Cholecystitis and cholelithiasis in children and adolescents.

One hundred patients aged 20 years and younger, admitted to Hurley Medical Center during a ten-year period for treatment of acute or chronic cholecystitis with or without cholelithiasis, were reviewed. Results demonstrated an increasing incidence of pediatric cases compared with previous reports, a predominance in parous teenagers, and only an 8 percent association with hemolytic diseases. The clinical presentation is very similar to that of adults. Maintaining a high index of suspicion in addition to the use of diagnostic aids such as cholecystogram, ultrasonography, and hepatobiliary imaging…

[Ultrasonography in the diagnosis of acute cholecystitis].

Data of ultrasonic scanning of 117 patients with acute cholecystitis are analyzed. The gallbladder was visualized in 95,7% of examinations. Operations were performed on 38 patients. Gallstones were revealed by echosonography in 94,7% of cases. Echosonography should be widely used in diagnosis of acute cholecystitis and in analysing the indications for surgical treatment.

[Causes of death in acute cholecystitis].

Macro- and microscopic alterations of organs in 115 patients dead from acute cholecystitis at the age from 23 to 88 were studied with special reference to clinico-laboratory findings. As a rule, there were changes in several organs, especially in the vessels and heart (99,1%), liver (93,1%), lungs (75,5%), kidneys (63,4%). A conclusion is made that the main causes of death are hepatic insufficiency (65,2%), peritonitis (13,9%), pancreanecrosis (7,8%), thromboembolism (6,1%). Other causes made 7%.

Ischaemic cholecystitis and infarction of the gallbladder.

The clinicopathological features of forty three patients with gallbladder disease are presented where ischaemia appeared to be the primary aetiological factor. Histopathological changes of severe ischaemia (ischaemic cholecystitis) were present in 16 patients and of infarction of the gallbladder in the remaining 27 patients. All patients with ischaemic cholecystitis and 19 of the patients with infarction of the gallbladder had a thick walled gallbladder due to a serosal reaction, and these 35 gallbladders all contained calculi. The remaining eight patients with infarction of the gallbladder ha…

[Etiology and therapy of cholecystitis and cholelithiasis].

Microscopic examinations of 1,000 ectomized gallbladders in patients with acute acalculous as well as calculous cholecystitides render possi … f the operated gall-bladders and the gallstones. Changes in the wall of the gall-bladder in chronic cholecystitis are reparable processes of lesions which develop during the gall attack. [Acute cholecystitis as a postoperative complication].

The literature data on acute cholecystitis occuring as a complication after operations on the abdominal organs are analysed. The author presents 3 own observations of cholecystitis in early postoperative period among 28,556 laparotomies. Taking into consideration the rarity of this complication, the author believes that the arguments adduced for simultaneous preventive cholecystectomy in operations for other abdominal organs diseases are dubious, especially in emergent abdominal surgery.

Cholescintigraphy in the diagnosis of acute cholecystitis.

Technetium-99-m-diethyl-IDA (Solco HIDA) cholescintigraphy was performed on 50 patients with suspected acute cholecysitis. The final diagnosis was acute cholecytitis in 34 cases, other biliary tract disease in 5 cases and nonbiliary disease in 11 cases. A nonfilling gallbladder was regarded as indicative of acute cholecystitis. The sensitivity in detecting acute cholecystitis was about 90%, the specificity about 80%; the predictive value of a positive test was about 90% and that of a negative test about 80%. The false positives consisted of two cases of acute pancreatitis with normal gallbladd…

Acute cholecystitis as a postoperative complication.

The clinical course and management of 40 patients who underwent operation for acute cholecystitis developing as a postoperative complication were reviewed. Of note was the mortality of 47%, the high incidence of gangrene, perforation, empyema, and cholangitis, and the atypical clinical presentation of acute cholecystitis under these conditions. Awareness of this possible complication, knowledge of its clinical features, and early surgical intervention are important facets of successful management.
