A study was undertaken to determine how well contacting fracture fragments of composite bone replicated the behavior of fracture fragments in real bone. Ten composite and ten real humeral diaphyses were transected and reconstructed with limited-contact dynamic-compression plates. Two screws were placed on each side of the transection site and a calibrated electronic sensor sheet was placed between the imitated fracture fragments. After insertion of the distal screws, pressure measurements were made during insertion of the first proximal screw in compression mode, during insertion of the second

To promote a quicker return to function, an increasing number of patients are treated with headless screws for acute displaced and even non-displaced scaphoid fractures. Therefore, it is imperative to understand and optimize the biomechanical characteristics of different implants to support the demands of early mobilization. The objective of this study was to evaluate the biomechanical fixation strength of 4 headless compression screws under distracting and bending forces. The Acutrak Standard, Acutrak Mini, Synthes 3.0, and Herbert-Whipple screws were tested using a polyurethane foam scaphoid

BACKGROUND: Distal radius fractures continue to show significant complication rates after operative treatment with locked plating. Failure occurs by screw loosening or screw penetration in the distal fragment. Placement of additional screws may enhance the stiffness of fracture fixation. The aim of this study was to determine the fatigue properties of different screw configurations in distal radius plate osteosynthesis with biomechanical tests and finite element analysis (FEA).
MATERIAL AND METHODS: Unstable distal radius fractures were created in 12 human cadaveric bone specimens and were fi

Pedicle screw stability is poor in osteopenic vertebrae attributable, in part, to low screw-bone interface strength. The current authors examined cement augmentation using a low curing temperature hydroxyapatite and bis-phenol-A glycidol methacrylate-based composite resin. This cement may stiffen the screw-bone interface and reduce the harmful effects associated with polymethylmethacrylate regarding temperature and toxic monomer. Thirty-five lumbar vertebrae from human cadavers were instrumented with pedicle screws, with one pedicle previously injected with cement and the other as the control.

Proximal phalanx (P1) fractures in horses are relatively common, and present significant treatment challenges, especially when the fractures are comminuted or infected. An innovative treatment approach includes attaching an external fixator to the third metacarpal bone (MC III), the healthy bone above fracture, to offload the injured P1 and protect it from load-bearing forces, particularly during post-surgical standing up. This study aims to develop the favourable mathematical and numerical models for screws configuration in this external fixator. Nine configurations (I-IX), varying in screw a

The use of miniplates and screws in elective osteotomies or acute trauma of the craniofacial skeleton has revolutionized this field. In jaw surgery, rigid fixation has eliminated the need for intermaxillary fixation in most cases. Bone grafts and bone segments are more easily kept in place by screws or plates, rather than the balancing act that had to be done with wire fixation. This has decreased morbidity and the risks of postoperative airway obstruction. Thus, more complex procedures can be done and younger children treated. Most craniofacial surgeons now using plates and screws are finding

The objectives of this study were (1) to determine the most advantageous screw locations within the humeral head when plate and screw fixation is to be used and (2) to determine the effect of positioning the screw tip abutting the subchondral bone. Ten paired humeral heads were harvested with a monoplanar cut through the anatomic neck. Through use of a standardized template, 7 holes were drilled and tapped in each specimen for insertion of 6.5-mm fully threaded cancellous screws perpendicular to the plane of the cut. Paired specimens were randomized into 2 groups, one with the screw purchase i

AIM: The authors report experience with 14 cases where two screws or "double insurance" screws were used for transfacetal fixation of each joint for stabilization of the lumbar spinal segment. The anatomical subtleties of the technique of insertion of screws are elaborated.
MATERIALS AND METHODS: During the period March 2011 to June 2014, 14 patients having lumbar spinal segmental instability related to lumbar canal stenosis were treated by insertion of two screws into each articular assembly by transfacetal technique. After a wide surgical exposure, the articular cartilage was denuded and bo

PURPOSE: To measure the interfragmentary compression generated across a simulated fracture in cadaveric scaphoids by 3 different headless compression screws.
METHODS: A transverse osteotomy was made through the waist of each scaphoid and a load cell to measure compression was interposed between the fragments, which were then fixed internally retrograde with either an Acutrak Standard (n = 10), Acutrak Mini (n = 12), or Bold (n = 10) screw. The surgeon was blinded to the measured compression, which was recorded during screw insertion and for the following 5 minutes. As a measure of scaphoid bo

