A reevaluation of the role of crystalloid preload in the prevention of hypotension associated with spinal anesthesia for elective cesarean section.

BACKGROUND: Hypotension after spinal anesthesia for cesarean section remains a common and serious complication despite the use of uterine displacement and volume preloading. The current study revaluated the role of crystalloid volume preloading in this context. METHODS: In a two-stage open sequential design, patients presenting for elective repeat cesarean section were allocated to receive either no preload or 20 ml/kg crystalloid administered over 15-20 min before spinal anesthesia. Hypotension was defined as a decrease in systolic pressure to less than 100 mmHg and to less than 80% of basel

The sedative effect of intravenous injection of low dose midazolam during spinal anesthesia in cesarean section.

In a randomized blind study, we compared the neonate Apgar score, umbilical venous pH, incidence of maternal hypotension and intra-operative maternal discomfort in elective Cesarean section performed under spinal anesthesia in two groups of parturients with or without low dose midazolam as a sedative agent before the deliveries. Each group consisted of 20 parturients. Ninety percent of the mothers in the midazolam group fell asleep smoothly before the operations started. The neonates in the midazolam group were scored similar to saline control group on Apgar score and umbilical venous pH. The

Assessment of ether anesthesia in normal and spinal cats using gas--liquid chromatographic analysis of blood.

A new gas-liquid chromatographic method was developed for the determination of diethylether in whole blood. Ether was quantitated by peak area ratio analysis with n-propanol as the internal standard using a flame ionization detector. Blood ether concentrations were determined in cats undergoing inhalational anesthesia by ether in oxygen. In normal spontaneously breathing cats, anesthesia began at ether concentrations of about 0.6 g/l, and respiratory arrest occurred at 2.4 g/l and above. Mean arterial blood pressure was well maintained throughout the entire anesthetic range. In spinal artifici

The Incidence of Acute Respiratory Infection Was Higher in the Older Adults with Lower Limb Fracture Who Receive Spinal Anesthesia Than Those Who Receive General Anesthesia.

INTRODUCTION: Acute respiratory infection (ARI) can significantly reduce postoperative quality of life and impair the recovery of older adult patients with lower-limb fractures, and its relationship with methods of anesthesia remains inconclusive. Using data from the National Health Insurance Research Database (NHIRD) of Taiwan, this study examined the data of patients who received surgical management for lower-limb fractures and compared those who underwent general anesthesia (GA) with those who underwent regional anesthesia (RA) in terms of their incidence of acute upper and lower respirator

Dexmedetomidine-associated acute apnea during lower extremity surgery under combined spinal-epidural anesthesia: a case report.

INTRODUCTION: Dexmedetomidine, a highly selective α2-adrenergic receptor agonist, is widely utilized in clinical practice for its excellent sedative and analgesic effects, and is commonly employed as an adjuvant sedative during the perioperative period. Common adverse reactions associated with dexmedetomidine include bradycardia and hypotension, while respiratory depression is relatively rare. CASE PRESENTATION: A 69-year-old female patient of Han ethnicity was admitted due to a fracture of the middle and lower segment of the right femur. The patient underwent open reduction and internal fixa

Comparative Study of Preprocedure and Real-Time Ultrasound-guided Combined Spinal Epidural Anesthesia in Obese Patients.

BACKGROUND: It is a great challenge to perform regional anesthesia in obese patients. In general, combined spinal epidural (CSE) anesthesia is performed blindly using the surface landmark of the midline approach. Obesity masks the surface landmark and makes the procedure difficult. AIM: This study aimed to compare the efficacy and ease of the real-time ultrasound (USG)-guided CSE (RUC) and preprocedure USG-guided CSE (PUC) approach in obese patients. SETTINGS AND DESIGN: This prospective randomized intervention study was conducted at a tertiary care center. METHODS: Eighty patients with the

Pericapsular Nerve Group Block versus Femoral Block for Positioning During Spinal Anesthesia for Hip Fracture Surgeries.

