Anesthesia for pediatric lithotripsy.

SUMMARY BACKGROUND: We describe 7 years experience of providing anesthesia for children undergoing lithotripsy in a hospital without pediatric inpatient facilities. A pediatric team, including anesthetist, pediatric nurse along with the equipment travel across the city with the patient from the children's hospital. As a high incidence of postoperative vomiting and discomfort was observed, the effect of increasing intraoperative analgesia and the use of antiemetic medication was examined. METHODS: From 1998 to 2004, 69 children (49 boys and 20 girls) were anesthetized for 120 procedures: 67 ex

Can Bispectral Index aid laryngeal mask placement in children?

BACKGROUND: Laryngeal mask airway (LMA) placement requires an adequate depth of anesthesia to prevent patient movement and adverse airway responses. Patient movement, coughing, stridor or laryngospasm at LMA placement may result in transient hypoxia, injury to the patient and prolong time spent in the anesthetic room. The Bispectral Index Score (BIS) is a relatively new tool that has not yet established its place in routine clinical pediatric anesthesia practice. One potential use may be to predict an adequate depth of anesthesia for successful intraoperative interventions such as LMA placemen

Early risk factors in pediatric renal transplantation at a single center.

BACKGROUND/PURPOSE: Renal transplantation is the preferred treatment for renal failure in childhood, but the incidence of graft failure is generally higher than that in adult recipients. A single center was studied to determine if there were any correctable factors that could contribute to graft failure. METHODS: Recipient, donor, and perioperative factors were analyzed using standard statistical tests in 59 pediatric renal transplants performed between 1992 and 1995 using standard cyclosporin-based immunosuppression. RESULTS: Three factors were found to be significantly different between th

Anesthesia and Bartter's syndrome: a case report and review.

A 22-year-old female with Bartter's syndrome presented at 40 weeks' gestation for elective cesarean section under general anesthesia. Her usual medication was ibuprofen and potassium supplements. Preoperative potassium was 3.3 mmol/L, and in anticipation of difficulties in fluid, electrolyte, and acid-base management, a central line and urinary catheter were inserted and blood gases measured. In the first 20 hours postdelivery she had a brisk diuresis and required 3.5 L of crystalloid to maintain her central venous pressure and 100 mmol of potassium to prevent significant hypokalemia. The main

Improving placement of central venous catheters in 22 hospitals in China: a best practice implementation project.

INTRODUCTION: Correct central venous catheter (CVC) placement is important, as inappropriate practices can lead to central line-associated bloodstream infections (CLABSIs), which increase the risk of death as well as imposing a heavy financial burden. OBJECTIVES: This multi-site project was conducted across 22 hospitals in China, and aimed to improve compliance with evidence-based bundles for the placement of CVCs among doctors, anesthetists, and operating room specialized nurses to reduce the rate of CLABSIs. METHODS: This project was guided by the JBI Model of Evidence-based Healthcare and…

Neuraxial anesthesia in obstetric patients with rare spinal pathologies: a scoping review.

BACKGROUND: Neuraxial anesthesia remains the primary approach for obstetric patients, emphasizing the need for a successful blockade. However, many anesthetists view spinal pathologies as contraindications for regional anesthesia. The existing literature on the use of neuraxial anesthesia in obstetric patients with rare spinal pathologies is largely limited to case reports and case series. The goal of this investigation is to evaluate the success rates and complications associated with neuraxial anesthesia in these obstetric patients. METHODS: We searched the databases: PubMed, Embase, Scopus…

Comparison of the recovery profile of sufentanil and remifentanil in total intravenous anesthesia: a systematic review and meta-analysis of randomized controlled trials.

INTRODUCTION: Remifentanil is a short-acting opioid and can be administered during surgery without the risk of delayed postoperative recovery but concerns about hyperalgesia and the shortages of remifentanil lead anesthetists to consider long-acting opioids for Total Intravenous Anesthesia (TIVA). Sufentanil is a more potent opioid with a longer context-sensitive half-life but can promote good postoperative analgesia due to its residual effect. This meta-analysis aimed to compare the recovery profile of remifentanil and sufentanil for TIVA. METHODS: The search strategy was performed in PubMed…

Outpatient treatment of abdominal wall hernia. A strategy to reduce the impact of the COVID-19 pandemic.

AIM: The aim of this study is to demonstrate the feasibility and efficacy of the treatment of abdominal wall hernias in ambulatory setting in selected patients to break down long waiting lists due to the COVID 19 pandemic. METHODS: From February to June 2021, we performed 120 hernia repair operations with local anesthesia in ambulatory settings without anesthetists. (105 inguinal hernia, 6 femoral hernia and 9 umbilical hernia). All patients were selected from our waiting lists first through a telephone interview through an adequate collection of the anamnesis and then clinically (LEE index a…

Effect of general anesthesia on delayed repeat enema in pediatric intussusception.

BACKGROUND: The study aimed to assess the efficacy and safety of delayed repeated enema (DRE) and to evaluate the effect of general anesthesia (GA) on DRE. METHODS: A retrospective analysis was conducted of all children below 3 years with primary intussusception who were offered DRE in our tertiary center, from 2014 until 2019. Following a standardized pneumatic enema protocol, those who showed a partially successful result were offered DRE 2 h later, either awake (Group A) or under GA (Group B). DRE under GA was our preferred procedure unless there were no pediatric anesthetists available at…

Effect of 4% nebulized lignocaine versus 2% nebulized lignocaine for awake fibroscopic nasotracheal intubation in maxillofacial surgeries.

INTRODUCTION: Securing a difficult airway during maxillofacial surgeries is a great challenge for anesthetists, and the flexible fiber-optic bronchoscope is the gold standard while managing such cases. While passing the flexible bronchoscope by the nasal route, the success rate is higher as compared with oral approach as the nasopharynx is in line with the larynx and prevents acute angulation in the oropharynx. MATERIALS AND METHODS: A randomized control trial was planned in 73 patients out of whom sixty patients gave consent for the procedure. The patients we randomly divided into two groups…
