BACKGROUND: Nodofascicular and nodoventricular (NFV) accessory pathways connect the atrioventricular node and the Purkinje system or ventricular myocardium, respectively. Concealed NFV pathways participate as the retrograde limb of supraventricular tachycardia (SVT). Manifest NFV pathways can comprise the anterograde limb of wide-complex SVT but are quite rare. The purpose of this report is to highlight the electrophysiological properties and sites of ablation for manifest NFV pathways.
METHODS: Eight patients underwent electrophysiology studies for wide-complex tachycardia (3), for narrow-co…

Radiofrequency catheter ablation has been established as a first line therapy for the curative treatment of patients with atrioventricular nodal reentrant tachycardia and atrioventricular tachycardia encompassing accessory pathways as well as for ablation of the "normal" AV-junction. For these indications, the success rates exceed 90%. Acute complications during ablation of accessory pathway and ablation of the "normal" AV-junction occur in approximately 2-5% of patients treated. The incidence of complications during modification of the atrioventricular node to cure AV-nodal reentrant tachycar…

OBJECTIVE: To compare present pacing practice with the recommendations recently published by the British Pacing and Electrophysiology Group and to assess the increase in annual budget required to implement these recommendations in a regional cardiothoracic unit.
DESIGN: Retrospective analysis of pacemaker implantation for 1991 with calculation of the costs required to implement the group's recommendations based on average 1991 costs of the types of pacing generators and electrode leads used.
SETTING: Regional cardiothoracic unit for South West Thames Health Authority.
PATIENTS: 433 consecut

Isolated sinus node dysfunction with its pursuant long-term risk for atrioventricular (AV) conduction disease poses a unique dilemma for proponents of CSP due to paucity of imprimatur guidelines. In such scenarios, the risk and prognosis of iatrogenic AV block is not well elucidated but is a valid concern. We report a case where CSP was complicated by iatrogenic AV block and peculiarly the rare phenomenon of intra-Hisian Wenckebach.

INTRODUCTION: Recent acute studies demonstrated that atrioventricular (AV) node vagal stimulation during atrial fibrillation (AF) decreases the mean ventricular rate, thus improving hemodynamics.
METHODS AND RESULTS: We report a case of a woman with acute heart failure (HF), chronic AF with untreatable ventricular rapid response, in severe hypotensive state due to a cardiogenic shock. The patient underwent left ventricular (LV) pacemaker implantation and received 50 Hz AV node stimulation, delivered through a posteroseptal atrial lead, thus allowing a 100% pacing. Hemodynamics improvements al

Sick sinus syndrome was disclosed in a 77 year-old man after a radiofrequency (RF) linear catheter ablation of cavotricuspid isthmus for persistent common atrial flutter. Prolongation of corrected sinus node recovery time, sinoatrial conduction time and AH interval, as observed after RF ablation, suggested binodal (sinus as well as atrioventricular node) dysfunction. Adams-Stokes syndrome due to sinus arrest was encountered on the day of RF ablation and a permanent DDD pacemaker was implanted consequently. Our experience indicates that careful observation is very important after the RF ablatio…

Adenosine triphosphate (ATP) and adenosine have strong negative chronotropic and dromotropic effects on the mammalian heart. The sensitivity of the sinus node and the atrioventricular node to ATP and adenosine manifests pronounced variability among species. For more than three decades, ATP has been used routinely in Europe in the acute therapy of paroxysmal supraventricular tachycardia. Preliminary clinical trials with adenosine in the United States suggest that this compound may have a similar therapeutic value. The exact mechanisms of action of ATP and adenosine on the mammalian heart are st…

Three cases are presented showing retrograde alternate Wenckebach periods. Retrograde alternate Wenckebach periods were defined as 2:1 ventriculoatrial (VA) block in which the conducted beats showed progressive prolongation of conduction (VA) time. The sequence terminates with two or three successively blocked ventricular beats. In one of the three cases the level of block was identified as being in the atrioventricular (AV) node. In the other two the level of block could not be clearly identified. This report provides further evidence for the concept of multilevel block within the AV node. Re

BACKGROUND: The Triangle of Koch (TOK) is an important anatomical region located in the right atrium of the heart. It contains the atrioventricular (AV) node and forms a crucial landmark during electrophysiological and cardiac surgical procedures. Variations in its boundaries and morphology may increase the risk of inadvertent injury to the cardiac conduction system. The present study aimed to evaluate the morphology (shape, boundaries, and anatomical variations) of the TOK in thirty donated human hearts and its clinical implications.
MATERIALS AND METHODS: Thirty formalin-fixed adult human h

AIMS: Accurate determination of intra-atrial conduction velocity (CV) is essential to identify arrhythmogenic areas. The most optimal, commonly used, estimation methodology to measure conduction heterogeneity, including finite differences (FiD), polynomial surface fitting (PSF), and a novel technique using discrete velocity vectors (DVV), has not been determined. We aim (i) to identify the most suitable methodology to unravel local areas of conduction heterogeneities using high-density CV estimation techniques, (ii) to quantify intra-atrial differences in CV, and (iii) to localize areas of CV

OBJECTIVE: To determine the clinical and electrophysiological characteristics of patients with paroxysmal palpitations and neck pounding during sinus rhythm.
METHODS: Clinical, electrocardiographic, and electrophysiological characteristics of six patients with paroxysmal palpitations and neck pounding during sinus rhythm were studied in basal conditions and when symptomatic. Response to treatment was observed.
RESULTS: Baseline ECGs were normal (four patients) or had first degree atrioventricular block with intermittent PR shortening. During symptoms, narrow QRS rhythms were seen without vis

Supraventricular tachycardia due to accessory atrioventricular connections in infants is usually well-tolerated. Rarely an infant can have supraventricular tachycardia that is incessant and refractory to medical therapy. We describe a patient with supraventricular tachycardia detected prenatally that caused severe cardiac dysfunction and hemodynamic instability after birth.

AAI pacing offers better hemodynamic characteristics than dual-chamber pacing and is the optimal mode for patients with sick sinus syndrome without AV conduction disorders. AAI pacing may be achieved by single-chamber atrial pacing, by programming a dual-chamber pacemaker to the AAI mode, or by programming a dual-chamber pacemaker to DDD mode with a long AV delay. The annual incidence of AV block development in patients with sick sinus syndrome is low, probably 1-5%, but there is no method of detecting patients immune or prone to future development of AV block. Chronotropic incompetence is oft
