This clinical case describes the communication profile of a functional speech disorder. A 48-year-old woman was admitted to the emergency service due to sudden changes in speech, generalized dystonia and gait ataxia. Magnetic resonance imaging showed multiple supra and infratentorial inflammatory lesions involving the posterior occipito-temporal lobes bilaterally, frontal convexity bilaterally, insula and paramedian frontal cortex on the left, diencephalon, rhombencephalon. On the third day, a speech pathology evaluation was conducted, revealing inconsistencies in speech: repetitions of initia

BACKGROUND: First language acquisition depends on intermodal perception, especially auditive, tactile-kinesthetic, and partly visual in addition to sensomotoric integration. The influence of the vestibular function for a physiological language development is still unknown.
PATIENT: A case history of a child with bilateral aplasia of all semicircular canals, normacusis in the right ear, and severe sensorineural hearing loss in the left (hearing aid supplied), without mental deficiency, allows us to draw initial conclusions.
RESULTS: Logopedic diagnostics revealed only a mild articulation diso

BACKGROUND: The locked-in syndrome is defined as quadriplegia and anarthria (loss of articulate speech) with the preservation of consciousness. It is typically caused by a lesion to the ventral pons. Conversion disorder is the deficit of voluntary motor or sensory function requiring an extensive work-up to exclude any organic cause.
METHODS AND RESULTS: After surgery for an implantation of a spinal cord stimulator, a 42-year-old woman presented with quadriplegia and lower facial diplegia, but was able to open and blink her eyes. We found no organic causes to explain her condition after approp

CONCLUSION: A right-handed 38-year-old man's complete loss of hearing could be diagnosed as cortical deafness caused by cerebral vascular lesions in bilateral auditory cortices.
OBJECTIVE: The aim of this case report was to study the pathophysiology of a particular patient who manifested deafness without residual hearing and lost somatosensory sensation, vestibular sensation, and articulation ability after a right internal carotid-posterior communicating artery (IC-PC) aneurysm and subarachnoidal hemorrhage.
METHODS: MRI, aphasia and neurological tests, subjective and objective audiometry, a

BACKGROUND: The anticipatory proportion (AP), the ratio between perseverative and anticipatory speech errors, is reduced in patients with brain injury. However, it is unknown whether the AP is also reduced in elderly speakers with cognitive impairment.
METHODS: 20 elderly speakers with a Mini Mental State Examination (MMSE) score of 25-27 and 20 elderly speakers with an MMSE score of 28-30 were assessed using a tongue-twister-based speech test, the Regensburg Word Fluency Test (RWT) and an object naming test.
RESULTS: The AP in the group of speakers with an MMSE score of 25-27 was significan

PURPOSE: A range of studies have shown difficulties in perceiving acoustic and phonetic information in dyslexia; however, much less is known about how such difficulties relate to the perception of individual words. The authors present data from event-related potentials (ERPs) examining the hypothesis that children with dyslexia have difficulties with processing phonemic information within spoken words compared to age-matched readers with typical development.
METHOD: The authors monitored ERPs to auditory words during a simple picture-word matching task. The key manipulation was the inclusion

The goal of this study was to gain a better understanding of the relationship between non-verbal auditory disorders and developmental dyslexia. This question has led to conflicting results in the literature, which we argued might be due to a failure to consider the heterogeneity of dyslexic profiles. This study included three groups of adult participants: unimpaired readers and dyslexic readers with or without a phonological deficit. Auditory temporal processing deficits, as measured by stream segregation thresholds, were present in most dyslexic participants with phonological disorders. In co

Recent evidence suggests that the primary source of dysfunction in dyslexia is the cerebellum. To examine the cerebellar deficit hypothesis of dyslexia, 20 children with dyslexia and 20 children without dyslexia were assessed using neuropsychological testing and quantitative magnetic resonance imaging. Results demonstrated that the volumes of both hemispheres and the vermis were not statistically significantly different between groups. However, children without dyslexia demonstrated greater rightward cerebellar hemisphere asymmetry. The relationship between cerebellar morphologic structure and

The purpose of this study was to assess the postoperative functioning of oral cancer patients with resections of the anterior tongue and floor of mouth, reconstructed with distal flap closure. Speech and swallowing performance was assessed for 11 men and 5 women preoperatively and at 1 and 3 months postoperatively following a standardized protocol. Speech tasks included an audio recording of a brief conversation and of a standard articulation test; swallowing function was examined using videofluoroscopy. Data were also collected on the number and duration of speech/swallowing treatment session

Augmentative and alternative communication (AAC) aids were used in three young, intellectually normal patients with dystonia musculorum deformans (DMD) who had severe speech and writing impediments. These aids included speech therapy, communication boards, and voice synthesizers for verbal communication and typewriters, memowriters, and computer software and printers for written communication. At times customized accessing was needed which required specific adaptive modifications. Implementation of the AAC aids system was determined effective for DMD patients in view of the intellect-sparing n

