Association Between Angiotensin Receptor-Neprilysin Inhibition, Cardiovascular Biomarkers, and Cardiac Remodeling in Heart Failure With Reduced Ejection Fraction.

BACKGROUND: Sacubitril/valsartan (S/V) treatment is associated with reverse cardiac remodeling and reductions in biomarkers reflecting ventricular wall stress and myocardial injury, such as NT-proBNP (N-terminal pro-B-type natriuretic peptide), hs-cTnT (high-sensitivity cardiac troponin T), and soluble suppressor of tumorigenicity 2 (sST2). How longitudinal changes in these biomarkers analyzed collectively are associated with cardiac remodeling in patients with heart failure with reduced ejection fraction treated with S/V is uncertain. METHODS: In a prospective study of S/V in patients with h

Diabetic Atrial Cardiomyopathy: Pathogenesis, Diagnosis, Management, AI-Driven Diagnosis, and Risk Prediction.

BACKGROUND: Diabetic atrial cardiomyopathy, a component of diabetic cardiomyopathy, is increasingly recognized. However, atrial-focused mechanistic and clinical frameworks remain less developed than ventricular paradigms. MAIN BODY: This review synthesizes the pathogenesis of diabetic atrial cardiomyopathy from conventional, multiomics, and translational perspectives. We highlight how diabetes-related metabolic stress, inflammation, gut microbiota dysregulation, electrophysiological remodeling, genetic susceptibility, and epigenetic regulation converge on atrial fibrosis, conduction heterogen

Obstructive sleep apnea is common and independently associated with atrial fibrillation in patients with hypertrophic cardiomyopathy.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is associated with arrhythmias and cardiovascular death. Left atrial enlargement and atrial fibrillation (AF) are considered markers for death due to heart failure in patients with HCM. Obstructive sleep apnea (OSA) is independently associated with heart remodeling and arrhythmias in other populations. We hypothesized that OSA is common and is associated with heart remodeling and AF in patients with HCM. METHODS: We evaluated 80 consecutive stable patients with a confirmed diagnosis of HCM by sleep questionnaire, blood tests, echocardiography, and

Inhibition of the voltage-gated potassium channel Kv1.5 by hydrogen sulfide attenuates remodeling through S-nitrosylation-mediated signaling.

The voltage-gated K+ channel plays a key role in atrial excitability, conducting the ultra-rapid rectifier K+ current (IKur) and contributing to the repolarization of the atrial action potential. In this study, we examine its regulation by hydrogen sulfide (H2S) in HL-1 cardiomyocytes and in HEK293 cells expressing human Kv1.5. Pacing induced remodeling resulted in shorting action potential duration, enhanced both Kv1.5 channel and H2S producing enzymes protein expression in HL-1 cardiomyocytes. H2S supplementation reduced these remodeling changes and restored action potential duration through

Model systems for the discovery and development of antiarrhythmic drugs.

Cardiovascular diseases are the leading cause of mortality worldwide and about 25% of cardiovascular deaths are due to disturbances in cardiac rhythm or "arrhythmias". Arrhythmias were traditionally treated with antiarrhythmic drugs, but increasing awareness of the risks of presently available antiarrhythmic agents has greatly limited their usefulness. Most common treatment algorithms still involve small molecule drugs, and antiarrhythmic agents with improved efficacy and safety are sorely needed. This paper reviews the model systems that are available for discovery and development of new anti

Impact of right atrial enlargement on clinical outcome in patients with atrial fibrillation.

BACKGROUND: Left atrial (LA) remodeling is associated with adverse cardiovascular events, including heart failure (HF) and stroke in patients with atrial fibrillation (AF). However, there are limited data on the value of right atrial (RA) remodeling in this population. We investigated the prognostic role of RA enlargement in patients with non-valvular AF. METHODS AND RESULTS: We analyzed 254 consecutive patients (age = 69 ± 12years, male:female = 165:89, mean left ventricular ejection fraction = 58.0 ± 7.2%) with non-valvular AF who underwent two-dimensional echocardiography from a single cen

Increased lysyl oxidase expression and collagen cross-linking during atrial fibrillation.

The aim of the study is to characterize the signal transduction leading to interstitial fibrosis in the pathogenesis of atrial fibrillation (AF) and atrial remodeling. Samples of the left atrial appendage (LA) from patients with AF showed higher collagen content (73 ± 5 vs. 38 ± 2 μg/mg protein) and 2.5-fold increased collagen crosslinking compared to patients with sinus rhythm (SR). Affymetrix-assays, RT-PCR and western Blot analysis revealed that LA of AF patients are characterized by increased lysyl oxidase (LOX) mRNA (218 ± 42%) and protein (253 ± 11%) expression. This was associated with …

The Association Between Matrix Metalloproteinase-1, -2, -3, -9, and -12 Gene Polymorphisms and Atrial Fibrillation.

