Application of a fasciocutaneous free flap for treatment of a severe soft tissue injury of the foot and ankle: a case report.

UNLABELLED: The authors report an unusual case wherein a fasciocutaneous free flap from an amputated upper limb was used to repair a severe soft tissue injury of the ipsilateral forefoot and ankle. After amputating the nonviable portions of the forefoot, a residual limb flap from the patient's forearm, pedicled with the brachial artery, was used to cover the lower extremity defect. Three years after the injury, the patient was able to maintain balance and ambulate without assistance on the reconstructed lower extremity. LEVEL OF CLINICAL EVIDENCE: 4.

Peroneal nerve injury with foot drop complicating ankle sprain--a series of four cases with review of the literature.

Foot drop has many etiologies. One rarely mentioned and often neglected reason for foot drop is an acute inversion sprain of the ankle. Over the past 14 years, a collection of 32 cases of foot drop have been compiled in our orthopaedic and physiatric practices. All cases had appropriate evaluations, including electrodiagnostic studies (electromyography and nerve conduction studies) to determine the location and type of injury. Treatment and follow-up are also discussed. Of the 32 case studies, four were caused by a straightforward acute inversion sprain of the ankle. These cases are described

The effect of selective tibial neurotomy and rehabilitation in a quadriplegic patient with ankle spasticity following traumatic brain injury.

Ankle spasticity following brain injury leads to abnormal posture and joint contracture; making standing or walking impossible. This study investigates the efficacy of selective tibial neurotomy (STN) and intensive rehabilitation in a patient who suffered ankle spasticity after brain injury. This case describes a 37-year-old man whose traumatic brain injury (TBI) resulted in severe right ankle spasticity and contracture. He was unable to stand due to severe right ankle spasticity and contracture. Intensive rehabilitation and STN allowed him to walk without brace at 6 months and run at 12 month

Anatomical reconstruction of chronic lateral ligament injury of the ankle using pedicle tendon of the extensor digitorum longus.

BACKGROUND: Since 1989, we have performed ligament reconstruction using the extensor digitorum longus tendon as a pedicle graft for patients with chronic lateral instability of the ankle. METHODS: Of those patients who underwent arthroscopy during the operation, thirteen joints were evaluated at an average follow-up of 7 years and 2 months. The overall average postoperative score was 95.4 points on Karlsson's score for the ankle ligament. Instability scores improved by 8.5-24.6 points. Our anatomical reconstructive surgery showed generally excellent functional results, with cosmetic advantage

Velocity-dependent ankle torque in rats after contusion injury of the midthoracic spinal cord: time course.

Progressive neurophysiological changes in the excitability of the pathways that subserved ankle extensor stretch reflexes were observed following midthoracic contusion. The purpose of the present study was to determine the nature and time course of velocity-dependent changes in the excitability of the ankle stretch reflex following T(8) contusion injury. These studies were conducted in adult Sprague-Dawley rats using a 10-g 2.5-cm weight drop onto the exposed thoracic spinal cord (using an NYU injury device and a MASCIS protocol). Velocity-dependent ankle torques and triceps surae EMGs were me

Serial plaster casting to correct equino-varus deformity of the ankle following acquired brain injury in adults.

PURPOSE: Proposed mechanisms via which serial casting might effect increased joint range, muscle extensibility and reduced reflex excitability are outlined in this review. Support for these mechanisms stems largely from animal experimental studies. The applicability of these data to human muscle is unknown. ISSUES: Equino-varus deformity of the ankle is a common secondary complication of acquired brain injury. It results from a combination of sequelae of the brain injury and subsequent immobility, including hypertonia, reduced muscle length and increased stiffness. Some evidence exists for th

Clinical outcomes of an olive wire and external fixator versus a metallic screw in the treatment of inferior tibiofibular syndesmosis injuries in ankle fractures.

BACKGROUND: Metal screws are the most widely used in treating syndesmotic injuries, but failure and the rigidity of the screws can threaten the success of the treatment and increase the cost of care. We have provided an alternative with an olive wire and external fixator(OWEF) used for syndesmotic fixation. METHODS: A retrospective longitudinal follow-up study was conducted. From February 2011 to January 2018, 58 of 72 patients with ankle fractures and associated syndesmotic disruption were treated with either screw or OWEF fixation. The costs, complications, and clinical outcomes using Oleru

Fewer reoperations after posterolateral plate positioning compared with lateral plate positioning in ankle fractures-a retrospective study on 453 AO/OTA 44-B injuries.

