[Oro-pharyngeal burn during electrodissection of the adenoid and tonsil].

We present a case of oro-pharyngeal burn which occurred during electrodissection of the adenoid and tonsil in a 5-year-old boy. We intubated the patient with an uncuffed spiral tube of appropriate size and noticed a slight gas leak during positive-pressure ventilation. Anesthesia was maintained with a mixture of 60% nitrous oxide, 40% oxygen and 2.5% sevoflurane. During manipulations of the right tonsil, orange-colored flame blew out about 5 cm from the mouth. Fortunately, the patient underwent the operative procedures without any further troubles, recovered fully from the grade 1 burn in the

Insulin-like growth factor-I blood levels in severely burned patients: effects of time post injury, age of patient and severity of burn.

OBJECTIVES: Insulin-like growth factor-I (IGF-I) is a polypeptide growth factor that stimulates protein synthesis. The aims of this study were to determine (1) the effect of a severe burn on blood IGF-I levels and (2) the variables controlling IGF-I level variations during recovery of these hypermetabolic patients. PATIENTS: Eleven patients, nine men and two women (age range 22-55 years) were studied for 25 days following a severe burn (18-75% of total body surface area, mean 36%). Nitrogen balances were recorded daily and total IGF-I levels were measured every 3 days. MEASUREMENTS: IGF-I wa

Burn excision within 48 hours portends better outcomes than standard management: A nationwide analysis.

BACKGROUND: Previous studies have debated the optimal time to perform excision and grafting of second- and third-degree burns. The current consensus is that excision should be performed before the sixth hospital day. We hypothesize that patients who undergo excision within 48 hours have better outcomes. METHODS: The American College of Surgeons Trauma Quality Programs data set was used to identify all patients with at least 10% total body surface area second- and third-degree burns from years 2017 to 2019. Patients with other serious injuries (any Abbreviated Injury Scale, >3), severe inhalat

Acute eosinophilic pneumonia associated with non-cigarette smoking products: a systematic review.

Acute eosinophilic pneumonia (AEP) is characterized by an acute onset respiratory illness with bilateral chest infiltrates and evidence of pulmonary eosinophilia. Cigarette smoking is the main risk factor, but drugs and other inhalational exposures have also been reported. Herein, the association between AEP and smoking devices other than cigarettes is reviewed The PubMed database was searched using terms such as "smoking", "vaping", "e-cigarette", "waterpipe", and "marijuana", along with other commonly used synonyms for these terms. In addition, eosinophilic lung diseases were also searched f

A Rare Case of Allergic Stomatitis Due to the Consumption of Boiled Cassava.

INTRODUCTION: Allergic stomatitis is an oral inflammatory disorder characterized by a painful, burning sensation, or itchiness in the oral cavity which clinically appears as erythematous plaques, vesiculation, ulceration, and/or hyperkeratosis. The causative agents include drugs, dental materials, and food ingredients. Allergic stomatitis might develop after a person eats, touches, or inhales food allergen which then triggers the hypersensitivity reaction. The most common types of food hypersensitivity generally involve type 1 and type 4 reactions or a combination of both. PURPOSE: This case

Acute Gene Expression Profile of Lung Tissue Following Sulfur Mustard Inhalation Exposure in Large Anesthetized Swine.

Sulfur mustard (HD) is a vesicating and alkylating agent widely used on the battlefield during World War I and more recently in the Iran-Iraq War. It targets the eyes, skin, and lungs, producing skin burns, conjunctivitis, and compromised respiratory function; early acute effects lead to long-term consequences. However, it is the effects on the lungs that drive morbidity and eventual mortality. The temporal postexposure response to HD within lung tissue raises the question of whether toxicity is driven by the alkylating properties of HD on critical homeostatic pathways. We have established an

Clinical and laboratory features of acute sulfur dioxide inhalation poisoning: two-year follow-up.

We present clinical and laboratory results (including nuclear imaging) obtained over a period of two years in two nonsmoking miners who were exposed to high concentrations of sulfur dioxide (SO2) after a mine explosion. Within 3 wk of the accident, both miners had evidence of severe airways obstruction, hypoxemia, markedly reduced exercise tolerance, ventilation-perfusion mismatch, and evidence of active inflammation as documented by positive gallium lung scan. Serial ventilation-perfusion scans over the first 12 months showed progressive improvement without returning to normal. After the init

Outcomes Following the Use of Nebulized Heparin for Inhalation Injury (HIHI Study).

