[Use of Airwayscope with pediatric intlock in a patient with first and second branchial arch syndrome].

First and second branchial arch syndrome is a congenital anomaly of craniofacial dysplasia involving organs derived from the second branchial arch. The main characteristics are microtia and mandibular hypoplasia. A 6-year-old boy was scheduled for adenoidectomy and bilateral myringotomy and tube placement. Slow induction was performed with oxygen, nitrous oxide, and sevoflurane. No difficulties were encountered during mask ventilation, and rocuronium was administered intravenously. His epiglottis was not visible during laryngoscopy. Therefore, we used the Airwayscope (AWS). His glottis was vis

Management of congenital fourth branchial arch anomalies: a review and analysis of published cases.

BACKGROUND/PURPOSE: Congenital fourth branchial arch anomalies are uncommon entities, heretofore described only in case reports, affecting primarily children, and typically presenting as a cervical inflammatory process. The aim of the study was to collect appropriate data on the diagnosis, treatment, and outcome of this condition and to suggest guidelines for its management. METHODS: We conducted a structured review of the literature for cases explicitly identified as congenital fourth branchial arch anomalies or meeting anatomical criteria for this condition. We computed descriptive statisti

The craniofacial surgeon as amateur geneticist.

Craniofacial surgeons, by nature and training, focus on how to correct anomalies rather than on why they occur. Surgeons often leave diagnosis and etiopathogenic speculation to geneticists. Craniofacial surgeons should cross over the specialty line and learn to think like geneticists. This article reviews definitions of basic words in the genetic language and emphasizes the three diagnostic levels, phenotypic, pathogenic, and genetic, for the principal categories of craniofacial anomalies. Whenever possible, examples are given to illustrate how genetic knowledge can influence surgical strategy

Familial hemifacial microsomia due to autosomal dominant inheritance. Case reports.

Hemifacial microsomia is a rare dentofacial anomaly which is regarded as a separate entity to Goldenhar syndrome and primarily affects the structures of the first branchial arch. It has a heterogeneous aetiology and tends to occur sporadically, though positive family histories have been reported. This paper reports on individuals in two generations of a family that has overlapping features of hemifacial microsomia and Goldenhar syndrome segregating as an autosomal dominant condition.

[Results of using autologous and demineralized allogeneic bone transplants in reconstructive operations on the mandible in children with the syndrome of the first and second branchial arches].

A comparative clinico-roentgenological assessment of results of transplantation of auto- and allogenic demineralized transplants in reconstructive operations on the mandible was made in 58 children with the syndrome of the first and second branchial arches aged from 1 to 14 years. It was found that the substitution of the transplants by a new bone tissue was developing similarly. Allogenic bone transplants have a number of advantages over autotransplants: a simpler treatment and modelling the absence of an additional trauma to the child and a shorter duration of the operation.

Morphology and morphometry of blood cells of two species of freshwater stingrays (Chondrichthyes: Potamotrygoninae) from the lower Solimões river, Amazon.

Cytomorphological evaluation of peripheral blood provides information about the health status of animals, in addition to being indicators of environmental contamination. However, reference data regarding the size and shape of elasmobranch blood cells is limited. This work aimed to determine the morphological and morphometric of Potamotrygon motoro and Potamotrygon orbignyi blood cells. The stingrays were captured in white water environments in the lower Solimões River region, state of Amazonas, Brazil. Blood was obtained through puncture of the branchial artery using syringes with 10% EDTA. Mo

Surgical management of nonmalignant parotid masses in the pediatric population: the Montreal Children's Hospital's experience.

Nonmalignant parotid masses in children can have protean etiologies ranging from infective parotitis to a benign neoplastic, vascular, or congenital origin. We review the 10-year experience of a tertiary care pediatric centre with respect to the surgical management of nonmalignant parotid masses. In total, 15 patients with nonmalignant masses of the parotid gland region underwent surgery. Five children were diagnosed with lymphoepithelial cyst or first branchial cleft cyst. Three children were diagnosed with parotid abscess, one of whom had atypical mycobacteria. Other diagnoses included lymph

Ypel1: a novel nuclear protein that induces an epithelial-like morphology in fibroblasts.

BACKGROUND: Embryonic development depends on the regulation of cell morphology and behaviour to carry out morphogenesis. One example of this is the development of the face, which is constructed from independent blocks of tissue that must grow in a coordinated way and then fuse to form a continuous tissue. RESULTS: We have isolated a novel gene of unknown function from mouse and quail embryos in a search for genes involved in craniofacial development. Sequence analysis of this gene, known as Ypel1, demonstrates a striking level of amino acid conservation between vertebrates and invertebrates b

Unilateral accessory tragi in a cat.

An accessory tragus-a congenital malformation of part of the external ear-is an abnormal appendage developed from the first pharyngeal (or branchial) arch. In humans, an accessory tragus can be associated with other abnormalities as part of congenital malformative syndromes, most of which are transmitted in an autosomal dominant mode. Three cutaneous exophytic lesions were detected in the right preauricular region of a 2.5-y-old, castrated male European shorthair cat. Two of these lesions were submitted for histologic examination and were diagnosed as accessory tragi. They consisted of a verti

Idiopathic Thyroid Abscess - A Rare Occurrence.