INTRODUCTION: Ankle fractures are commonly treated by open reduction and internal fixation with plate and screws. Unplanned return to theatre is common, in many cases to extract prominent osteosynthesis material from the lateral malleolus as swelling subsides. We hypothesised that patients operated with a posterolateral plate placement on the lateral malleolus would have fewer reoperations, and fewer complications, compared to patients with a lateral plate placement. MATERIALS AND METHODS: From a prospectively collected database of all orthopaedic surgery performed at our institution, we iden

Investigating the effects of conventional thermoplastic ankle-foot and the neoprene ankle-foot orthoses on the kinetics and kinematics of gait in people with foot drop following traumatic injury of the peroneal nerve: A pilot study.

BACKGROUND: Adopting compensatory walking mechanisms by people with foot drop due to traumatic injury of the peroneal nerve costs altered gait kinetics and kinematics. Therefore, orthoses are generally recommended to minimize the deployment of compensatory gait mechanisms. OBJECTIVES: To investigate the immediate effects of a low-cost, Neoprene Ankle-Foot Orthosis (NAFO) and the thermoplastic ankle-foot orthosis (AFO) with the shoe-only condition on kinematics and kinetics of gait of people with foot drop following peroneal nerve traumatic injury. METHODS: Seven people with foot drop due to

Epidemiology of ankle sprains and anterior cruciate ligament injuries in youth basketball athletes in Niigata, Japan: A regional survey on injury management and healthcare professional involvement.

OBJECTIVE: Ankle sprains are highly prevalent among youth basketball players, particularly in the under-15 age group, and inadequate post-injury management may contribute to the development of chronic ankle instability and may be associated with an increased risk of anterior cruciate ligament injuries. This study aimed to determine the incidence of ankle sprains and post-injury management among under-12 and under-15 basketball players through a large-scale questionnaire survey. Additionally, we examined the prevalence of chronic ankle instability and anterior cruciate ligament injuries to asse

Comparative results of conservative treatments for isolated anterior talofibular ligament (ATFL) injury and injury to both the ATFL and calcaneofibular ligament of the ankle as assessed by subtalar arthrography.

BACKGROUND: There have been no reports describing the results of conservative treatment of acute lateral ligament injury of the ankle in detail in terms of the severity of the injury, and the results of conservative treatment for injury with severe instability are still controversial. The purpose of this study was to assess the results of nonoperative treatment of acute lateral ligament injury according to its severity. METHODS: Fifty-five consecutive acute lateral ankle ligament injuries in 54 patients who were treated nonoperatively were followed up as a prospective study. Twenty-seven were

Preoperative CT parameters to predict tibiofibular syndesmosis injury associated with ankle fracture: a propensity score-matched analysis.

BACKGROUND: Untreated ankle fractures with concomitant tibiofibular syndesmosis injury often lead to postoperative pain and early traumatic arthritis. CT has advantages in the preoperative diagnosis of combined ankle injuries. However, a few studies have investigated the best preoperative CT parameters to predict tibiofibular syndesmosis injuries associated with ankle fractures. This study aimed to identify and evaluate the optimal preoperative CT parameters for predicting tibiofibular syndesmosis injuries associated with ankle fractures. METHODS: We retrospectively analyzed 129 patients who …

How Common is Subsequent Posterior Tibial Tendon Dysfunction or Tarsal Tunnel Syndrome After Ankle Sprain Injury?

Posterior tibial tendon dysfunction (PTTD) and tarsal tunnel syndrome (TTS) are debilitating conditions reported to occur after ankle sprain due to their proximity to the ankle complex. The objective of this study was to investigate the incidence of PTTD and TTS in the 2 years following an ankle sprain and which variables are associated with its onset. In total, 22,966 individuals in the Military Health System diagnosed with ankle sprain between 2010 and 2011 were followed for 2 years. The incidence of PTTD and TTS after ankle sprain was identified. Binary logistic regression was used to ident

The coordination patterns of the foot segments in relation to lateral ankle sprain injury mechanism during unanticipated changes of direction.

Preventing lateral ankle sprain injuries (LAS) in females competing in court sports is a high priority, as an athlete's risk for re-injury and developing long term dysfunction increases significantly after sustaining an acute LAS. Stability to the ankle joint is passively provided by the joint congruity and ligaments, and actively by the muscles acting on the foot. The ankle joint is most stable when loaded and dorsiflexed. However, during unanticipated changes of direction, typical in court sports, the foot is often in a vulnerable unloaded, plantarflexed position. Stability of the forefoot a

Bilateral and asymmetrical contributions of passive and active ankle plantar flexors stiffness to spasticity in humans with spinal cord injury.

Spasticity is one of the most common symptoms present in humans with spinal cord injury (SCI); however, its clinical assessment remains underdeveloped. The purpose of the study was to examine the contribution of passive muscle stiffness and active spinal reflex mechanisms to clinical outcomes of spasticity after SCI. It is important that passive and active contributions to increased muscle stiffness are distinguished to make appropriate decisions about antispastic treatments and to monitor its effectiveness. To address this question, we combined biomechanical and electrophysiological assessmen
