The authors describe a case of accidental catheterization of the subdural extra-arachnoid space during epidural analgesia for labour. The epidural catheter had been inserted at the L3-L4 interspace without any problem. A severe hypotension occurred 90 min after the onset of analgesia. A T4 upper sensory level was associated with a complete motor blockade. Total spinal anaesthesia was suspected but ruled out because of delayed onset of analgesia. Extensive epidural anaesthesia was also eliminated as the local anaesthetic dose (15 mL of bupivacaine 0.125%) was not excessive for this patient. Aft

We have found nodules histologically resembling arachnoid villi adjacent to pulmonary venules in a woman of 30 years with primary pulmonary hypertension. Such lesions have been reported previously in cases of mitral stenosis and pulmonary thromboembolism and we believe them to absorb excess water from the interstitial tissues of the alveolar walls, thus protecting the lung from the development of edema. In this way they seem to have a similar function to that of cerebral arachnoid villi and granulations which transport cerebrospinal fluid into the dural sinuses.

Accidental subdural injections and catheterisations are a complication of epidural and spinal anaesthesia. The incidence of subdural spread in myelographies is estimated to be over 10% by the spinal technique. With spinaloscopy in an anatomic human model, we analysed the puncture process and the influence of different needle types on the incidence of subdural injection. We compared 22-gauge Sprotte, Quincke, and 18-gauge Tuohy needles in median and paramedian approaches with various bevel orientations. METHOD. The studies were performed in a preserved and recently expired cadaver donated to th

We experienced a rare case of arachnoid cyst in the right cerebellomedullary cistern. A 59-year-old female was admitted to our clinic because of lower cranial nerve palsy (deviation of uvula to the left side, swallowing disturbance, curtain sign, hoarseness, atrophy of the right sternocleidomastoid muscle, and deviation of the tongue to the right side). MRI demonstrated a mass lesion compressed towards the medulla oblongata in the right cerebellomedullary cistern. MRI was very helpful as a diagnostic tool, since there is no bone object in the posterior fossa. Although by CT cisternography, the

Arachnoid cysts of the quadrigeminal cistern are rare. Patients with this lesion are usually normal at birth, and signs of increased intracranial pressure (IICP) develop at about 4 to 12 months of age. However, there has been no report about this mechanism. A case of arachnoid cyst of the quadrigeminal cistern is presented. In this case, the process of the cyst growth was observed during the course of follow-up for shunted hydrocephalus. Symptoms coincided with the growth of the cyst. Therefore, the development of the clinical symptoms was thought to be due to IICP caused by cyst expansion. Ou

Metrizamide computed tomographic myelography demonstrates well: partial- and full-thickness diastematomyelia; the presence, shape, and (a)symmetry of the two hemicords; focal persistence of two anterior spinal arteries, one supplying each hemicord; the single or double subarachnoid space(s), arachnoid tubes, and dural tubes which encompass the hemicords; the dural septum formed by the medial walls of the two dural tubes; the dural cleft situated between the two walls of the dural septum; the bone spur within the dural cleft; and the presence of any tethering dorsal fibrous bands or aberrant do

Four arachnoïd cysts and two arachnoïd communicating pouchs in sub, retro and supra cerebellar situation are presented. These formations are rarely encountered in adults and distinction is often difficult from other cystic lesions whose structure is different (neuroglial and ependymal cysts). Intellectual slowing, mild ataxia or urinary disturbances, related to chronic hydrocephalus may be the only clinical symptoms. After computed tomography, isotopic cysternography, rather than metrizamide cysternography, must precise the possible communication between the cystic pouch and the subarachnoïd s

We present a case of symptomatic posterior fossa arachnoid cyst (PFAC) that was successfully treated with endoscopic surgery. A woman in her 60s presented with progressive lightheadedness. Brain MRI showed a non-enhancing cystic lesion in the cerebellum that exerted pressure on the fourth ventricle, causing hydrocephalus, which further exacerbated 6 months later. Considering that PFAC was the cause of the patient's symptoms, an endoscopic cystoventriculostomy was performed. Postoperatively, the patient's symptoms improved, and MRI showed reduction of the cyst and improvement of hydrocephalus.

