Long-Term Outcomes After Severe Acute Brain Injury Requiring Mechanical Ventilation: Recovery Trajectories Among Patients and Mental Health Symptoms of Their Surrogate Decision Makers.

BACKGROUND: Long-term outcomes of patients with severe acute brain injury (SABI) and their surrogate decision makers (SDMs) are insufficiently explored. METHODS: We conducted a prospective, single-center, observational study of patients with SABI who required mechanical ventilation between September and November 2021. Two telephonic interviews were conducted at 6-12 months and 18-24 months post SABI. Patients' functional outcomes at both time points were measured on the Glasgow Outcome Scale-Extended and categorized as dead (1), dependent (2-4), or independent (5-8). SDMs were interviewed at

Nutritional ultrasound cut off for predicting complications and mortality in lung cancer patients: a multicenter longitudinal study from the VALONC project.

INTRODUCTION: Lung cancer remains a leading cause of cancer-related mortality, often exacerbated by malnutrition and muscle depletion that traditional weight-centric metrics may fail to identify. This study assessed the clinical and prognostic relevance of rectus femoris (RF) ultrasound as a practical bedside tool for evaluating muscle-related parameters and clinical outcomes. METHODS: This multicenter longitudinal study involved 374 patients (114 females, 260 males) across 10 Spanish hospitals. Researchers performed ultrasonographic assessments of the RF, measuring cross-sectional area (RF-C

Multi-Organ Metatranscriptomics Establishes Organ-Specific Functional Baselines in the Crested Ibis (Nipponia nippon).

BACKGROUND: The crested ibis (Nipponia nippon) is an iconic East Asian species on the International Union for Conservation of Nature's (IUCN) Red List, with recurrent stress-related sudden death reported in captivity and in the wild. However, the mechanisms underlying stress responses remain poorly characterized. METHODS: Using opportunistic post-mortem samples, we generated, to our knowledge, the first organ-resolved metatranscriptomic reference dataset for this species in a stress-associated context. Matched libraries from small intestine, liver, spleen, and lung were sequenced and processe

Constructing a tumor immune microenvironment-driven prognostic model in acute myeloid leukemia using bioinformatics and validation data.

The tumor immune microenvironment (TIME) is a critical determinant of prognosis in acute myeloid leukemia (AML). This study aimed to develop a prognostic model based on immune-related hub differentially expressed genes (hub-DEGs) to refine risk stratification and identify therapeutic targets. Transcriptomic and clinical data from 149 TCGA-AML patients were analyzed using ESTIMATE and xCell algorithms to infer immune scores. Differentially expressed genes (DEGs) between high/low immune score groups were identified, followed by functional enrichment, protein-protein interaction (PPI) network ana

Neutrophil-Related Genes Predict Prognosis and Contribute to Immunosuppression in Acute Myeloid Leukemia.

INTRODUCTION: Acute myeloid leukemia (AML) prognosis remains challenging due to limited biomarkers integrating tumor microenvironment (TME) dynamics. Neutrophils, key mediators of immune regulation, exhibit dual roles in cancer progression, yet their prognostic significance in AML is poorly defined. This study aimed to construct a neutrophil-related gene signature for AML risk stratification and explore its clinical and immunological implications. METHODS: Utilizing transcriptomic and clinical data from TCGA (The Cancer Genome Atlas), GEO (Gene Expression Omnibus), and OHSU cohorts (n=1537),

Age-dependent involvement of gut mast cells and histamine in post-stroke inflammation.

BACKGROUND: Risk of stroke-related morbidity and mortality increases significantly with age. Aging is associated with chronic, low-grade inflammation, which is thought to contribute to the poorer outcomes after stroke seen in the elderly. Histamine (HA) is a major molecular mediator of inflammation, and mast cells residing in the gut are a primary source of histamine. METHODS: Stroke was induced in male C57BL/6 J mice at 3 months (young) and 20 months (aged) of age. Role of histamine after stroke was examined using young (Yg) and aged (Ag) mice; mice underwent MCAO surgery and were euthanized

Alzheimer's disease polygenic risk's association with all-cause dementia through the plasma metabolome in the UK Biobank study.

Dementia, with Alzheimer's Disease (AD) accounting for 60-70% of its occurrences, is a multifactorial disorder marked by cognitive and functional decline. Polygenic risk scores (PRS) stratify genetic risk for complex diseases, including AD. Integrating PRS with metabolomics offers a pathway to better understand AD etiology and identify biomarkers for early detection. We explored the association between AD PRS, metabolomics, and dementia risk using data from the UK Biobank. The analysis emphasizes sex-specific associations and the mediating role of metabolites between AD PRS and dementia risk.

"It's about fulfilling my role": a qualitative study of dignity and life narrative meanings for older adults in palliative care within a Confucian-family context.

BACKGROUND: Dignity is a cornerstone of quality palliative care. While Dignity Therapy has been developed to support dignity, its cultural foundations are rooted in Western individualistic values. In Confucian-family contexts, where interdependence and familial roles are paramount, the conceptualization of dignity and the relevance of life narratives may differ significantly. Understanding these culturally embedded perspectives is crucial for developing person-centered care that is truly resonant. AIM: This study aimed to explore how older adults in a Confucian-family context perceive and arti

[Postoperative cognitive disorders in traumatic hip surgery in elderly subjects].

The aging population inevitably leads to an increase in the number of surgical procedures performed on older people. The effects of anesthesia coupled with surgery are recognized as precipitating a post-operative cognitive impairment whose etiology seems to be multifactorial with a predominance of the inflammatory cause related to surgery. Objectives. The main objective of our work is to determine the incidence and risk factors of cognitive decline after hip injury surgery. This is the continuation of a publication focusing on the epidemiological and evolutionary profile of the same cohort [1]

A Real-Life Study in Patients Newly Diagnosed with Autoimmune Hashimoto's Thyroiditis: Analysis of Asthenia as Admission Complaint.

Background: Amid the large panel of autoimmune thyroid diseases, Hashimoto's thyroiditis (HT) represents a major point across multidisciplinary daily practice. When it comes to the clinical picture, particularly in regard to asthenia (also described as "fatigue" or "decreased energy"), the differential diagnosis is challenging, and a meticulous anamnesis should be backed up by focused lab investigations. Our objective was to analyze the thyroid panel in newly diagnosed patients with HT in relationship with the presence of asthenia as an admission complaint. Methods: This was a retrospective, m