BACKGROUND: Minimally invasive cochlear implant surgery by using a microstereotactic frame demands solid connection to the bone. We aimed to determine the stability of commercially available orthodontic miniscrews to evaluate their feasibility for frame's fixation. In addition, which substitute material most closely resembles the mechanical properties of the human temporal bone was evaluated.
METHODS: Pull-out tests were carried out with five different types of orthodontic miniscrews in human temporal bone specimens. Furthermore, short fiber filled epoxy (SFFE), solid rigid polyurethane (SRPU

BACKGROUND: The high incidence of scapular notching in reverse total shoulder arthroplasty (RTSA) has spurred several methods to minimize bone loss. However, up to 93% of RTSAs accompanying scapular notching have been reported to maintain good implant stability for over 10 years. This study was conducted to investigate the relationship between scapular notching and glenoid fixation in RTSA.
METHODS: Cadaveric testing was performed to measure the notch-induced variations in strain on the scapular surface and micromotion at the bone-prosthesis interface during arm abductions of 30°, 60°, and 90

OBJECTIVES: Proximal humerus fractures are commonly treated with open reduction and internal fixation with periarticular locking plates. This study compared the geometry of proximal humerus locking plate's screw distribution, amount of bone-screw interface, and the volume the screws occupy within the humeral head, as well as how leaving the screws short of the articular surface affects these measures.
METHODS: Locking plates from 7 manufacturers were applied to foam humerus models. The entry and exit hole of each screw trajectory was digitized using a 3-dimensional motion tracking system. A t

BACKGROUND: Tibial tunnel preparation may contribute to improved soft tissue graft fixation.
HYPOTHESIS: Step dilation produces greater tunnel wall bone volume than does extraction drilling and increases fixation strength. Bioabsorbable interference screw divergence decreases fixation strength, regardless of tunnel preparation method.
STUDY DESIGN: Controlled laboratory study.
METHODS: Twenty porcine tibias were divided into 2 groups of 10 with matching mean apparent bone mineral density. One group received 9-mm-diameter extraction-drilled tunnels, and the other group received 7-mm-diameter

The titanium hollow screw reconstruction plate (THORP) system for reconstruction of lower jaw defects provides a functional stable fixation and is used as a long-term or permanent implant in tumor surgery and in traumatology. The rigid fixation of the head of the screw to the plate, avoiding unphysiologic loads to the bone underneath the plate, and the titanium plasma-coated perforated hollow screws, enabling the development of direct bone-titanium contact as well as the ingrowth of bone into the lumen and perforations, are the major advantages of this system compared to conventional systems.

Stabilization of the occipitocervical junction and fixation of the occiput is challenging. Rigid fixation with plates and screws has reduced dependence on external immobilization and improved fusion rates. The strength of the occipital fixation can be compromised as plates constrain the ability to place occipital screws along the midline. Bilateral contoured rods with medial offset connectors allow placement of 6 occipital screws in the thickest and strongest bone along the midline, while easily connecting to screws in the cervical spine. The purpose of this study is to review complications an

INTRODUCTION: PMMA-augmentation of pedicle screws strengthens the bone-screw-interface reducing cut-out risk. Injection of fluid cement bears a higher risk of extravasation, with difficulty of application because of inconsistent viscosity and limited injection time.
OBJECTIVE: To test a new method of cement augmentation of pedicle screws using radiofrequency-activated PMMA, which is suspected to be easier to apply and have less extravasations.
METHODS: Twenty-seven fresh-frozen human cadaver lumbar spines were divided into 18 osteoporotic (BMD ≤ 0.8 g/cm2) and 9 non-osteoporotic (BMD > 0.8 g

Pedicle or lateral mass screws, which are usually used to fix atlantoaxial instability, increase the risk of vertebral artery (VA) injury in patients with bone or arterial anomalies or osteoporotic bone. Here, we report the use of a unilateral C1 posterior arch screw-C2 laminar screw posterior fixation with a contralateral C1 lateral mass screw for VA preservation in a patient with bow hunter's syndrome (BHS). A 65-year-old male presented with recurrent loss of consciousness in the right rotational and backward-bending head positions for 1 year. Cerebral angiography in the same head position s