Dislocation of the temporomandibular joint (TMJ) is most often spontaneous, but it can be associated with direct or indirect head and neck trauma. The disorder may be treated in general by simple closed techniques, if managed acutely. If the dislocation becomes chronic, however, open reduction is usually required. The article discusses various methods for an open surgical approach described in the literature. A case of a prolonged TMJ dislocation is presented, involving a severe bilateral mandibular dislocation with the condyles displaced into the infratemporal fossa and the lateral poles of t

This paper reports on some preliminary aspects of a collaborative cross-linguistic study of normal and disordered Japanese and British English speech. The investigation compares lateralised productions of parallel s which are abnormal in the two languages. EPG and acoustic recordings were made of four Japanese and four British subjects. The EPG patterns were classified according to certain criteria, such as the presence or absence of complete constriction between the tongue and the hard palate, and the area and location of this contact. Findings revealed that lateralised articulations varied b

We recently showed genomewide linkage of centrotemporal sharp waves (CTS) in classic Rolandic epilepsy (RE) families to chromosome 11p13, and fine-mapped this locus to variants in the ELP4 gene. Speech sound disorder (SSD) is a common comorbidity in RE subjects, of unknown etiology, which co-aggregates in family members in a manner that could hypothetically be explained by shared underlying genetic risk with CTS. Furthermore, the neural mechanism of SSD is unknown, although individuals with rare, Mendelian forms of RE are described with severe verbal and oromotor apraxia. We therefore first pe

OBJECTIVE: This study was designed to examine the relationship between reason for referral and subsequent DSM-III-R diagnosis in institutionalized individuals with severe to profound mental retardation. A heavy emphasis was placed on articulating how diagnostic criteria are applied in this population.
METHOD: The study population consisted of 251 patients consecutively referred for initial psychiatric consultation from a large series of institutionalized patients with predominately severe to profound mental retardation. On the basis of the chief complaint, subjects could be grouped into six o

BACKGROUND: The symptoms and their progression in amyotrophic lateral sclerosis (ALS) are typically studied after the diagnosis has been confirmed. However, many people with ALS already have severe dysarthria and loss of adequate speech at the time of diagnosis. Speech-and-language therapy interventions should be targeted timely based on communicative need in ALS.
AIMS: To investigate how long natural speech will remain functional and to identify the changes in the speech of persons with ALS.
METHODS & PROCEDURES: Altogether 30 consecutive participants were studied and divided into two group

Muenke syndrome is an autosomal dominant craniosynostosis syndrome resulting from a defining point mutation in the Fibroblast Growth Factor Receptor3 (FGFR3) gene. Muenke syndrome is characterized by coronal craniosynostosis (bilateral more often than unilateral), hearing loss, developmental delay, and carpal and/or tarsal bone coalition. Tarsal coalition is a distinct feature of Muenke syndrome and has been reported since the initial description of the disorder in the 1990s. Although talocalcaneal coalition is the most common tarsal coalition in the general population, it has never previously

Observing a speaker's articulations substantially improves the intelligibility of spoken speech, especially under noisy listening conditions. This multisensory integration of speech inputs is crucial to effective communication. Appropriate development of this ability has major implications for children in classroom and social settings, and deficits in it have been linked to a number of neurodevelopmental disorders, especially autism. It is clear from structural imaging studies that there is a prolonged maturational course within regions of the perisylvian cortex that persists into late childho

Foreign accent syndrome (FAS) is a speech disorder that is defined by the emergence of a peculiar manner of articulation and intonation which is perceived as foreign. In most cases of acquired FAS (AFAS) the new accent is secondary to small focal lesions involving components of the bilaterally distributed neural network for speech production. In the past few years FAS has also been described in different psychiatric conditions (conversion disorder, bipolar disorder, and schizophrenia) as well as in developmental disorders (specific language impairment, apraxia of speech). In the present study,

This article seeks to formulate a situated approach to mental disorder that overcomes some of the problems of contemporary diagnostic psychiatry. A framework is needed that aims to integrate neuroscientific knowledge about the brain and other aspects of the person with knowledge about the environment. Inspired by the work of researchers such as Thomas Fuchs, Jerome Wakefield, and Dorte Gannik, I articulate four basic principles for a theory of psychopathology as situated, which hopefully point in this direction. These principles state that a theory of psychopathology as situated is relational;

There are numerous abnormalities that present with similar signs and symptoms to arthritis. In this article some of these conditions that can masquerade as arthritis are discussed. Synovial osteochondromatosis is an uncommon benign disorder marked by the metaplastic proliferation of multiple cartilaginous nodules in the synovial membrane of the joints, bursae, or tendon sheaths. Pigmented villonodular synovitis, also known as diffuse-type tenosynovial giant cell tumor, is a locally destructive fibrohistiocytic proliferation, characterized by many villous and nodular synovial protrusions which
