Degloving Injuries of Upper Extremity: A Strategy with Full Thickness Skin Mesh.

Degloving injuries of the upper extremity during work are rare nowadays, because of effective worker protection devices. However, these devastating injuries still occur today during motor vehicle car accidents and surgeons have to be aware of the possibilities of wound coverage for these large, contaminated wounds. We present two cases of degloving injuries of the hand and entire forearm using degloved skin as meshed full thickness skin graft to cover the entire wound. Two patients were admitted to our hospital, presenting large degloving injuries of the entire forearm and hands. Both patients

Current Concepts in Radial Club Hand.

Radial club hand is a complex congenital abnormality of the radial or pre-axial border of the upper extremity. It has a wide range of phenotypes from hypoplasia of the thumb to complete absence of the radius and the first ray. Centralization with tendon transfer is a popular method for maintaining the correct position of radial club hand. On the other hand, various corrections were devised, e.g. radialization after distraction to emphasize the fact that the head of the ulna is positioned under the radial carpal bones and is no longer placed in a slot in the center of the carpus, microvascular

From Vision Changes to Hospice: The Rapid Progression of Orbital Metastasis in Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC) is an aggressive primary liver malignancy with a high potential for metastasis, commonly involving the lungs, bones, and lymph nodes. However, orbital metastasis is an exceptionally rare presentation. This case describes a 73-year-old man with a history of HCC, hypertension, type 2 diabetes with neuropathy, chronic obstructive pulmonary disease (COPD), hepatitis C, liver cirrhosis, and portal hypertension who presented with a two-week history of blurry vision in the left eye, numbness on the left forehead, and a sensation of orbital fullness. He also reported an

A staged approach to managing a combination of a terrible triad injury and an Essex-Lopresti fracture dislocation: an open forearm crush injury-a case report.

BACKGROUND: Open and crushed forearm injury is a complex and rare injury affecting the upper extremity. It results in damage to various structures, including bones, soft tissues, and neurovascular bundles, ultimately leading to functional impairment. Typically, these injuries occur owing to high-energy trauma. CASE PRESENTATION: A 19-year-old South Asian male was seen at our trauma center emergency department following an accident where his arm got entangled in a machine belt. He sustained a serious open injury on his left forearm, spanning from the elbow to the wrist, affecting the proximal

Complex Regional Pain Syndrome: An update.

Complex regional pain syndrome (CRPS) is a perplexing painful syndrome of the extremities usually following a harmful event. It is distinguished in two types, mainly depending on the presence of nerve injury. Although its prevalence may vary depending on social and ethnic factors, middle-aged women seem to suffer most often and the upper limb is the most commonly affected extremity. Apart from pain, which is the dominating feature, the clinical picture unfolds across several domains: sensory, motor, autonomic and trophic. This syndrome develops in two phases, the acute (warm) phase, with the c

The classification systems of stress fractures: a systematic review.

BACKGROUND: Stress fractures of the upper and lower extremity are troublesome overuse injuries in athletes and nonathletes alike. These injuries have a broad spectrum of severity and prognosis. We performed a systematic search of the literature, which revealed multiple classification systems; however, we did not uncover a general system that offered both validated radiographic and clinical parameters. METHODS: A literature search was conducted using Ovid/Medline, Embase, and the Cochrane Library, with publication dates ranging from 1960 to December 2009. Inclusion criteria included all studie

Technique for Rapid Hand Transplant Donor Procurement Through the Elbow.

Background: Hand and distal forearm allotransplantation has advanced over the last 20 years from experimental to a viable treatment option for bilateral upper extremity amputation. Despite widespread growth of this field, there are few technical reports that elaborate the details of donor arm procurement. This article details a technique for rapid donor procurement through the elbow for mid to distal forearm-level hand allograft procurement. Methods: Nine arm procurements were performed on deceased tissue-only donors provided by the local organ procurement organization, including two bilateral

Feasibility of the PERFormance guided fracture Rehabilitation Method (PERFoRM) protocol for upper extremity fractures.

