent preoperative lateral skull film, flexible endoscopy, and intraoperative rigid endoscopy. RESULTS: The average adenoidal-nasopharyngeal (A/N) ratio obtained from lateral skull film, flexible endoscopy, and intraoperative rigid endoscopy was 72.9, 79.5, and 81.6, respectively. There was a moderate correl

Coblation intra-capsular tonsillectomy: A prospective tertiary center trial.

INTRODUCTION: There is a growing interest in intra-capsular coblation tonsillectomy (ICT) for the treatment of obstructive sleep apnea (OSA) in children. Literature remains controversial regarding which intra-capsular tonsillectomy (IT) technique is most effective and with least morbidity. Therefore, the aims of this study are to objectively measure the post-operative morbidity and the effectiveness of the ICT technique. MATERIALS AND METHODS: 107 children undergoing ICT (with or without adenoidectomy) for upper airway obstruction due to tonsillar hypertrophy at a tertiary center university h…
