[A young woman with a labile mood, hyperactivity, hyperthermia and exhaustion: symptoms of lethal catatonia].

A 22-year-old woman went through a period with a labile mood that first turned into psychotic hyperactivity and was then followed by hyperthermia and exhaustion. This was accompanied by diminished consciousness, sinus arrhythmia and respiratory insufficiency. With a working diagnosis of 'lethal catatonia' she was treated by electroshock and later also with the antipsychotic agent quetiapine. After a few electroshock treatments her vital functions improved and ultimately, the patient recovered. Lethal catatonia is characterised by acute excitation, catalepsy, autonomic instability and fever, wh

Are There Any Differences in Clinical and Biochemical Variables between Bipolar Patients with or without Lifetime Psychotic Symptoms?

INTRODUCTION: Psychotic symptoms occur in more than half of patients affected by Bipolar Disorder (BD) and are associated with an unfavorable course of the disorder. The objective of this study is to identify the differences in the clinical and biochemical parameters between bipolar patients with or without psychotic symptoms. METHODS: A total of 665 inpatients were recruited. Demographic, clinical, and biochemical data related to the first day of hospitalization were obtained via a screening of the clinical charts and intranet hospital applications. The two groups identified via the lifetime

On the Borderline Syndrome and Its Relationship to Schizophrenia.

This article explores the historical and phenomenological overlap between borderline personality disorder and schizophrenia, highlighting the presence of frank psychotic symptoms and psychotic and psychotic-like thinking in borderline psychopathology. Early theorists conceptualized borderline personality as a schizophrenia-like syndrome, and many pioneers in the understanding of borderline pathology began their careers with interest in schizophrenia. Similarities between the two syndromes include affect-laden perception; ideas of reference; paranoid tendencies, especially suspicious and reacti

Clinical Usefulness of Aripiprazole and Lamotrigine in Schizoaffective Presentation of Tuberous Sclerosis.

Tuberous sclerosis is not as rare as once thought and has high psychiatric comorbidities. However, bipolar or psychotic features associated with tuberous sclerosis have been rarely reported. This report first presents a tuberous sclerosis patient, resembling a schizoaffective disorder of bipolar type. A patient with known tuberous sclerosis displayed mood fluctuation and psychotic features. Her symptoms did not remit along with several psychiatric medications. After hospitalization, the patient responded well with lamotrigine and aripiprazole without exacerbation. As demonstrated in this case,

Psychosis after epilepsy surgery: report of three cases.

Temporal lobe epilepsy surgery has become a successful alternative in patients with refractory epilepsy. However, the outcome of epilepsy surgery may be affected by the occurrence of postsurgical psychiatric symptoms, such as psychosis. This report describes three cases of refractory temporal lobe epilepsy and hippocampal sclerosis, which, after anterior temporal lobectomy, presented with acute psychosis. One of them had a history of acute psychosis, and all of them met criteria for Cluster A personality disorder (schizoid/schizotypal) during psychiatric assessment prior to surgery. The three

Clozapine: a clinical review of adverse effects and management.

Clozapine (Clozaril) is a novel and unique prototype atypical, tricyclic, dibenzodiazepine-derivative, antipsychotic agent. It has been proven effective and significantly superior to placebo, as well as to conventional neuroleptics, in several placebo-controlled, double-blind studies in treatment-resistant schizophrenia. It has also been found to produce an incidence of extrapyramidal symptoms (EPS) as low as that found with placebo. Approximately 30-60% of all schizophrenic patients who fail to respond to typical antipsychotics may respond to clozapine. It was the first major advance that mar

Change in the life expectancy gap among people with mental illness: a 14-year population-based cohort study in Australia.

BACKGROUND: People with psychiatric disorders die substantially earlier than the general population, with life expectancy gaps of 10-20 years reported internationally. Prior Australian data indicated this gap was widening, whereas evidence from other countries has been mixed. We aimed to determine whether the life expectancy gap in people with hospitalised psychiatric disorders versus the general population is changing in Queensland, Australia, and to identify which causes of death drive excess mortality. METHODS: We conducted a retrospective population-based cohort study using linked Queensl

[Twin studies in psychiatry].

Twin research in medicine and psychology has blossomed during the last decades. A brief presentation of twin research methods is given, followed by a review of psychiatric studies, particularly Norwegian ones. The Nordic investigations are of great importance because many of these studies have been able to avoid shortcomings in sampling by linking national twin registers to disease registers. This procedure is of consequence for the estimation of heritability. Twin research shows that the genetic contribution to Alzheimer's disease, manic-depressive (bipolar) disorder, schizophrenia and childh

Deficits of ideokinetic praxis in chronic schizophrenics on a modified version of the Luria-Nebraska Motor Scale.

One disturbance of motility in schizophrenia is the blocking of voluntary action, or ambitendency. Although the traditional explanation for this disorder is psychological, the documentation of psychomotor slowing in the literature suggests the possibility of a neurologic pathogenesis. It was hypothesized that disconnection between the ideatory and motor components of a voluntary action described as ambitendency is similar to the ideokinetic apraxia of disease of the left frontal cortex. It was also hypothesized that because of this presumed anatomical locus, manifestation of ideokinetic apraxi

Morbidity risks of schizophrenia and affective disorders among first degree relatives of patients with schizophrenia, mania, depression and surgical conditions.