PURPOSE: Upper extremity fractures are increasingly common in Western Europe due to an aging population and rising osteoporosis rates. Treatment approaches vary significantly, influenced by fracture type, bone quality, and patient- surgeon preferences, with limited consensus on optimal rehabilitation. A key challenge is identifying when to initiate safe, early functional rehabilitation, as guidelines lack clarity on progressive mobilization. MATERIALS & METHODS: A prospective observational study of operatively treated proximal humeral and distal radius fractures was performed. Feasibility was

Gastric adenocarcinoma concurrent with paravertebral plasmacytoma: A case report.

Here, we report the case of a 77-year-old male patient who was revealed to have an unsuspected case of gastric adenocarcinoma with paravertebral plasmacytoma following biopsy. Plasmacytoma may be classified into two main groups: Multiple myeloma and plasmacytoma without marrow involvement. It comprises isolated plasmacytoma of the bone and extramedullary plasmacytoma. Extramedullary plasmacytoma (EMP) accounts for 3% of all plasmacytomas; however, ~80% are located in the upper respiratory tract and upper gastrointestinal tract. It occurs extremely rarely in paravertebral areas. Case reports of

Anesthesia for cesarean section in a patient with Holt-Oram syndrome.

Holt-Oram syndrome is a rare genetic disorder affecting the heart and upper limbs (atriodigital dysplasia). The manifestations of the limb defects may vary in severity from subtle carpal bone defects and triphalangeal thumb to digit aplasia and upper extremity phocomelia. Cardiac abnormalities include atrial and/or ventricular septal defects, anomalies in pulmonary venous return and various dysrhythmias. We present the anesthetic management of a parturient with this syndrome who underwent elective cesarean section and tubal ligation, conducted under combined spinal-epidural anesthesia with a l

Observations of the blood supply to the fibula.

In the surgery of upper and lower extremities autotransplantation of the fibula is often used to bridge extensive bone defects. However, insufficient blood supply affects the long-term results. Microsurgery yields better results. We investigated the blood supply of the fibula to guarantee a careful operation method and the selection of suitable grafts. In 295 fibulae the foramina nutritiva and the periosteal vessels were investigated light microscopically. Moreover, by means of five amputation preparations the course of A. poplitea and its ramifications and of all nutritive vessels was shown r

Concomitant Gastric and Duodenal Metastases From Cervical Cancer Presenting With Upper Gastrointestinal Bleeding: A Rare Case.

Upper gastrointestinal bleed is a frequent medical emergency, which may arise from multiple pathologies. Metastatic lesions to the stomach and duodenum, although rare, may be an underlying cause. Specifically, metastasis from a primary cervical malignancy is extremely infrequent, with only a few cases reported in the literature. In addition, concomitant metastatic lesions in the stomach and duodenum from cervical cancer have not been reported yet. We present the case of a 46-year-old female patient known to have cervical squamous cell carcinoma, metastatic to the lungs, bone, and liver, presen

Plaque morphea with neurological involvement—an extraordinary uncommon presentation.

Localized scleroderma is traditionally considered to be limited to the skin, subcutaneous tissue, underlying bone, and in the craniofacial subtype, also nervous system involvement. However, recent studies have also described other systemic manifestations in these patients. Despite many reports of neurological involvement in patients with the craniofacial linear localized scleroderma, it is extremely rare in patients with the other subtypes of localized scleroderma. Here, we report an extraordinary case of localized scleroderma en plaque (classic morphea), located to the upper trunk and neck, a

A Lodged Barramundi Fish-Bone Stabbing the Piriform Fossa in a Child.

Unintentional ingestion of a fish bone is common in children, especially among families with high consumption of seafood. Complications in children are extremely rare. We describe a 3-year-old healthy boy who had a large bone of Barramundi fish lodged in his upper esophagus causing significant distress. Soft tissue neck radiograph revealed a mildly thickened epiglottis and bulbous and hypertrophied adenoid soft tissue. A 21-mm foreign body was noticed. A flexible fiberoptic laryngoscopy revealed a large fish bone emerging from the left piriform fossa and arcing over the left arytenoid to hover
