Clinical and ultrasound findings of 'swimmer's shoulder' and its association with training history in elite Portuguese swimmers: a cross-sectional study.

OBJECTIVES: To assess the prevalence of shoulder injuries and ultrasound findings in elite Portuguese swimmers and their association with sy … abnormalities was noted, specifically supraspinatus tendinosis (91%), tears (29%), and subacromial bursitis (29%). Specific training for injury prevention was associated with higher rates of infraspinatus tendinosis (p = 0.047), and supraspinatus tears were linked to greater swimming distances per week (p < 0.001) and practice (p < 0.001), more years of p …

Editorial Commentary: The Indication and Technique of Iliotibial Band Release for External Snapping Hip During Hip Arthroscopy in Patients With Femoroacetabular Impingement Syndrome.

Patients with femoroacetabular impingement syndrome (FAIS) often have extra-articular disorders, such as external snapping hip (ESH). We recommend that obvious ESH be addressed by endoscopic transversal iliotibial band (ITB) release during hip arthroscopy for FAIS because the residual serious snapping caused by ESH negatively affects the outcome of hip arthroscopy. However, for mild ESH without indications for severe trochanteric bursitis on magnetic resonance imaging, we still propose that physical therapy, extracorporeal shock wave therapy, or local injection be performed for pain relief. Su…

Elbow tendinopathy and occupational biomechanical overload: A systematic review with best-evidence synthesis.

OBJECTIVES: To evaluate the evidence of an association between occupational and non-occupational exposure to biomechanical risk factors and lateral elbow tendinopathy, medial elbow tendinopathy, and olecranon bursitis. METHODS: We carried out a systematic review of the literature. We searched MEDLINE (up to November 2019) and checked the reference lists of relevant articles/reviews. We aimed to include studies where (a) the diagnosis was based on physical examination (symptoms plus clinical signs) and imaging data (if any); and (b) the exposure was evaluated with video analysis and/or direct …

Distribution of sonographically detected tendon abnormalities in patients with a clinical diagnosis of chronic achilles tendinosis.

PURPOSE: We conducted a retrospective study of the distribution of sonographically detected abnormalities in the heels of patients who had a clinical diagnosis of Achilles tendinosis. METHODS: One hundred eighteen symptomatic heels in 73 patients who had a clinical diagnosis of chronic Achilles tendinosis were examined over a 12-month period by the same experienced sonologist. The distribution of altered tendon architecture and features suggesting retrocalcaneal bursitis or Achilles paratendinosis were evaluated. RESULTS: Sonograms of 118 symptomatic heels demonstrated that 96 (81%) had abno…

Sliding-screw plate fixation of proximal femoral fractures: radiographic assessment.

The sliding compression screw-sideplate combination is currently the most widely employed device for internal fixation of stable and unstable intertrochanteric fractures of the femur. The normal and abnormal radiographic appearances of this device in the immediate post-operative period are discussed. Potential long-term complications including mal- or non-union, intra-articular penetration, metal failure, rotation of the proximal fracture fragment, disengagement, trochanteric bursitis, leg length discrepancy, delayed cervical stress fracture, and ischemic necrosis are reviewed.

Imaging of acute musculoskeletal infections in children and their differential diagnoses.

Musculoskeletal complaints are a frequent cause of emergency department visits in pediatric patients. While many of these visits are due to acute trauma, infection is also common and can cause substantial morbidity. This article provides a review of acute pediatric musculoskeletal infections with an additional emphasis on important differential diagnoses to consider. The scope includes cellulitis, abscess, necrotizing soft tissue infections, pyomyositis, infectious tenosynovitis, septic bursitis, septic arthritis, and osteomyelitis. Important differential considerations for these musculoskelet…

Enlarged Iliopsoas Bursa Compressing the Femoral Nerve Resulting in Femoral Mononeuropathy and Denervation Edema.

Iliopsoas bursitis, although not rare, may manifest clinically as a detectable mass in the groin area. This condition has the potential to apply pressure on the adjacent femoral neurovascular bundle, leading to isolated mononeuropathy. We present an uncommon case of femoral mononeuropathy in a 46-year-old female patient caused by an enlarged iliopsoas bursa. The case is noteworthy due to its rarity and the clinical and diagnostic challenges associated with assessing individuals with a groin mass, hip flexor weakness, and femoral nerve palsy.

Clinical manifestations of Whipple's disease mimicking rheumatic disorders.

Whipple's disease is a rare, chronic, systemic disorder caused by Tropheryma whipplei infection. The most common symptoms are weight loss, arthralgia, diarrhea and abdominal pain. Other organ involvement can also occur in the patients. Joint manifestations may mimic rheumatoid arthritis or spondyloarthritis. Arthalgia, arthritis, spondylodiscitis, bursitis and/or tenosynovitis are seen in the majority of the patients. This explains why some of the symptoms are misdiagnosed as those of rheumatic diseases. Understanding of Whipple's disease is important for differential diagnostics of several rh…

Disseminated protothecosis.

Here is presented, a rare case of disseminated protothecosis in a 10-year-old boy with combined immunodeficiency, hitherto unreported from India. Even though it is difficult to diagnose clinically, observation of the sporangiospores within the sporangium in culture gives the accurate laboratory identification of Prototheca spp. In this patient, failure to eradicate the infection with amphotericin B and recurrence with olecranon bursitis along with skin lesions and splenomegaly was observed. Disseminated protothecosis in a child with combined immunodeficiency and failure to eradicate the infect…

Hip arthroscopy: complications in 1054 cases.

Although arthroscopic surgery of the hip is becoming a more frequently done operation, there is relatively few data in the literature regarding the frequency of complications. In the current prospective study of 1054 consecutive hip arthroscopies, the authors report an overall complication rate of 1.4%. Complications included neurapraxia, portal wound bleeding, portal hematoma, trochanteric bursitis, and instrument breakage. The only major complication was one case of septic arthritis. It is concluded that, done properly, hip arthroscopy can be a considered a low-risk procedure.

Rationale for treatment of hip abductor pain syndrome.

Patients with lower back or buttock pain that radiates into the posterior or lateral leg are often referred to physical therapy with a diagnosis of sciatica. Often the physical exam does not reveal neurologic findings indicative of radiculopathy. Instead, there is hip abductor muscle pain and weakness. This syndrome involves muscle imbalances that result in overuse strain of the gluteus medius and gluteus minimus muscles, myofascial trigger points, and trochanteric bursitis. This paper describes hip abductor pain syndrome and provides a rationale for the diagnosis and treatment.
