Pericarditis, pneumonia and brain abscess due to a combined Actinomyces--Actinobacillus actinomycetemcomitans infection.

A patient is described in whom there developed a pulmonary infiltrate and constrictive pericarditis caused by a combined Actinomyces and Actinobacillus actinomycetemcomitans infection, presumably originating from his poor dentition. The diagnosis was only made following repeated thoracotomy. After surgery, long-term treatment with antibiotics led to complete clinical recovery. None the less, some months later he was found to have a brain abscess which resolved during a further course of antibiotics. The variable clinical picture of actinomycosis is discussed, as well as the role of other bacte…

Subperiosteal abscess of the orbit with multiple abscesses in the brain and body due to Chromobacterium violaceum sepsis in an infant: a case report and review of the literature.

Chromobacterium violaceum is a rare pathogen known to cause severe, life-threatening infections in humans. Its typical clinical presentations include skin and soft tissue infections, fulminant septicemia, multiple visceral abscesses, and diarrhea. We report the case of a 2-month-old male infant who initially presented with acute fever and subsequently developed acute proptosis of the right eye, accompanied by a palpable mass in the right mastoid area. Computed tomography (CT) scans of the orbits, paranasal sinuses and brain revealed a subperiosteal abscess at the right sphenoid wing, right oto

Multiple cerebral abscesses because of Listeria monocytogenes: three case reports and a literature review of supratentorial listerial brain abscess(es).

BACKGROUND: Central nervous system involvement often follows bacteremia because of Listeria monocytogenes. Meningitis is clinically the most common manifestation, while brain abscess occurs in about 1% of patients. Brain abscess is usually solitary but in recent years, probably in part because of the availability of computerized tomography and magnetic resonance imaging, several reports have described two or more separate supratentorial abscesses. METHODS: We have described three patients with listerial brain abscesses and reviewed the North American and European literature of brain abscess(e…

Pseudallescheria boydii brain abscess: association with near-drowning and efficacy of high-dose, prolonged miconazole therapy in patients with multiple abscesses.

Brain abscess caused by Pseudallescheria boydii is a highly lethal infection, usually seen in immunosuppressed patients. Five patients with P. boydii brain abscesses are described. Four of these patients acquired their infection after near-drowning; 1 patient developed an abscess after penetrating head trauma. Two patients survived their infections, which included involvement of other body sites (lung, eye, bone) as well as multiple undrained brain abscesses, after prolonged courses of high-dose parenteral miconazole (80-90 mg/kg/d). Progressive increases in miconazole dosage during the treatm…

Solitary Nocardia farcinica brain abscess in an immunocompetent adult mimicking metastatic brain tumor: rapid diagnosis by pyrosequencing and successful treatment.

BACKGROUND: Nocardia brain abscess carries a higher morbidity and mortality rate than other bacterial cerebral abscesses, with reported mortality rates of 55% and 20% in immunocompromised and immunocompetent patients, respectively. To prevent a delay in diagnosis and treatment, an aggressive therapeutic approach is required. In the present study, a rapid and accurate molecular diagnostic approach using pyrosequencing (PS), a semiautomated molecular genotyping method of nucleotide sequencing-by-synthesis, was performed. CASE DESCRIPTION: A 53-year-old man developed word-finding difficulties, f…

A Report on the Diagnosis and Treatment of Multiple Fungal Critical Brain Abscesses A Case Report.

Multiple Candida glabrata (C. glabrata) brain abscesses are an exceptionally rare central-nervous-system infection. Clinical data on diagnosing and managing multiple Candida glabrata brain abscesses are still scarce. Here we reported a 66-year-old patient with multiple C. glabrata brain abscesses confirmed by cerebrospinal-fluid puncture. The patient showed marked improvement following intracranial pressure-lowering therapy and antifungal therapy. By detailing the relevant MRI findings and therapeutic approach, this article aimed to provide a reference for the future clinical diagnosis and man…

Imaging-based predictors of progression in brain abscess: the role of loculation and ADC gradient.

