Cutaneous fungal infections in the elderly.

Because of impaired host defenses and a favorable environment at specific anatomic sites, there is an increased prevalence of seborrheic dermatitis, mucosal and cutaneous candidiasis, tinea pedis, and onychomycosis in the geriatric population compared with other age groups. Both KOH and fungal culture are timely, convenient, and cost-effective methods of diagnosis. Sensitivity of these tests depends on proper technique for specimen collection and experience. KOH 20% with DMSO and DTM are highly recommended. Treatment should be tailored to the diagnosis and the individual patient. This includes

Oral substitution in patients sensitized by transdermal clonidine treatment.

The safety of oral clonidine substitution in 29 patients with confirmed localized allergic contact dermatitis due to transdermal clonidine treatment was evaluated. Only 1 patient had a skin reaction to oral clonidine. The dose of topical clonidine-inducing sensitization, and the oral challenge dose administered later, appear to be insufficient to elicit a systemic reaction in most patients. An examination of current patch test technology seems indicated in view of the fact that a number of patients who gave a negative reaction to 48-h applications of 9% clonidine in petrolatum responded positi

Atrial natriuretic peptide accelerates human endothelial progenitor cell-stimulated cutaneous wound healing and angiogenesis.

Atrial natriuretic peptide (ANP) is a powerful vasodilating peptide secreted by cardiac muscle cells, and endothelial progenitor cells (EPCs) have been reported to stimulate cutaneous wound healing by mediating angiogenesis. To determine whether ANP can promote the EPC-mediated repair of injured tissues, we examined the effects of ANP on the angiogenic properties of EPCs and on cutaneous wound healing. In vitro, ANP treatment enhanced the migration, proliferation, and endothelial tube-forming abilities of EPCs. Furthermore, small interfering RNA-mediated silencing of natriuretic peptide recept

F-fucoidan from Saccharina japonica is a novel inducer of galectin-9 and exhibits anti-allergic activity.

Fucoidan is a sulfated polysaccharide from brown sea algae. In the present study, it was demonstrated that oral administration of F-fucoidan from Saccharina japonica possessed anti-allergic effects using the passive cutaneous anaphylaxis reaction, but not by intraperitoneal administration. The inhibitory mechanism was dependent on galectin-9, which belongs to a soluble lectin family that recognizes β-galactoside and prevents IgE binding to mast cells. The anti-allergy properties of F-fucoidan were cancelled by an intravenous dose of anti-galectin-9 antibody or lactose, which bind competitively

IL-17A Is the Critical Cytokine for Liver and Spleen Amyloidosis in Inflammatory Skin Disease.

Systemic amyloidosis is recognized as a serious complication of rheumatoid arthritis or inflammatory bowel disease, but also of inflammatory skin disease. However, the detailed molecular mechanism of amyloidosis associated with cutaneous inflammation remains unclear, and therapeutic approaches are limited. Here, we investigated the pathophysiology of amyloidosis secondary to cutaneous inflammation and the therapeutic effects of Janus kinase (JAK) inhibitors by examining a mouse model of spontaneous dermatitis (KCASP1Tg mice). Moreover, KCASP1Tg mice were crossed with interleukin-17A (IL-17A) k

Breathable, Stretchable Electroporation Patches for Drug Delivery.

Arthritis has become a widespread global health issue with the aging population. Wearable transdermal drug delivery offers a promising treatment with high bioavailability and sustained drug concentrations. However, current technologies struggle with issues such as high cost, low comfort, risk of infection, or tissue pain and damage. Here, we present a breathable, stretchable electroporation patch (BSEP) that seamlessly integrates the traditional drug patch with electroporation-enhanced transdermal drug delivery technology in a low-cost manner. Conductive ink was patterned and deposited onto a

Breaching the Skin Barrier: Efficient Transdermal Delivery of Natural Collagen for Aging Skin Rejuvenation.

The supplementation of exogenous collagen is an effective strategy for combating skin aging─a complex physiological process characterized by the continuous degradation of endogenous collagen─its therapeutic efficacy has been severely limited due to poor transdermal permeability. In this study, we developed a novel noninvasive transdermal delivery system (Rh2-CLs) that effectively and safely delivers natural collagen via a novel liposome formulation for aging skin rejuvenation. The permeability of collagen was significantly enhanced by the encapsulation in the optimized liposomes, demonstrating

Research progress of microneedle drug delivery system in the treatment of autoimmune diseases.

Autoimmune diseases represent a heterogeneous group of disorders characterised by immune system dysregulation, wherein aberrant responses to self-antigens result in cellular and tissue damage. According to statistics, there are over 80 different types of autoimmune diseases worldwide, among which psoriasis and rheumatoid arthritis (RA) are relatively common. Current therapeutic strategies emphasise long-term management to mitigate symptoms and retard disease progression. Conventional approaches, including systemic administration of oral medications, injectables, and biologics, are frequently l

Efficient Loading and Sustained Delivery of Methotrexate Using a Tip-Swellable Microneedle Array Patch for Psoriasis Treatment.

Methotrexate (MTX), a primary treatment for moderate to severe psoriasis, is limited in clinical use due to suboptimal results and severe side effects from subcutaneous (SC) injection and oral administration. Microneedles offer a promising alternative for direct MTX delivery to targeted skin lesions, but issues such as drug wastage, dosage inaccuracy, and limited drug residence time in the lesions remain. This study introduces a tip-swellable microneedle array patch (TSMAP) using photo-cross-linked methacrylated hyaluronic acid (MeHA) and biocompatible resin for effective MTX loading and susta

Beclomethasone loaded liposomes enriched with mucin: A suitable approach for the control of skin disorders.

Inflammatory skin disorders are the fourth leading cause of chronic non-fatal conditions, which have a serious impact on the patient quality of life. Due to their treatment with conventional corticosteroids, which often result in poor therapeutic efficacy, relapses and systemic side effects from prolonged therapy, these diseases represent a global burden that negatively impacts the global economy. To avoid these problems and optimize corticosteroid benefits, beclomethasone was loaded into liposome formulations specifically tailored for skin delivery. These formulations were enhanced with mucin

Pregabalin-induced delayed cutaneous hypersensitivity reaction occurring after 40 days of use: a case report.

Pregabalin is the first-line treatment for neuropathic pain. Cases of cutaneous hypersensitivity reactions caused by pregabalin generally occur within 2 weeks of initiating medication. We report a rare case of a delayed cutaneous hypersensitivity reaction caused by pregabalin, which was confirmed by a drug provocation test. A 72-year-old man with severe herpes zoster neuralgia developed maculopapular drug eruption covering 80% to 90% of his total body surface area after 40 days of combined multidrug analgesia. A drug provocation test for pregabalin was positive. The time interval between initi
