Bilateral choroid plexus hyperplasia: a case report and management strategies.

INTRODUCTION: Choroid plexus hyperplasia (CPH) is a rare cause of cerebrospinal fluid (CSF) overproduction and shunt-resistant hydrocephalus in infants. If treated with a ventriculoperitoneal (VP) shunt, these patients secondarily develop CSF accumulation along the shunt tract and within the peritoneum. The surgical management of this condition is not as clearly defined as in the case of a choroid plexus papilloma or carcinoma. CASE REPORT: An 8-day-old male patient presented with bulging fontanelle, head circumference = 42 cm (>98th percentile), and cranial ultrasound demonstrating communica…

Choroid plexus hyperplasia and monosomy 1p36: report of new findings.

Monosomy 1p36 is a newly delineated multiple congenital anomalies/mental retardation syndrome characterized by mental retardation, growth delay, epilepsy, congenital heart defects, characteristic facial appearance, and precocious puberty. It is now considered to be one of the most common subtelomeric micro-deletion syndromes. This article reports new findings of choroid plexus hyperplasia and dextrocardia with situs solitus in a patient who had deletion of chromosome 1p26.33 with a brief review of the literature.

Corticosteroids, 11beta-hydroxysteroid dehydrogenase isozymes and the rabbit choroid plexus.

The epithelial cells of the choroid plexus (CP) are responsible for cerebrospinal fluid (CSF) secretion into the ventricles of the brain. The balance between CSF production and drainage, in part, facilitates a normal intracranial pressure. The secretion of Na(+) and anions by the CP creates an osmotic gradient driving water into the ventricles. This is opposite to classical Na(+) transporting tissues, such as the kidney, where Na(+) and water reabsorption is mediated by 11beta-hydroxysteroid dehydrogenase type 2 that protects the mineralocorticoid receptor by abrogating active cortisol to inac…

[Adult choroid plexus papilloma of the posterior fossa: extraventricular location].

Choroid plexus papillomas (CPPs) are rare neuroectodermal neoplasms accounting for 0.4% of all intracranial neoplasms in adults. Most of them are located in the posterior fossa in adults. Although CPPs commonly arise from the 4th ventricle, they occasionally extend to extraventricular space. Furthermore some occur primarily in the extraventricular region. It is difficult to diagnose CPP preoperatively when the main portion of the tumor is not located in the 4th ventricle. We present a case of a 54-year-old male manifesting slurred speech, nystagmus and cerebellar ataxia. Magnetic resonance ima…

The choroid plexus-cerebrospinal fluid system: from development to aging.

The function of the cerebrospinal fluid (CSF) and the tissue that secretes it, the choroid plexus (CP), has traditionally been thought of as both providing physical protection to the brain through buoyancy and facilitating the removal of brain metabolites through the bulk drainage of CSF. More recent studies suggest, however, that the CP-CSF system plays a much more active role in the development, homeostasis, and repair of the central nervous system (CNS). The highly specialized choroidal tissue synthesizes trophic and angiogenic factors, chemorepellents, and carrier proteins, and is strategi…

Renal cell carcinoma metastatic to the choroid mimicking intraventricular meningioma.

BACKGROUND: Metastatic renal cell carcinoma to the choroid plexus is a rare condition and can be easily confused with meningioma. METHODS: A 61-year-old female presented with progressive neurologic deterioration and MRI findings of obstructive hydrocephalus and a homogeneously contrast enhancing 3 cm oval mass in the trigone of the left lateral ventricle. RESULTS: Despite radiologic, intraoperative gross features, and frozen pathology all consistent with meningioma, the final pathology revealed metastatic renal cell carcinoma. CONCLUSION: Renal cell carcinoma metastatic to the choroid plexu…

Choroid plexus adenoma: case report and review of the literature.

CASE REPORT: An 8-month-old infant with macrocephaly was found to have hydrocephalus and a cystic third ventricular tumour; biopsy of the tumour showed a choroid plexus adenoma. The tumour was attached to the ependymal lining and was strongly adherent to the walls and floor of the anterior third ventricle. TREATMENT: After biopsy, it was felt that a radical resection would carry a high risk of injury to the floor of the third ventricle and cause new neurological deficits. Therefore, an external ventricular drain was inserted and bilateral ventriculo-peritoneal shunts were inserted 1 week post…

[Gyrate atrophy of the choroid and retina: a case report].

