[General practitioner plays a central role as case manager. Diagnosis and therapy of alcoholism].

The family doctor plays an important role in the treatment of alcoholism, especially in the initial contact and motivation phases, but also in the aftercare phase following outpatient or inpatient detoxification and rehabilitation. The central tasks are the diagnosis of alcohol dependency and possible neurological and somatic sequelae, mediation, information about consequences and motivation to undergo further treatment. After successful treatment (abstinence)--prevention of relapse, where necessary using so-called anti-craving drugs such as acamprostate.

Sweet liking and family history of alcoholism in hospitalized alcoholic and non-alcoholic patients.

The present study was designed to test the hypothesis that preference for stronger sweet solutions may be associated with the genetic risk for alcoholism. Thirty-two male patients with alcohol dependence admitted for alcoholism in-patient treatment and 25 non-alcoholic control subjects were used in the study. Hedonic response to sweets was evaluated using the sweet preference test. Family history of alcoholism was evaluated using a Russian version of the Michigan Alcoholism Screening Test modified for the assessment of the alcohol-related behaviour of the subject's biological father. Similar t…

Blockage of drug resistance in vitro by disulfiram, a drug used to treat alcoholism.

BACKGROUND: P-glycoprotein (P-gp) pumps a wide range of cytotoxic drugs out of cells. Inhibiting maturation of P-gp would be a novel method for circumventing P-gp-mediated multidrug resistance, which complicates cancer chemotherapy and treatment of patients infected with human immunodeficiency virus. We examined the effect of disulfiram (Antabuse(TM)) on the maturation and activity of P-gp. METHODS: Embryonic kidney cells were transfected with a complementary DNA for the P-pg gene, and the effects of disulfiram on the sensitivity of the transfected cells to cytotoxic agents were determined. Spider angiomas in patients with liver cirrhosis: role of alcoholism and impaired liver function.

BACKGROUND: Spider angioma is a common sign in patients with liver cirrhosis, but the pathogenesis is still unclear. Alcohol and hyperestrog … longer prothrombin time (16.8 +/- 0.8 versus 14.8 +/- 0.4 sec; P = 0.01), and higher prevalence of alcoholism (41% versus 20%; P = 0.04) than those without. Stepwise logistic regression showed that alcoholism and serum bilirubin level were the only significant and independent predictors associated with the presence of spider angiomas in cirrhotic patients ( …

Is aberrant mammary tissue a marker for chronic alcoholism or kidney-urinary tract malformations?

BACKGROUND: Numerous publications describe the relationship between aberrant mammary tissue (AMT) and kidney-urinary tract malformations, individual/ familial alcoholism and sense organ disorders. PATIENTS AND METHODS: We investigated these possible associations and reviewed 72 cases observed in our Department during the past 3 years: 30 men and 42 women, 17 of them with bilateral AMT (7 men and 10 women) and 25 patients from 9 families. Diagnosis was made according to Kajawa's classification. A detailed family history was performed asking for individual or familial alcoholism, especially in …

Adulthood functioning: the joint effects of parental alcoholism, gender and childhood socio-economic stress.

The popular literature has publicized the adjustment difficulties of adult children of an alcohol-dependent parent (ACOAs); however, empirical studies do not provide consistent support. We examined the impact of parental alcoholism, degree of childhood socio-economic stress and gender on three broad categories of adulthood functioning (psychopathology, socio-economic attainment and marital stability). These effects were investigated with a heterogeneous sample of 400 men and 226 women participating in studies at the University of Michigan Alcohol Research Center. Parental alcoholism and childh…

Family history of alcoholism and psychiatric co-morbidity in Brazilian male alcoholics and controls.

The present study was aimed at replicating, in a sample of Brazilian subjects, findings generated mostly in developed countries on the relative prevalence of alcohol problems, associated psychiatric disorders, family history of alcoholism and other familial psychiatric disorders in alcoholics and controls. A group of Brazilian male alcoholics (n = 103) and controls (n = 63) at first admission to ambulatory or inpatient treatment were interviewed individually using sections of the Structured Clinical Interview for DSM-III-R. Information on demographic characteristics, alcohol-related problems, …

[The role of acetaldehyde in the pathogenetic mechanisms of the development of chronic alcoholism].

Experiments on white rats were carried out to confirm an important role of acetaldehyde in pathogeny of alcoholism. It is evidenced by results of experiments when animals were given acetaldehyde and medichronal (a drug which combines acetaldehyde) and by data obtained in the course of studies of changes in the content of endogenic ethanol and acetaldehyde, activity of alcohol dehydrogenase and aldehyde dehydrogenase, concentration of biogenic amines (catecholamines and serotonin) in the blood and brain structures.

Mechanisms of alcohol abuse and alcoholism in adolescents: a case for developing animal models.