BACKGROUND: Arachnoid cysts account for approximately 1% of all intracranial mass lesions, with suprasellar arachnoid cysts (SACs) representing a rare subset. Hydrocephalus is the most common presentation, occurring in nearly 90% of the cases. Due to their rarity, no universal consensus exists regarding classification, optimal surgical management, or outcome assessment.
OBJECTIVE: To compare outcomes of ventriculocystostomy (VC) and ventriculocystocisternostomy (VCC) in the endoscopic management of suprasellar arachnoid cysts.
METHODS: This retrospective study reviewed patients with SACs who

BACKGROUND: Dorsal arachnoid web (DAW) is a rare spinal pathology involving a thickened band of arachnoid tissue that compresses the thoracic spinal cord, potentially causing myelopathy. Patients with DAW who present with pain or neurological complaints should be evaluated thoroughly to determine the most likely cause of symptoms, particularly in the context of clinical presentations that overlap with other neurological or pain syndromes.
CASE REPORT: A 76-year-old man with a history of inclusion body myositis and prior middle cerebral artery stroke presented with left-sided pain and weakness

This case report describes a 4-year-old Golden Retriever with chronic progressive motor impairment of pelvic limbs. A neurological examination revealed non-painful ambulatory paraparesis, and magnetic resonance imaging detected an eccentric cystic lesion that compressed the thoracic part of the spinal cord. The surgical approach consisted of spinal cord decompression and lesion excision, along with vertebral column stabilization. A neuropathological examination revealed dural and arachnoid hypertrophic calcifying changes with lymphoplasmacytic and neutrophilic infiltration. This is the first d

INTRODUCTION: Sellar arachnoid cysts are rare intracranial lesions with variable clinical presentations, making their optimal management uncertain. This systematic review consolidates current knowledge on their epidemiology, radiological features, surgical management, and outcomes.
MATERIALS AND METHODS: A literature search, following PRISMA-P 2015 guidelines, was conducted in MEDLINE/PubMed, Google Scholar, and Ovid Embase. Studies published in English from the year 2000 onwards were included. Data extraction focused on patient demographics, clinical presentation, surgical approaches, outcom

Foix-Chavany-Marie syndrome, also known as opercular syndrome, is a rare neurological condition typically caused by bilateral lesions of the opercular cortex. Involvement of arachnoid cysts in the pathogenesis is extremely uncommon. We report the case of a 66-year-old male who presented with sudden-onset dysarthria, dysphagia, and bilateral lower facial paralysis, but retained the ability to yawn and smile, demonstrating the characteristic dissociation between voluntary and involuntary orofacial movements. Magnetic resonance imaging revealed an acute infarction in the left corona radiata and a

The conformation of spinal arachnoid diverticula (SAD) and their clinical implications are poorly characterized in dogs. This retrospective cross-sectional study describes different SAD conformations in dogs and aims to identify if there is an association between SAD conformation and clinical features, localization, syringomyelia (SM) presence, concurrent vertebral condition, treatment option, and short as well as long-term outcome. Sixty-two dogs were included (12 cervical and 50 thoracolumbar SAD). All dogs with a cervical SAD had a cranial tethered conformation and were not included in the

Cerebral arachnoid cysts (ACs) are one of the most common and poorly understood types of developmental brain lesion. To begin to elucidate AC pathogenesis, we performed an integrated analysis of 617 patient-parent (trio) exomes, 152,898 human brain and mouse meningeal single-cell RNA sequencing transcriptomes and natural language processing data of patient medical records. We found that damaging de novo variants (DNVs) were highly enriched in patients with ACs compared with healthy individuals (P = 1.57 × 10-33). Seven genes harbored an exome-wide significant DNV burden. AC-associated genes we
