OBJECTIVE: To develop an endoscopic transnasal approach to atlas tumors and study its practicability.
METHODS: This article comprises two components: an illustrative case report and observational data on 50 volunteers. As to the case report, a 34 year old man presented with occipital pain for more than 3 months and underwent systematic investigation in Qilu Hospital of Shandong University. CT and MRI scans showed bony destruction in the craniovertebral junction (CVJ) suggestive of tumor. Via an endoscopic transnasal approach to the suspected atlas tumor through the inferior nasal meatus, a Ga

BACKGROUND: The first cervical vertebra (C1) is a unique ring-shaped structure and is largely cartilaginous at birth. Our objectives were to retrospectively analyze the age of closure of the synchondroses and determine age-dependent morphological characteristics of the C1 vertebra using computed tomographic (CT) scans.
METHODS: Helical CT scans were made in fifty-four children as part of routine imaging of patients presenting to the emergency room with head injury, to exclude cervical spine trauma. Axial and sagittal reconstruction images of the C1 ring were analyzed for widths of the anterio

The authors present their technique of occipital-axis posterior wiring and fusion for atlantoaxial dislocation associated with an occipitalized atlas. The technique consists of drilling a 3 x 1-cm horizontal groove in the occipital bone 1 cm posterior to the foramen magnum and building up a bony bridge along the posterior margin of the foramen magnum. This bony bridge is referred to as an "artificial atlas." Conventional wiring and fusion is performed between the artificial atlas and the C-2 lamina, interposing a strut bone graft. Since the compression force on tightening the wire is vertical,

A case is reported of median cleft face syndrome with bifid tongue and odontoid peg and failure of formation of the anterior arch of the atlas. These are features which have not been reported previously. The preoperative CT demonstrated its potential to enhance our understanding of complex and rare craniofacial deformities.

A five-year-old boy sustained a vertical compression injury to the occiput, initial findings were consistent with a cervical spine injury. Evaluation, including plain films, tomograms, and CT scanning, confirmed a C1 fracture through congenital anterior and posterior arch defects. Flexion and extension tomograms showed this rare lesion to be stable, and no surgical intervention was required. Because congenital anomalies of the cervical spine can be confused with traumatic lesions, thorough evaluation is warranted. Treatment should be based on signs of instability, if present.

Atlantoaxial dislocation (AAD) or subluxation (AAS) is a potentially life-threatening disorder caused by instability between the atlas and axis. Typical symptoms include neck pain, torticollis, and neurological deficits, but dizziness has rarely been reported in association with AAD/AAS. We describe an adolescent girl who presented with non-spinning dizziness, neck pain, and torticollis after prolonged head flexion and extension. Neurological examination showed rightward torticollis without nystagmus. Cervical spine imaging revealed atlantoaxial rotatory dislocation and fixation, while CT angi

STUDY DESIGN: The lesion of the lower four cranial nerves, commonly called Collet-Sicard syndrome, can be caused by a blunt head and neck trauma. It may be associated to an isolated fracture of the occipital condyle or of the atlas.
OBJECTIVE: The aim of this report is to assess the modality of the trauma, the type of fracture, the anatomic characteristics, the treatment and clinical results of this syndrome. We discuss 14 cases of fracture of occipital condyle and of atlas and 1 personal case.
METHODS: We analyzed 14 cases collected from the literature between 1925 and 2013, reported a furt

We report a case of tuberculous vertebral osteomyelitis of the first and second cervical vertebrae with extensive adjacent soft-tissue involvement and extension into the mediastinum and bilateral flanks. The clinical presentation of tuberculous vertebral osteomyelitis depends on the vertebrae involved. The characteristic syndrome, Pott's disease, reflects the consequence of infection of the lower thoracic and lumbar spine, the most common site of tuberculous vertebral osteomyelitis. Cervical involvement is unusual: tuberculosis affects the cervical vertebrae in approximately 0.03% of all cases

BACKGROUND: Intraoperative vascular injuries in the cervical spine are rare, but carry significant morbidity and mortality when they do occur. There is a need to better characterize the risk of vertebral artery injury (VAI) after posterior C1-C2 fusion. The aim of this study was to investigate the rate of VAI in patients undergoing posterior C1-C2 cervical fusion.
METHODS: An electronic database search was performed to identify studies that reported rates of VAI following posterior cervical fusion at C1-C2 level. Patient-specific risk factors, surgical indication, surgical technique, and othe

INTRODUCTION: Patients with combined C1-2 fractures were often treated by posterior arthrodesis. However, elderly patients with multiple injuries (such as brain injury), the large surgical trauma of posterior arthrodesis will increase the risk of perioperative mortality. A minimally invasive technique may be better for them, and decrease the risk of perioperative mortality.
MATERIALS AND METHODS: Seven patients with combined C1-2 fractures underwent percutaneous anterior odontoid screw and anterior C1-2 transarticular screws (percutaneous triple anterior screws fixation). The surgical techniq

The acute calcific tendinitis of the longus colli muscle is a rare inflammatory process of the prevertebral muscles. The clinical picture includes acute neck pain, limited range of motion of the cervical spine with stiffness and odynophagia. The laboratory findings demonstrate inflammatory signs. The incidence of the disease peaks between the third and sixth decade. The knowledge of the characteristic radiologic findings and the self-limiting course prevents the patient from needless medical and surgical interventions.

OBJECT: The object of this study was to describe an unusual fracture subtype within C-1 injuries with a propensity to result in late deformity and pain. Most patients with C-1 injuries are nonsurgically treated using external immobilization unless there is an injury of the transverse atlantal ligament. The authors describe an unusual variant involving a unilateral sagittal split with a high tendency to late deformity and pain. They also review the literature and treatment of C-1 fractures.
METHODS: A retrospective review of 12,671 CT scans from a Level I trauma center over a 6-year period yie

The author describes weapon traumatic lesions in an adult male skeleton, that was excavated in the Italic necropolis of Opi Val Fondillo, Central Italy. The preservation of the skeleton is very good. The skull shows a linear lesion across the diploE of the right parietal and occipital bones; the edge of the traumatic lesion is smooth and perpendicular to the bone surface. The injury was probably inflicted with a sharp-edged weapon and the violence of the stroke caused the detachment of bone fragments and fractures that radiate from the point of impact. A sharp-edged linear traumatic lesion, pr

PURPOSE: To study the time and mode of the development of the colliculus atlantis, the rate of its occurrence, the causes for its absence, and the radiological-clinical importance in the analysis of open-mouth-view radiographs.
MATERIAL AND METHODS: Retrospective analysis of standardized radiographs of the cervical spine in more than 20,000 adults and 100 children. Study of 234 human skeletons of different ages and of 38 isolated adult atlases. Cadaveric dissection of 42 adults (age 48-87). Axial radiographs of isolated atlases and analysis of the bony structures of the colliculus atlantis.