One thousand five hundred and seventy eight first degree relatives of schizophrenics, manics, depressives and controls were personally interviewed using the Iowa Structured Psychiatric Interview Form without knowledge of the probands' diagnoses. Our data, based on blind diagnostic assessment of the relatives, support the distinction between schizophrenia and affective disorders, although the distinction between schizophrenia and mania was not clear-cut. Our data could not support familial subtyping of paranoid and non-paranoid schizophrenia, and unipolar and bipolar affective disorders. Future

Scales for rating psychotic and psychotic-like experiences as continua.

Although psychotic symptoms are usually viewed as dichotomous events, both patients and nonpatients often report isolated psychotic experiences as well as psychotic-like experiences, the latter being attenuated versions of psychotic experiences. College students, previously identified as probably at high risk for psychosis, were interviewed using a modified Schedule for Affecting Disorders and Schizophrenia--Lifetime Version (SADS L). Eighty types of deviant experiences reported in the interviews fell into six categories: transmission of one's own thoughts, passivity experiences, thought withd

[Electroconvulsive therapy in people with intellectual disability].

Electroconvulsive therapy (ECT) is a highly effective treatment option for severe mental illness. Although people with intellectual disability (ID) have similar prevalence rates of mental disorders in comparison to the general population their access to ECT remains challenging. A systematic literature review was carried out on treatment with ECT in patients with ID and a case report on a patient with ID who underwent ECT is presented, to highlight a typical clinical routine. A total of 100 articles with 208 different case reports were retrieved. In summary, the results underline the effectiven

The effects of cocaine exposure in adolescence: Behavioural effects and neuroplastic mechanisms in experimental models.

Drug addiction is a devastating disorder with a huge economic and social burden for modern society. Although an individual may slip into drug abuse throughout his/her life, adolescents are at higher risk, but, so far, only a few studies have attempted to elucidate the underlying cellular and molecular bases of such vulnerability. Indeed, preclinical evidence indicates that psychostimulants and adolescence interact and contribute to promoting a dysfunctional brain. In this review, we have focused our attention primarily on changes in neuroplasticity brought about by cocaine, taking into account

A controlled family study in panic disorder.

There are only a few family studies in panic disorder. Although there is some evidence that panic disorder is familial, the exact figures of the familial risk for this disorder are at variance across different studies; the impact of comorbidity and of the gender of relatives is also unclear. Family studies in panic disorder controlling for the comorbidity in probands are therefore indicated. This study presents the morbid risks in families of 40 "pure" panic disorder probands (DSM-III-R) without a history of psychotic disorders, major depression or alcoholism compared with families of 80 contr

The epidemiologic evidence linking autoimmune diseases and psychosis.

This review summarizes the epidemiologic evidence linking autoimmune diseases and psychosis. The associations between autoimmune diseases and psychosis have been studied for more than a half century, but research has intensified within the last decades, since psychosis has been associated with genetic markers of the immune system and with excess autoreactivity and other immune alterations. A range of psychiatric disorders, including psychosis, have been observed to occur more frequently in some autoimmune diseases, such as systemic lupus erythematosus and multiple sclerosis. Many autoimmune di

Cannabidiol versus placebo as adjunctive treatment in early psychosis: study protocol for randomized controlled trial.

BACKGROUND: Psychotic disorders are a leading cause of disability in young adults. Antipsychotics have been the primary intervention for psychosis for over 60 years, and yet, we have made little progress in treating negative symptoms, neurocognition, and functional disability. There is growing evidence that cannabidiol (CBD) is effective in treating positive psychotic symptoms, possibly also negative and neurocognitive symptoms, and moreover is well tolerated compared to other psychotropic medications. Anecdotally, patients participating in the Cognitive Assessment and Risk Evaluation (CARE) E

Delusions of fatal contagion among refugee patients.

Delusions of fatal contagion were encountered in about 10% of refugee psychiatric patients from Southeast Asia belonging to one ethnic group, the Hmong. Psychotic depression was the most common diagnosis among the Hmong patients with delusions of contagion, whereas paranoid and schizophrenic diagnoses predominate in patients with similar delusions from other refugee groups. Some cases also occurred as a shared delusional disorder. Associated findings included isolation from the community, intrafamilial conflict, failure to acculturate, and sexual frustration or conflict. Most patients responde

[Imipramine and neuro-psychiatry. Retrospective study of 557 out-patients treated in a neuro-psychiatric unit (author's transl)].

A retrospective study of 557 outpatients who had been treated with imipramine has been undertaken in a neuro-psychiatric unit. The usual dose was 75 mg per day, most often associated with other psychotropic drugs. The main indication was a depressive state for 75 p. c. of the patients: in that case the efficacy reached 65 p. c. The achievement of such results with moderate doses might indicate a possible relationship between the optimal dosage of the antidepressive drug and the intensity of the mood disorder. 12.5 p. c. of the patients were treated for a post-contusional syndrome. Association between cognitive phenotype in unaffected siblings and prospective 3- and 6-year clinical outcome in their proband affected by psychosis.

BACKGROUND: Cognitive alterations are a central and heterogeneous trait in psychotic disorders, driven by environmental, familial and illness-related factors. In this study, we aimed to prospectively investigate the impact of high familial risk for cognitive alterations, unconfounded by illness-related factors, on symptomatic outcomes in patients. METHODS: In total, 629 probands with non-affective psychosis and their sibling not affected by psychosis were assessed at baseline, 3- and 6-year follow-up. Familial cognitive risk was modeled by three cognitive subtypes ('normal', 'mixed' and 'impa
