BACKGROUND: Friedreich's ataxia is a rare, neurodegenerative, multisystem disorder. While ataxia is a hallmark, non-ataxia signs, including muscle weakness, spasticity, and dysphagia are equally disabling. The Inventory of Non-Ataxia Signs (INAS) is a symptom list transformable to a 16-item count.
OBJECTIVE: To evaluate the responsiveness of a modified INAS in this population.
METHODS: Participants were drawn from the European Friedreich's Ataxia Consortium for Translational Studies (EFACTS). The modified INAS count (presence/absence, 0-16 scale) and modified INAS sum (severity-weighted, 0-8…

OBJECTIVE: Although the diagnosis of inherited ataxias is ultimately genetic, this usually means an extensive and expensive process. This justifies the search for distinct clinical signs that may potentially help orient molecular diagnosis.
METHODS: We explored the vestibulo-ocular reflex (VOR) with the video Head Impulse Test in patients diagnosed with spinocerebellar ataxia (SCA) type 3 (n = 15), type 1 (n = 4) and type 2 (n = 4), Friedreich's ataxia (FA) (n = 9) and healthy controls (n = 40). We estimated the latency, regression (VORr) and instantaneous VOR gain at 40, 60 and 80 ms (VOR40,…

BACKGROUND: Spinocerebellar ataxia (SCA) is a heterogeneous group of neurodegenerative disorders. The mutational basis for most of these disorders is an expanded CAG repeat sequence within the coding regions of the genes involved. The prevalence of SCA in the ethnic Chinese on Taiwan remains unclear. Moreover, there has been no report of SCA type 6 (SCA6) among Chinese people.
OBJECTIVES: To characterize the prevalence of SCA in the ethnic Chinese on Taiwan, and to specifically characterize Chinese patients with SCA6 in terms of clinical and molecular features.
PATIENTS AND METHODS: Using a …

The spectrum of autoimmune movement disorders is very broad with regard to clinical presentations, associated antibodies, and underlying immunopathophysiology. Early and adequate immunotherapy is key in the management to achieve good outcomes, avoid deterioration, or even death. The rarity of these disorders, however, entails a lack of gold-standard treatment trials we could base our management decisions upon. Here, we summarize existing knowledge about the treatment of autoimmune movement disorders and cerebellar ataxias with neuronal antibodies. We start with a brief introduction of the basi…

BACKGROUND: ATP7B is the gene that codes copper-transporting ATPase 2, whose homozygous variant causes Wilson disease, typically presenting with hepatic dysfunction and neurological manifestations. Although monoallelic ATP7B variant carriers are considered largely asymptomatic, recent observations suggest they may exhibit atypical movement disorders, including cerebellar ataxia. OBJECTIVE: To investigate the prevalence of monoallelic ATP7B variants in patients with various movement disorders including ataxia and to characterize associated clinical and neuroimaging findings. METHODS: This singl…

Late-onset neurodegenerative diseases have long been conceptualized as disorders arising from cumulative cellular stress and age-related decline, with pathology emerging at the time of symptom onset. However, emerging evidence challenges this view, suggesting that developmental perturbations may establish early vulnerabilities that predispose specific neuronal populations to degeneration later in life. In the cerebellum, mutations causing spinocerebellar ataxias (SCAs) such as SCA1 and SCA6 affect genes involved in normal circuit formation, resulting in subtle early abnormalities in Purkinje c…

Ataxia-Telangiectasia (A-T) is a rare neurodegenerative multisystem disease caused by mutations in the A-T Mutated (ATM) gene resulting in cerebellar ataxia, immunodeficiency and an increased cancer risk. Copper and zinc play important roles in similar cellular processes as the ATM protein, such as cell growth, apoptosis, and oxidative stress. This study aimed to explore copper and zinc levels in individuals with A-T since imbalances in these trace elements may contribute to the clinical features commonly seen in A-T and therefore be a target for novel therapies; furthermore we aimed to assess…

A 70-year-old woman presented with slowly progressive mobility difficulty and involuntary jerking movements, which had started at age 55 years. On examination, she had generalised action-induced myoclonus and gait ataxia. Extensive investigation over 10 years had not given a diagnosis. Genetic testing was re-visited and she was found to carry a likely pathogenic variant and a variant of unknown significance in the neuraminidase gene. Dermal fibroblast culture identified significantly reduced neuraminidase activity in keeping with a diagnosis of late-onset sialidosis. Taking into account bioche…

OBJECTIVE: Frequent and objective assessment of ataxia severity is essential for tracking disease progression and evaluating the effectiveness of potential treatments. Wearable-based assessments have emerged as a promising solution. However, existing methods rely on inertial data features directly correlated with subjective and coarse clinician-evaluated rating scales, which serve as imperfect gold standards. This approach may introduce biases and restrict flexibility in feature design. To address these limitations, this study introduces a novel contrastive learning-based model that leverages …

OBJECTIVE: This study explores the utility of various evoked fields in elucidating the pathophysiology of Friedreich's ataxia (FA) and potentially contributing to developing more targeted diagnostic and therapeutic strategies.
METHODS: Thirty-seven patients with FA aged 27.6 ± 7.4 years and a control group of 17 healthy subjects were enrolled in the study. The neuromagnetic response to auditory, tactile, visual, somatosensory, auditory and tactile oddball stimulation were acquired. For all the components of interest, latency and amplitude were measured and correlated with clinical data.

OBJECTIVE: Spinocerebellar ataxias (SCA) are hereditary cerebellar degenerative disorders with a common feature of dysarthria, involving impaired phonatory and articulatory control of speech, thereby affecting social communication. In this study, we investigated whether acoustic measures could objectively measure speech dysfunction and identify common indicators across SCA types 1, 2, and 3.
METHODS: We recorded speech from 24 SCA patients and 24 age-matched controls during connected speech and sustained vowel tasks. Speech was assessed using the Acoustic Voice Quality Index (AVQI) and sub-me…

The incidence of paraneoplastic syndrome (PNS) is on the rise, attributed to the growing detection of antibody modalities in both the serum and cerebrospinal fluid (CSF). PNS can occur as different neurological symptoms. The revised guidelines streamline the diagnostic approach but identifying PNS still requires the detection of neurological manifestations concurrent with cancer, along with the presence of specific PNS autoantibodies.
