The Impact of Membrane Stabilization for Alveolar Ridge Preservation in Periodontally Compromised Extraction Sockets: A Non-Inferiority Randomized Controlled Trial.

AIM: The aim of this study was to investigate the non-inferiority of alveolar ridge preservation (ARP) with membrane stabilization compared with non-stabilization, focusing on changes of soft-tissue contour in periodontally compromised extraction sockets. Secondary outcomes included changes in hard-tissue contour, patient-reported outcomes and new bone formation. MATERIALS AND METHODS: Twenty-four patients with periodontally compromised teeth were randomly assigned to ARP with (test group) or without (control group) membrane stabilization. To assess profilometric and hard-tissue dimensional c

Lateral Approach to the Maxillary Sinus and Mandibular Canal in Severely Atrophied Posterior Alveolar Bone.

PURPOSE: To present a technique to rehabilitate atrophied alveolar ridges in the posterior maxilla and mandible using bone lateral to the maxillary sinus and to the inferior alveolar nerve and to present a retrospective study of the technique. MATERIALS AND METHODS: Severe resorption of the posterior region of the maxilla and mandible was treated following a conservative approach. Patients who presented this bone crest condition that impeded the placement of implants and had an anatomy that allowed the inferior alveolar nerve or the maxillary sinus to be approached laterally were treated. The role of alveolar ridge width in dental implantology.

Sometimes, preoperative planning in dental implantology, based on sufficient alveolar height, cannot be verified due to transversal deficiencies. A total of 102 bony mandibles and 95 maxillae were analysed after classification of atrophy, simulating implant insertion at 518 standardised edentulous cross sections with regard to anterior/posterior, mandible/maxilla and class of atrophy. Furthermore, the relation of alveolar height to possible implant length in 86 patients was evaluated retrospectively. Implant length reduction compared with alveolar height was necessary in 10% (mandibles) and 7.

Influence of residual ridge resorption patterns on fixture placement and tooth position, Part III: Presurgical assessment of ridge augmentation requirements.

Implant diagnostic methods using computed tomography with barium-coated templates have revealed the relationship between the optimal final tooth position and the residual alveolar process or ridge. While information with regard to emergence axis, anatomic limitations, fixture length, buccolingual cantilever, and prediction of the the final prosthetic design is available, presurgical information relating to the reconstruction potential of smile zone deformities remains elusive. The use of a complete provisional wax-up and the modification of a barium-coated template design can provide the impla

Morbidity of harvesting of retromolar bone grafts: a prospective study.

20 retromolar bone grafts were harvested in outpatients for augmentation of the implant site from January to June 2000 (10 female, 10 male, 40.9 +/- 12.8 years, minimum 17 years, maximum 66 years). The aim of the study was to assess typical complications of this procedure in a prospective manner. For the determination of the superficial sensory function of the inferior alveolar and the lingual nerve, an objective method was used. The bone grafts were harvested for single tooth reconstruction. In 14 cases a ridge augmentation and in 6 cases an endoscopically controlled crestal sinus floor eleva

Periodontally accelerated osteogenic orthodontics combined with autogenous bone grafting.

This case report documents the first use of particulate autogenous bone graft with the corticotomy-assisted rapid orthodontic procedure known as periodontally accelerated osteogenic orthodontics (PAOO). A 41-year-old man, with class II, division 2 crowded occlusion, was treated with the PAOO procedure. Buccal mucoperiosteal flaps were reflected, and selected vertical and horizontal corticotomy was performed around the roots in both the maxillary and mandibular arches. Particulate bone graft was harvested from the rami and exostosis for alveolar ridge augmentation. Orthodontic movement was init…

Inverted periosteal flap: an alternative to the buccal advancement flap for tension-free, watertight closure.

The buccal advancement flap has been universally used in oral and maxillofacial surgery for closure of alveolar ridge incisions. It involves scoring of the buccal periosteum to enhance flap mobility and then stretching the buccal tissues medially (palatally or lingually) to obtain tension-free, watertight closure of a wound. Its applications have included, but have not been limited to, alveolar ridge augmentation procedures and closure of oral-antral communications. However, the buccal advancement flap technique has several major disadvantages. First, because the buccal flap is advanced cresta…

[Follow-up studies of 3-dimensional osteoplastic reconstruction of the extremely atrophied maxilla combined with implants].

In the severely resorbed maxilla, a 10-year success rate of only 49-74% of implants in combination with autogenous bone grafts has been reported. We developed a modified technique of antral inlay grafting and lateral and vertical onlay grafting of the severely resorbed maxilla for inserting implants to retain dentures. The clinical and radiologic results are presented. In 21 patients with severely resorbed edentulous maxillae, a total of 20 bilateral and one unilateral antral inlay graftings and lateral and vertical onlay graftings were performed after a prosthodontic setup. We opened the maxi

Morbidity of anterior iliac crest and calvarial bone donor graft sites: a 1-year randomized controlled trial.

Autogenous bone graft harvesting is still commonly considered the gold standard for the reconstruction of a severely resorbed maxillary alveolar ridge; however, the preferred donor site remains a subject of debate. This study compared the morbidity of calvarial and iliac crest donor sites after harvesting. Twenty edentulous patients with an insufficient volume of maxillary bone for reliable implant placement were assigned randomly to either calvarial (n=10) or anterior iliac crest (n=10) bone harvesting groups. All patients underwent a maxillary sinus floor elevation procedure combined with wi

Piezoelectric surgery in mandibular split crest technique with immediate implant placement: a case report.

Implant supported rehabilitation of thin edentulous ridges with horizontal atrophy necessitates a regenerative approach. Within the procedures for horizontal bone defects augmentation, ridge expansion techniques permit dislocation of the buccal bone plate in a labial direction and simultaneous implant insertion in single-stage surgery, abbreviating overall treatment time. The piezoelectric ridge expansion technique permits to obtain the expansion of very mineralized bone crests without excessive traumas or the risk of ridge fractures. The case reported shows an implant treatment for partial ede