BACKGROUND: Pericapsular nerve group (PENG) block is a novel approach for dynamic pain relief in hip fractures. Studies on the effectiveness of PENG block for positioning for spinal anesthesia are very few in the Indian population. This study aimed to identify the superior efficacy of PENG block over femoral block for positioning during spinal anesthesia for hip surgeries. This study will help use the PENG block as a better approach to currently used blocks without sparing any articular branches of the nerves to the hip joint. MATERIALS AND METHODS: Seventy-four patients who were scheduled fo

Segmental Thoracic Spinal Anesthesia for Critical Patients Undergoing Abdominal Surgeries: A Case Series and Literature Review.

Segmental thoracic spinal anesthesia (STSA) is emerging as a promising alternative for high-risk patients undergoing abdominal surgeries, particularly those who are not optimal candidates for general anesthesia (GA). By selectively targeting the thoracic spinal segments responsible for abdominal innervation, STSA aims to provide precise anesthesia and pain management while minimizing systemic side effects. This case series presents the outcomes of several critical patients who underwent abdominal surgeries under STSA. All patients who were considered at high risk for GA and underwent abdominal

Fatal rhabdomyolysis and disseminated intravascular coagulation after total knee arthroplasty under spinal anesthesia: A case report.

BACKGROUND: Rhabdomyolysis develops as a result of skeletal muscle cell collapse from leakage of the intracellular contents into circulation. In severe cases, it can be associated with acute kidney injury and disseminated intravascular coagulation, leading to life threatening outcomes. Rhabdomyolysis can occur in the perioperative period from various etiologies but is rarely induced by tourniquet use during orthopedic surgery. CASE SUMMARY: A 77-year-old male underwent right total knee arthroplasty using a tourniquet under spinal anesthesia. About 24 h after surgery, he was found in a drowsy

Steroid-induced rapid recovery from respiratory dysfunction in a patient with myasthenia gravis after spinal anesthesia.

We report an 83-year-old man with myasthenia gravis (MG) who developed respiratory depression after spinal anesthesia for transurethral laser enucleation of the prostate. He became less responsive after complained of dyspnea, with a decrease of SpO2 to 83% approximately 13 min after intrathecal administration of 0.5% isobaric bupivacaine 3 ml. With a diagnosis of exacerbation of MG, hydrocortisone 100 mg was administered, following which both consciousness and spontaneous respiration rapidly improved. Cold sense was observed below the C4 dermatome. We provided general anesthesia without using

Associated factors with umbilical arterial pH after cesarean delivery under spinal anesthesia: a retrospective cohort study.

BACKGROUND: Maximum decrease of blood pressure and number of minutes of hypotension were independently associated with umbilical arterial pH. However, the impact of hypotension considering the duration of it on umbilical arterial pH is unknown. METHODS: Pregnant women aged ≥ 20 years who delivered a baby at full-term via a cesarean delivery under a single-shot spinal anesthesia between January 2017 and March 2019 were included. The main outcome was to predict umbilical arterial pH, based on the value of the time integral of hypotension. Patient demographics, patient comorbidities, and intraop

Does spinal anesthesia lead to postoperative urinary retention in same-day urogynecology surgery? A retrospective review.

INTRODUCTION AND HYPOTHESIS: Spinal anesthesia has been reported to be a risk factor for postoperative urinary retention (POUR) in various surgical specialties. We hypothesized that spinal anesthesia was a risk factor for POUR after outpatient vaginal surgery for pelvic organ prolapse (POP). METHODS: This was a retrospective review of an urogynecology database for all outpatient POP vaginal surgeries performed in 2014 to evaluate the risk of POUR after general versus spinal anesthesia. A standardized voiding trial was performed by backfilling the bladder with 300 ml of saline. A successful tr

Phenylephrine vs ephedrine in cesarean delivery under spinal anesthesia: A systematic literature review and meta-analysis.

BACKGROUND: In the past 20 years, many studies compared phenylephrine with ephedrine to prevent or treat hypotension in elective or emergency cesarean delivery and parturients with pre-eclampsia. A meta-analysis of the abovementioned trials is needed. METHODS: Several databases (PubMed, Embase, Web of Science and Cochrane Library) were searched from inception to April 2018 for trials comparing phenylephrine with ephedrine in cesarean delivery. The primary outcome is the incidence of maternal hypotension. RESULTS: Thirty-six trials (2439 patients) with elective cesarean delivery, three trials