Inhalation injury (IHI) causes significant morbidity and mortality in burn victims due to both local and systemic effects. Nebulized heparin promotes improvement in lung function and decreased mortality in IHI by reducing the inflammatory response and fibrin cast formation. The study objective was to determine if nebulized heparin 10,000 units improves lung function and decreases mechanical ventilation duration, mortality, and hospitalization length in IHI with minimal systemic adverse events. This retrospective, case-control study evaluated efficacy and safety of nebulized heparin administere

Crack Pipes Distribution in People Who Use Drugs Attending a Harm Reduction Service in Bologna, Northern Italy. Results of a Cohort Study.

BACKGROUND: In Italy, little is known regarding the potential for harm reduction programs targeting crack cocaine smoking to reduce health problems from crack smoking. This study targeted people who use crack cocaine attending a harm reduction service in Northern Italy. Our aim was to evaluate the effects of the delivery of glass crack pipes in relation to the modification of consumption styles and of problems perceived as a consequence of crack use after 30 and 60 days from enrollment. METHODS: Cohort study. Outreach workers recruited 40 people who use crack to participate in a study of the

An Unusual Case of Cecal Perforation: Accidental Ingestion of a Tooth in an Elderly Trauma Patient.

Foreign body ingestion is a common occurrence in the United States, with the majority passing asymptomatically. In cases where complications occur, such as intestinal perforation, it may present as an acute abdomen with diagnostic challenges regarding the etiology. A 70-year-old male was brought to the emergency department (ED) after he jumped from the second floor of a burning building, sustaining 10% second-degree burns to his forearms. He was intubated for concerns of inhalational injury and resuscitated. His intensive care unit (ICU) course included the management of respiratory failure, s

Bone marrow mesenchymal stem cells alleviate smoke inhalation injury by regulating alveolar macrophage polarization via the CD200-CD200R pathway.

Источник пометил публикацию как отозванную

INTRODUCTION: Smoke inhalation injury (SII) is the most common cause of death in burn patients who are victims of fire. The inflammatory response to smoke inhalation is an important factor leading to acute lung injury (ALI) or acute respiratory distress syndrome (ARDS), so finding effective anti-inflammatory targets is the key to treating SII. Our previous study demonstrated that bone marrow mesenchymal stem cells (BMSCs) can regulate the M2-type polarization of alveolar macrophages, inhibit the inflammatory response, and have a good therapeutic effect on SII. However, the potential mechanism

Occupational contact dermatitis in the pharmaceutical industry.

Occupation-induced skin reactions are not infrequently observed in the pharmaceutical industry. Workers may come in contact with irritant substances and also with chemically reactive intermediates or drugs that may be potential sensitizers. The skin lesions can be located at the site of contact, usually the hands, although airborne reactions on exposed and even nonexposed areas (eg, by particles trapped under clothing) are not uncommon. Generalized reactions may occur due to inhalation or transcutaneous absorption. An accidental exposure to a highly allergenic compound may cause a chemical bur

Serum copper concentration as an index of clinical lung injury.

The purpose of this ongoing study is to determine whether thoracic radiotherapy for lung cancer produces an early increase in serum copper (Cu) concentration, an increase which might predict clinical outcome. Copper and iron concentrations were measured in serum obtained from nonsmall cell lung cancer patients at 0, 1, 2, 4, and 6 weeks after the start of radiotherapy. Control groups included patients irradiated for breast cancer (low dose of radiation to the lung), for endometrial, cervical or prostatic cancer (no dose to lung), and patients with congestive heart failure, pulmonary hypertensi

Hydrocarbon toxicity: A review.

CONTEXT: Clinical effects of hydrocarbon exposure have been reported since 1897. These substances are ubiquitous, and their exposures are common. The specific hydrocarbon and route of exposure will determine the clinical effect, and an understanding of this is helpful in the care of the hydrocarbon-exposed patient. OBJECTIVE: To complete a comprehensive review of the literature on hydrocarbon toxicity and summarize the findings. METHODS: Relevant literature was identified through searches of Medline (PubMed/OVID) and Cochrane Library databases (inclusive of years 1975-2013), as well as from

Toxic smoke inhalation in fire victim dogs.

Fifteen dogs were found dead in a house that was on fire. Several of these dogs were partially burned. Four dogs were submitted for postmortem examination, 2 of which were determined to have died prior to the fire. Of the 2 submitted fire fatalities, only 1 dog had burns on its body (dorsum and right side of body). Internally, both dogs had soot deposits mixed with mucus in the larynx, trachea, and primary bronchi. Microscopically, soot was identified within both airways and alveolar spaces. There were no macroscopic or microscopic indications of vital heat exposure. High levels of carboxyhemo
