A young male patient was brought to the Emergency Department after falling from a moving car. It was noted that he had a left scalp hematoma with a deformed right upper limb. Computer tomography of the head, cervical spine, thoracic and lumbar spine, thorax, abdomen, and pelvis was performed. The CT scan showed multiple cervical spine fractures, a fracture of the first rib with localized pneumothorax, and multiple lung contusions. There was also a fracture of the right humerus. CT angiography showed evidence of a complete cutoff of the right axillary artery at its origin in the axilla, which w

BACKGROUND: Thoracic outlet syndrome (TOS) identifies the clinical condition determined by the mechanical compression and entrapment of the subclavian vessels and the brachial plexus cords within the space delineated by the scalene muscles, the clavicle, and the first rib. To date, there are no concluding explanations concerning the real causes of the appearance of TOS in children. This is the first study to investigate the existence, frequency, and type of thoracic outlet anomalies in the prenatal stage (human fetuses).
METHODS: Eighty cervical dissections (40 consecutive spontaneously abort

A group of 88 spontaneously occurring variations of the skeleton were studies in adult CD-1 mice which had been exposed in utero to the herbicides 2,4,5-T or Trifluralin, administered to their dams by gavage, in an attempt to detect subtle biological effects of these compounds even in the absence of gross malformations. Sixty-seven characters showed variation in this population. Of these, 19 were significantly different in frequency from untreated controls in mice having receiving a teratogenic dose of 2,4,5-T, with an average difference of 23.7%. Seventeen of them were increased in frequency.

The study of the structures that make up the brachial plexus has benefited particularly from the high resolution images provided by 3T magnetic resonance scanners. The brachial plexus can have mononeuropathies or polyneuropathies. The mononeuropathies include traumatic injuries and trapping, such as occurs in thoracic outlet syndrome due to cervical ribs, prominent transverse apophyses, or tumors. The polyneuropathies include inflammatory processes, in particular chronic inflammatory demyelinating polyneuropathy, Parsonage-Turner syndrome, granulomatous diseases, and radiation neuropathy. Vasc…

A 63-year-old man underwent anterior mediastinal tracheostomy for postradiotherapy stoma ulceration exposing the left anterolateral tracheal side, cervical vessels, and pharynx. An anterior chest wall defect (hemiclaviculectomy, manubriectomy, and resection of anterior third of first and second ribs bilaterally) was covered by a myocutaneous pectoral flap, and a new tracheostoma was constructed in the middle of the skin island. At postoperative day 7, a protrusion of the right upper lobe outside the thoracic cavity through the anterior chest wall defect was detected. Surgical repair by a right

PURPOSE: Considerable variation exists in strategies for the diagnosis and treatment of neurogenic thoracic outlet syndrome (NTOS). Additionally, different subtypes of NTOS present with different constellations of symptoms and signs. The aim of this study was to gather and evaluate different treatment plans from high-volume NTOS surgeons for standardized case scenarios.
METHODS: Six case scenarios were distributed to 19 self-designated level IV expert NTOS surgeons. These included standardized patients presenting with Gilliat-Sumner hand, upper plexus involvement with scapular winging, pector

In NTOS, when conservative management has failed, surgical resection of the 1st or a cervical rib is often combined with scalenectomy and brachial plexus neurolysis. Computational musculoskeletal models have been developed to predict mechanical joint loads on the human spine, such as the forces and moments applied to vertebral and facet joints and the forces that act on ligaments and muscles because of difficulties in the direct measurement of joint loads. However, many whole-spine models lack certain elements. For example, the detailed facet joints in the cervical region or the whole spine region may not be implemented. In this study, a detailed cervico-thoraco-lumbar multibody musculoskeletal model with all major ligaments, separated structures of facet

The objective diagnosis of thoracic outlet syndrome (TOS) remains a challenge. The purpose was to present our experience and discuss the utility of electromyography, electroneurography and somatosensory evoked potentials after ulnar nerve stimulation in patients with surgically verified neurovascular compression at the thoracic outlet. Twenty patients (median age 29.5, 16 women) with operatively verified neurovascular compression by a cervical rib or band were examined. All complained of pain and paraesthesias in the hand. In 12 vasomotor disturbances (pallor or lividity, low skin temperature,…

Fibromatosis is rare and locally invasive tumor arising from musculoaponeurotic tissue, and also has a high postoperative recurrence rate. So, we must perform wide resection of the tumor and surrounding tissue. A case of fibromatosis originating from the right anterior scalene muscle was reported. A 31-year-old female in whom cervical fibromatosis had been pointed out by her home doctor, was referred to our hospital for treatment. CT scan, MRI and angiography showed that the tumor developed from the cervical lesion to the upper thoracic and invaded the right subclavian artery. The tumor origin

Dissection and plastination of the nervous system have been used to teach anatomy; however, there is no record of a dissection and plastination of a dog's complete nervous system. The objective of this study was to dissect and preserve the whole nervous system of a dog, using the plastination technique, as a tool for teaching veterinary neuroanatomy. A 7-month-old dog was perfused through the common carotid artery with an ethanol-based soft embalming solution, and 10% formalin was injected through the cisterna magna. An incision was made at the level of the dorsal midline, and the muscle tissu

We report on the first detailed study of the atlas-axis complex in the lizard clade Dibamidae, a family of poorly known fossorial squamates distributed in tropical or subtropical climates. This skeletal bridge is characterized by several features, such as the complete absence of the first intercentrum or the appearance of the first free cervical rib on the axis (usually less developed in Dibamus relative to that in Anelytropsis). Our study shows morphological differences of the atlas-axis complex in the Mexican blind lizard Anelytropsis relative to those of Asian Dibamus, the only two known ex…

BACKGROUND: The indications for immediate intubation in trauma are not controversial, but some patients who initially appear stable later deteriorate and require intubation. We postulated that initially stable, moderately injured trauma patients who experienced delayed intubation have higher mortality than those intubated earlier.
METHODS: Medical records of trauma patients intubated within 3 h of arrival in the emergency department at our university-based trauma center were reviewed. Moderately injured patients were defined as an ISS < 20. Early intubation was defined as patients intubated f

A considerable difficulty in surgical management of apical chest tumors is represented by the potential involvement of the thoracic outlet anatomic structures (i.e.: vertebral body, subclavian vessels, first rib). Among the various techniques proposed, the anterior trans-cervical approach popularized by Dartevelle significantly improved the radical treatment of these tumors. This approach offers a wide access to the thoracic outlet with satisfactory control of subclavian vessels, safe dissection of brachial plexus and upper part of the mediastinum, permitting a radical treatment. However, this

This study proposes the description of the development of the postcranial axial skeleton, including vertebrae, gastralium, ribs, sternum, and interclavicle, in Melanosuchus niger. Six nests were marked and two eggs removed from each nest at 24-hr intervals until hatching. For posthatching evaluation, 30 hatchlings were kept in captivity and one exemplar was euthanized at three-day intervals. Samples were diaphanized using potassium hydroxide (KOH), alizarin red S, and Alcian blue. A routinely generally used method was applied for histological evaluation. It was difficult to define in which ver

Based upon experience with cervicothoracic esophageal carcinomas in which resection of the manubrium, adjacent clavicles, and ribs has facilitated exposure of the tumor, it has been found that a partial upper sternal split (without resection) provides access to the upper thoracic esophagus to the level of the carina. With a knowledge of the anatomic relationships of the esophagus in this area, this direct anterior approach has been used for both benign and selected malignant diseases involving the upper thoracic esophagus. A partial median sternotomy has been used in 11 patients with the follo