Thyroid abscesses are rare as the gland has a rich blood supply, well-developed capsule and is high in iodine. However, clinicians must be aware of this fact and make an early diagnosis as it can lead to serious morbidity if left untreated. Infection may involve one or both lobes of the thyroid gland, although the left lobe is more often affected and is successfully treated with surgery and antibiotics. This case study involves a 22-year-old woman with a history of painful swelling over the neck accompanied by low-grade fever without any underlying cause. Both the ultrasound and computed tomog

Papillary Thyroid Carcinoma Cervical Lymph Node Metastasis with Cystic Change Differentiated from Congenital Cystic Lesions with the Assistance of Immunohistochemistry: A Case Study.

Diagnosis of cystic papillary thyroid carcinoma (PTC) lymph node metastasis at head neck region can be a challenge in the absence of known PTC history. The congenital cystic lesions of head neck, especially thyroglossal duct cyst (TGDC) and branchial cleft cyst (BCC), are major differential diagnoses in this clinicopathological scenario. The location of cyst and morphology of lining epithelium are critical clues for reaching correct diagnosis. However it is not uncommon that the flattened bland epithelial lining can be seen in both cystic metastases and congenital cystic lesions. Given that Pa

Genotype phenotype correlation of 30 patients with Smith-Magenis syndrome (SMS) using comparative genome hybridisation array: cleft palate in SMS is associated with larger deletions.

BACKGROUND: Smith-Magenis syndrome (SMS) is rare (prevalence 1 in 25 000) and is associated with psychomotor delay, a particular behavioural pattern and congenital anomalies. SMS is often due to a chromosomal deletion of <4 Mb at the 17p11.2 locus, leading to haploinsufficiency of numerous genes. Mutations of one of these gemes, RAI1, seems to be responsible for the main features found with heterozygous 17p11.2 deletions. METHODS: We studied DNA from 30 patients with SMS using a 300 bp amplimers comparative genome hybridisation array encompassing 75 loci from a 22 Mb section from the short ar

Gene expression analysis of Gata3-/- mice by using cDNA microarray technology.

Transcription factor Gata3 is implicated in the formation of autosomal dominant hypoparathyroidism, sensorineural deafness, and renal anomaly (HDR) syndrome. We pursued to identify the potential Gata3 target genes by profiling the gene expression pattern in E9.5 Gata3-/- mouse embryos. Altogether four independent microarray hybridizations were carried out on NIA Mouse15K cDNA arrays. We discovered two hundred and sixty one genes that are downregulated in Gata3 mutant embryos at E9.5 (with a minimal 2.0-fold change). The majority of the differentially expressed genes belong to two functional gr

Twist is required for patterning the cranial nerves and maintaining the viability of mesodermal cells.

Twist encodes a basic helix-loop-helix transcription factor that is required for normal craniofacial morphogenesis in the mouse. Loss of Twist activity in the cranial mesenchyme leads to aberrant migratory behaviour of the neural crest cells, whereas Twist-deficient neural crest cells are located in an inappropriate location in the first branchial arch and display defective osteogenic and odontogenic differentiation (Soo et al. [2002] Dev. Biol. 247:251-270). Results of the present study further show that loss of Twist impacts on the patterning of the cranial ganglia and nerves but not that of

Selective vulnerability of embryonic cell populations to ethanol-induced apoptosis: implications for alcohol-related birth defects and neurodevelopmental disorder.

BACKGROUND: Ethanol-induced cell death has been characterized in very few stages of embryogenesis. This investigation comprehensively maps patterns of both programmed and ethanol-induced cell death in the central nervous system and craniofacial region at 0.5-day intervals from gestational day (GD) 6.5 to 11 in mice. METHODS: A teratogenic dosage of ethanol (2.9 g/kg) or vehicle was administered via two intraperitoneal injections to pregnant C57BL/6J mice at various stages of gestation. Cell death patterns were characterized using Nile blue sulfate vital staining and histological analysis of p

Deletion of a conserved regulatory element required for Hmx1 expression in craniofacial mesenchyme in the dumbo rat: a newly identified cause of congenital ear malformation.

Hmx1 is a homeodomain transcription factor expressed in the developing eye, peripheral ganglia, and branchial arches of avian and mammalian embryos. Recent studies have identified a loss-of-function allele at the HMX1 locus as the causative mutation in the oculo-auricular syndrome (OAS) in humans, characterized by ear and eye malformations. The mouse dumbo (dmbo) mutation, with similar effects on ear and eye development, also results from a loss-of-function mutation in the Hmx1 gene. A recessive dmbo mutation causing ear malformation in rats has been mapped to the chromosomal region containing

Descriptions of new and rare crabs associated with coral rubble and sponges from Kanyakumari, southeastern Arabian Sea, southern India (Decapoda: Brachyura: Epialtidae and Pilumnidae).

Three new brachyuran crab species are described from coral rubble and sponges collected in the southern Indian state of Tamil Nadu. The epialtid, Acanthophrys keeae n. sp., was obtained from coral rubble and is the first species from the genus reported from the Indian Ocean. It is most similar to A. costatus Griffin & Tranter 1986, from eastern Australia, but can be distinguished by the proportionately longer postorbital spine, more strongly granulated gastric region, relatively less inflated and narrower branchial region, sinuous outer margin of the basal antennal article which has the median…

Adult Cystic Lymphangioma of the Parotid Gland: An Unwonted Presentation.

Cystic lymphangioma of the parotid gland is an uncommon congenital lymphatic malformation. Its occurrence in patients of advanced age is infrequent. Patients usually present with painless soft swelling, often having experienced a long duration of symptoms. Lymphangioma among the salivary glands frequently involves the parotid gland. When evaluating cystic lesions of the parotid gland, cystic lymphangioma should be included in the differential diagnosis in addition to Warthin's tumor, branchial cyst, cystic pleomorphic adenoma, and cystic mucoepidermoid tumor.