This paper reviews the ontogeny of neurotransmitter systems and neuropharmacological challenge within transmitter systems and discusses the actions of alcohol on such systems during the juvenile through adolescent periods. To place the animal research within the context of human development, an attempt is made to first examine some fundamental principles of developmental research as they relate to the adolescent period. Evidence presented from animal studies indicates that unique neurochemical and behavioral changes are occurring during postnatal development, including adolescence, that could

The psychometric assessment of alcoholism in forensic groups: the MacAndrew scale and response bias.

Although the MacAndrew Alcoholism scale is the most widely used Minnesota Multiphasic Personality Inventory (MMPI) measure of vulnerability to alcohol abuse, its accuracy has not been studied in patients intrinsically motivated to exaggerate or minimize psychopathology. We examined the usefulness of the MAC in predicting alcohol abuse in a forensic clinical sample. Results indicate the MAC (a) was not more effective than direct inquiry in this group, (b) scores were correlated negatively with minimization and positively with exaggeration for subjects with histories of alcohol abuse, (c) offere…

Combined dopamine and serotonin agonists: a synergistic approach to alcoholism and other addictive behaviors.

Successful phentermine and racemic fenfluramine treatment of 150 obese patients led to the use of this therapy in 12 alcoholic patients. Eleven of 12 consecutive patients, most within hours, reported a total loss or marked decrease in alcohol craving. Their reported consumption of alcohol ceased or decreased markedly. The hypothesis is proposed that this treatment is successful because of the dual and balanced increase in the bioavailability of the neurotransmitters, dopamine and serotonin, in the nucleus accumbens. The prospect is raised that cocaine, nicotine, narcotic, and amphetamine addic

Gender differences in the specificity of alcoholism transmission among the relatives of opioid addicts.

Gender differences in the specificity of drug versus alcohol transmission were examined among 201 opioid addicts and their 877 first-degree relatives using direct interviews and a structured family history method based on the Schedule for Affective Disorders and Schizophrenia Research Diagnostic Criteria. A strong association of parental alcoholism with alcoholism among the proband addicts was found, suggesting some specificity for drug versus alcohol abuse. We also found that among the 477 siblings, those with alcoholism alone did not have parents with drug abuse and those parents with drug a…

[Biological changes caused by ethanol: their sequelae and importance in the diagnosis of alcoholism].

Ethanol has profound effects on cellular function, causes hormonal imbalance and either directly or indirectly results in nutritional deficiencies. Together with genetic and environmental factors these metabolic changes eventually cause functional and structural damage to liver, heart, central nervous system and other organs. In order to prevent organ injury the early recognition of the alcoholic patient is important. The combination of physician interview, questionnaire and laboratory markers of ethanol abuse [MCV, GGT, AST and others] is useful for the diagnosis of alcoholism. Abstinence is …

A controlled family history study of Tourette's syndrome, II: Alcoholism, drug abuse, and obesity.

The behaviors associated with Gilles de la Tourette's syndrome (TS), including impulsive, compulsive, attentional, learning, conduct, and mood disorders, have often been described as substrates for the development of alcoholism and/or drug abuse. As the authors' experience with TS pedigrees indicated that alcoholism and/or drug abuse were common in relatives of TS probands, they examined, by the family history technique, 1750 relatives over 14 years of age in 130 TS proband and 25 control families. Significant, life-disrupting problems with alcoholism and/or drug abuse were present in 14.5% of…

Alcohol and drug use by college males as a function of family alcoholism history.

Family history of alcoholism increases the risk for development of alcoholism in male offspring. The present questionnaire study examined self-reported alcohol and drug use in 744 college males as a function of DSM-IIIR alcohol dependence diagnoses in first- and/or second-degree biological relatives. Substance use was most prevalent and most frequent in students with both first- and second-degree alcohol-dependent family members, was intermediate in students with only first-degree affected relatives, and was least in students with no affected relatives. Students with both first- and second-deg…

Clinical phobias and avoidant personality disorder among alcoholics admitted to an alcoholism rehabilitation setting.

One hundred seventy-three abstinent patients were screened for phobias and avoidant personality disorder. Ninety-six patients were interviewed and diagnosed by two independent assessors who were in agreement on 74% of the diagnoses. In DSM-III terms, over half of the sample (51.4%) met criteria for agoraphobia (8.5%), social phobia (7.8%) and avoidant personality disorder (35.1%). More than 70% of the patients in each diagnostic category were men. For the vast majority of the patients in the sample the disorder preceded the abuse of alcohol which was used by at least 40% of them to relieve the

Severe alcoholism in the mental health sector: II. Effects of service utilization on readmission.

A follow-up study of a cohort of suburban alcoholics who are comprehensively served by the public mental health sector used a Cox survival time analyses to examine the relationship of patient characteristics, receipt of outpatient services and readmission to inpatient care. Patients were followed up until their first readmission to inpatient care or for 2 yr subsequent to an inpatient episode. As expected, established chronicity was associated with short "survival" in the community, as were youth and living alone. For first admissions, the receipt of aftercare was associated with a decreased l