Atrial fibrillation (AF) is a prevalent cardiac arrhythmia associated with significant morbidity and mortality. Structural remodeling of the left atrium, particularly myocardial fibrosis, plays a key role in AF pathogenesis. Matrix metalloproteinases (MMPs) are critical regulators of extracellular matrix remodeling and may contribute to atrial fibrosis through genetic variation. This case-control study included 179 patients with AF and 56 controls. Eight polymorphisms across five MMP genes (MMP1, MMP2, MMP3, MMP9, and MMP12) were analyzed using PCR-based methods. Associations between single-nu

[Obstructive sleep apnea and atrial fibrillation - mechanisms, diagnosis and treatment].

Obstructive sleep apnea syndrome is characterized by repeated airway obstructions leading to intermittent hypoxia and sleep fragmentation. It is a major independent risk factor for atrial fibrillation, with a high prevalence of obstructive sleep apnea among patients with atrial fibrillation. The pathophysiological mechanisms include intermittent hypoxia causing sympathetic activation and atrial remodeling, as well as negative intrathoracic pressure fluctuations that promote arrhythmia. Diagnosis relies on polysomnography and standardized tools such as the SCOPER system. Continuous positive air…

De-risking Atrial Fibrillation: Refining Anticoagulation Decision-Making.

Traditional stroke risk assessment in atrial fibrillation relies on clinical scoring tools such as CHA₂DS₂-VASc, which do not capture the full spectrum of individual thromboembolic risk. Contemporary trials have further challenged this traditional approach, highlighting the importance of atrial fibrillation burden and patient-specific factors in guiding therapy. Advanced echocardiographic markers, including left atrial volume index, strain, and appendage morphology, correlate with atrial remodeling, while biomarkers reflecting cardiac stress and inflammation may further refine risk stratificat…

Prediction of left atrial fibrosis with speckle tracking echocardiography in mitral valve disease: a comparative study with histopathology.

BACKGROUND AND OBJECTIVES: Left atrial (LA) fibrosis is a main determinant of LA remodeling and development of atrial fibrillation. However, non-invasive prediction of LA fibrosis is challenging. We investigated whether preoperative LA strain as measured by speckle tracking echocardiography could predict the degree of LA fibrosis and LA reverse remodeling after mitral valve (MV) surgery. SUBJECTS AND METHODS: Speckle tracking echocardiography and LA volume measurements were performed in 50 patients one day before MV surgery. LA tissues were obtained during the surgery, and the degrees of thei

Pathophysiology and Management of Arrhythmias Associated with Atrial Septal Defect and Patent Foramen Ovale.

Atrial septal defects (ASDs) are among the most common of congenital heart defects and are frequently associated with atrial arrhythmias. Atrial and ventricular geometrical remodelling secondary to the intracardiac shunt promotes evolution of the electrical substrate, predisposing the patient to atrial fibrillation and other arrhythmias. Closure of an ASD reduces the immediate and long-term prevalence of atrial arrhythmias, but the evidence suggests that patients remain at an increased long-term risk in comparison with the normal population. The closure technique itself and its timing impacts

Pearls and Pitfalls in Catheter Ablation of Persistent Atrial Fibrillation.

Catheter ablation of atrial fibrillation (AF) has evolved over the past 20 years from being a novel, unproven procedure to a commonly performed procedure. Triggers are important for the initiation of AF and a suitable substrate is important for perpetuation of AF. Remodeling, including electrical and structural remodeling, is common in patients with persistent AF. Therefore, targeting the remodeled atrium is a critical issue during persistent AF ablation. However, ablation outcomes remain suboptimal despite aggressive substrate modification. Empirical linear ablation is not recommended because

Atrial Fibrillation Ablation in Heart Failure with Preserved Ejection Fraction.

Catheter ablation (CA) for atrial fibrillation (AF) in patients with heart failure with preserved ejection fraction (HFpEF) addresses the significant overlap of these conditions, often found in patients with comorbidities such as obesity, diabetes, and hypertension. HFpEF is characterized by diastolic dysfunction, leading to high left atrial pressures and atrial remodeling, which promotes AF. While treatments for HFpEF are limited, recent studies suggest CA can reduce AF burden, improve quality-of-life, and lower hospitalization rates. Current and upcoming randomized trials aim to establish CA…

Electrophysiological properties of the fibrillating atrium: implications for therapy.

1. Atrial fibrillation (AF) is the most commonly occurring cardiac dysrhythmia and remains a challenge to medical therapy. Although the disorder has been recognized for over 100 years, surprisingly very little is understood about its pathophysiology. Over the past decade, a variety of experimental and animal models of AF have been developed and these have provided insights into the mechanism of AF. 2. The pathophysiology of AF is complex. Atrial fibrillation can be caused either by a single source of very rapid impulses or, in the majority of cases, by multiple random re-entering wavelets. The
