Amebiasis is a neglected tropical disease which is caused by the protozoan parasite Entamoeba histolytica. This disease is one of the leading causes of diarrhea globally, affecting largely impoverished residents in developing countries. Amebiasis also remains one of the top causes of gastrointestinal diseases in returning international travellers. Despite having many side effects, metronidazole remains the drug of choice as an amebicidal tissue-active agent. However, emergence of metronidazole resistance in pathogens having similar anaerobic metabolism and also in laboratory strains of E. hist…

Although amebic brain abscess is a rare form of invasive amebiasis, when present, it is frequently lethal. This disorder always begins with the infection of the colon by Entamoeba histolytica trophozoites, which then travel to extra-intestinal tissues through the bloodstream. Amebic brain abscesses are produced when trophozoites invade the central nervous system. Computerized axial tomography scans can be used to diagnose the presence or absence of a brain abscess with a certainty of 100%. However, this diagnostic tool does not reveal the etiological agent of disease. By analyzing the clinical…

Amebiasis, infection with the protozoan parasite Entamoeba histolytica, is the third leading cause of death from parasitic diseases. Despite its importance, we know little about protective immunity to amebiasis. Now, studies from a cohort of children in Bangladesh have provided some critical insights into immunity to intestinal amebiasis. Children with mucosal IgA antibodies to the amebic adherence lectin were found to be resistant to reinfection with E. histolytica. However, immunity was short-lived, and approximately 20% of children in the cohort had a second episode of E. histolytica infect…

One of the hallmarks of tissue necrosis produced by the human protozoan parasite Entamoeba histolytica, the causative agent of human amebiasis, appeared to be the lack of inflammatory reaction to the invading trophozoites. Recent evidence suggests, however, that inflammatory cells do appear during early stages of amebic destructive lesions and that they contribute to the establishment of foci of tissue necrosis in intestinal and liver lesions. The present analysis of the fine-structural changes that take place during early stages of amebic liver abscesses induced in hamsters after the intrapor…

A patient with an amebic liver abscess had an incomplete response to an initial course of metronidazole, but subsequently responded to a prolonged course of the agent. Recurrence of disease in the contralateral hepatic lobe responded to chloroquine along with a luminicide. We took the initial failure of metronidazole to be secondary to either the advanced nature of the disease or the development of partial resistance to metronidazole. The recurrence was the result of reinfection of the liver by persistent organisms in the bowel, emphasizing the importance of giving a lumicide in the initial tr

The clinical findings in 56 patients with hepatic amebiasis are reviewed. This illness was most frequent in male black patients, 20-39 years old. The clinical picture is dominated by upper right quadrant abdominal pain, general malaise, pain on percussion of the right hypochondrium and tender hepatomegaly. Jaundice is not rare and appears to have no prognostic significance. The importance of early diagnosis is emphasized.