PURPOSE: Brain abscess remains a life-threatening condition despite advances in neurosurgical and antimicrobial therapy, and reliable imaging-based markers to guide postoperative risk stratification and surveillance remain limited. This study aimed to identify clinical and radiographic factors associated with abscess progression in neurosurgically treated patients, with particular emphasis on the apparent diffusion coefficient (ADC) gradient. METHODS: We retrospectively reviewed 53 patients with brain abscess who underwent neurosurgical treatment at Gachon University Gil Medical Center betwee…

Brain Abscesses Complicating Bacterial Meningitis-A Nationwide Cohort Study From the Netherlands.

BACKGROUND: Brain abscesses rarely complicate bacterial meningitis while their clinical characteristics and outcomes have not been systematically assessed. We studied prevalence, pathogen distribution, clinical characteristics including treatment, and outcomes of brain abscess in bacterial meningitis patients. METHODS: In a prospective Dutch national cohort of community-acquired bacterial meningitis (January 2006 to October 2023), we identified patients with a brain abscess diagnosed by the treating physician. Presence of abscesses was confirmed by re-evaluation of the cranial imaging. RESUL…

The Conservative Treatment of a Rare Postoperative Complication of DBS-Brain Abscess: Case Series.

BACKGROUND: Deep Brain Stimulation (DBS) is an established treatment option for movement disorders such as Parkinson's disease and dystonia, so more and more patients (over 160,000 patients till 2019) worldwide have undergone DBS for a variety of neurological and non-neurological conditions, with numbers increasing each year. This case series primarily discusses a very rare complication following DBS surgery -- brain abscess. CASE PRESENTATION: We administered vancomycin and metronidazole to patients who developed brain abscesses after undergoing DBS surgery based on previous literature. Afte…

Isolated brain abscess caused by Aspergillus tubingensis in a patient with alcoholic liver cirrhosis.

A 67-year-old Japanese man with alcoholic liver cirrhosis without any other known immunosuppressive conditions presented with a solitary brain abscess caused by Aspergillus tubingensis. A. tubingensis, a cryptic species within the Aspergillus section Nigri, has been reported to exhibit resistance to voriconazole. This case highlights the necessity for considering central nervous system aspergillosis as a differential diagnosis, even in patients without significant immunosuppression. Moreover, molecular identification and antifungal susceptibility testing are crucial for accurate diagnosis and …

Experience with surgical evacuation of neonatal brain abscess: A patient series and review of literature.

BACKGROUND: Neonatal brain abscess is a focal intracerebral infection that, although rare in neonates, is associated with high mortality rates and long-term complications. In neonates, it mainly occurs as a complication of neonatal sepsis and meningitis. This case series reports three cases of neonatal brain abscesses involving infants who presented with fever and other symptoms and were treated with antibiotics, surgical evacuation, and prophylactic anti-epileptic treatment. Following treatment, all infants showed marked radiological improvement and reduced abscess size. In addition, pus cult…

An Exceptional Case of a Supra-tentorial Streptococcus Salivarius Brain Abscess-A Case Report.

INTRODUCTION: During the COVID-19 pandemic, incidence of brain abscesses is difficult to assess. Numerous studies reported benign and severe post SARS-CoV-2 vaccine side effects, including rare cases of brain abscesses associated with COVID-19 or Anti-SARS-CoV-2 vaccines. Here in, we report what we believe to be, up to date, the fourth known case in the medical literature of a streptococcus salivarius brain abscess, the first intra parenchymatous or supra-tentorial streptococcus salivarius brain abscess and also the first that occurs following an anti-SARS-CoV-2 vaccine. CASE PRESENTATION: We…

Esophageal Perforation Presenting Initially as Multiple Brain Abscesses Secondary to Streptococcus intermedius.

Brain abscess is a life-threatening infection that can occur secondary to contiguous or hematogenous spread. Several underlying conditions can lead to brain abscesses, such as dental infection, otitis media, sinusitis, and immunosuppression. Esophageal perforation leading to brain abscesses is extremely rare. We report a rare case of a 32-year-old man who presented to the emergency department with progressive headaches and upper-extremity weakness. Upon further evaluation, computed tomography (CT) revealed multiple brain abscesses secondary to Streptococcus intermedius infection. The patient e…
