Psychophysical spectral sensitivities were obtained from aphakic human subjects in the photopic range. The foveal spectrum has a primary peak near 550 nm and a secondary peak about 0.7 log units lower around 380 nm. Off fovea, beyond most of the macular pigment, the curve is quite broad. While the peak is in the green, the gradual drop-off to 330 nm varies over only half a log unit. The two curves were compared in order to obtain an estimate of the macular pigments' spectrum. Five cases of delayed nonexpulsive suprachoroidal hemorrhage after trabeculectomy in aphakic eyes are reported. Four cases were done under general anesthesia and in three there was straining and bucking at extubation. The most common presentation was sudden severe ocular pain one day postoperatively, associated with marked decrease in vision and low intraocular pressure. To examine the long-term stability of epikeratophakia and its effect on the development of young eyes, the change in corneal curvature and refractive error over time were reviewed in pediatric patients who underwent epikeratophakia for optical correction of aphakia. We used epikeratophakia for rehabilitation in 11 contact lens-intolerant aphakic children aged 9 months to 14 years. Minimum follow-up was five months; the longest was 12 months. There were no graft failures, and no patients were unavailable for follow-up. Objective postoperative overrefractions on all patients revealed that an average of 93% of the desired correction was obtained. Two patients with visual loss after cataract extraction were found to have prominent pallid optic disc edema, peripapillary hemorrhage, and cystoid maculopathy. A diagnosis of anterior ischemic optic neuropathy, with coincidental aphakic cystoid macular edema, was initially considered. 65 aphakic patients (89 eyes) wore extended wear contact lenses for an average period of 22.8 months. 74% of the patients developed one or more readily detectable ocular changes during the course of the study. 72 unselected patients wearing aphakic spectacle correction were examined more than 6 months postoperatively. Approximately one third had poor adaptation in spite of satisfactory acuity. No correlation with age was found in the 60-85 year range. In the last four years we have treated with the mobile argon-laser beam 25 cases of aphakic pupillary block glaucoma (p. b. g.). According to the time of manifestation of the p. b. g. the patients were divided into an "early" and a "late" group.