BACKGROUND: Gyrate atrophy of the retina and choroid is a rare disease, with recessive autosomal transmission, characterized by progressive chorioretinal atrophy causing blindness. It results from a congenital deficit in aminotransferase ornithine. CASE REPORT: The authors present the case of a young patient aged 15 years old consulting for a progressive fall of visual acuity with hemeralopia. Eye funduscopy showed regions of confluent rounded chorioretinal atrophy. The visual field, the electroretinogram and the retinal angiography were all alterated. Gyrate atrophy of the retina and choroid…

Retinal pigment epithelial autotransplantation: morphological changes in retina and choroid.

BACKGROUND: Replacement of degenerating retinal pigment epithelial (RPE) cells has been proposed to promote photoreceptor survival in retina … dement of the RPE (wound type 2). Light and electron microscopy and scanning electron microscopy of choroidal vascular casts were performed at 3, 7, 14 and 30 days after surgery. Quantitative analysis of the choriocapillaris bed area was performed by automated image analysis and the results were analyzed by paired Student's t-test. RESULTS: Retinal pigme …

[Carcinomatosis of the choroid plexus: clinical case and necropsy findings].

INTRODUCTION: Carcinomas of the choroid plexus are a rare malignant variety of papillary tumours which originate in the choroid plexus. Their occurrence in adults has been questioned because of the difficulty in differentiating them from the metastases of adenocarcinomas. CLINICAL CASE AND CONCLUSIONS: We present the case of a 57 year old man with a rapidly progressive illness characterized by disorientation, behaviour changes and amnesia, progressive deterioration of the level of consciousness until he was in deep coma and died (all within one month). There was no clinical evidence of the pr…

Choroid plexus papilloma in the posterior third ventricle: case report.

OBJECTIVE AND IMPORTANCE: The case of a 42-year-old woman with a choroid plexus papilloma (CPP) arising from the posterior wall of the third ventricle is described. This case is very unusual because the tumor did not have any connection with the choroid plexus but was attached to the normal brain parenchyma. CLINICAL PRESENTATION: The patient presented with the symptoms of increasing intracranial pressure. Magnetic resonance imaging revealed hydrocephalus related to a small mass in the posterior third ventricle, occluding the aqueduct. INTERVENTION: Surgery confirmed that the tumor was locat…

Fluorescence histochemical demonstration of adrenergic terminations in the human choroid.

With the existing literature on the aspects of anatomic and physiological ocular choroid membrane variables in mind, the authors verify the possibility of applying the methods of Axelsson et al and of Lindvall and Bjorklund, as simplified by Furness and Costa for fluorescent microscopy, to the study of human choroid nerve topographic distribution. The material for the study was obtained from three human eyeballs, two enucleated because of malignant melanoma of the choroid and one because of neoplasia of the ciliary body. A binocular dissecting microscope was used. A fluorescent microscope was …

Glossopharyngeal neuralgia associated with vascular compression and choroid plexus papilloma.

We report two cases of presumed idiopathic glossopharyngeal neuralgia that were discovered intraoperatively to be associated with compression by choroid plexus papillomas, and by a variable degree of vascular compression at the root entry zones of cranial nerves IX and X. The combination of the two entities in glossopharyngeal neuralgia has not previously been reported.

Spontaneous isolated lipoma of the choroid plexus in the baboon.

A case of symptomless, solitary lipoma of the choroid plexus is described. The tumour was found in the left lateral ventricle of an adult female baboon (Papio papio) in the course of post-mortem examination. Routine histological investigation showed that the tumour was composed exclusively of characteristic adipose cells with scarce collagen septa and without other hamartoma-like constituents, such as glial cells, neurons, cartilage or muscle fibres. The tumour mass was lined by a typical single layer of cuboidal cells; no calcifications were observed either inside the tumour or in the adjacen…

Fetal lateral ventricle choroid plexus cysts: the dilemma of amniocentesis.

Although lateral ventricle choroid plexus cysts in the second-trimester fetus have been considered a benign developmental phenomenon of no clinical significance, an association with trisomy 18 has been suggested. We designed a prospective study to determine whether karyotype analysis is indicated when such cysts are encountered on prenatal sonogram. During a 20-month period, 5400 low-risk pregnant women were examined sonographically in the second trimester to evaluate the prevalence of fetal lateral ventricle choroid plexus cysts and the incidence of ultrasonographic anomalies associated with …

[Primary tumors of the choroid plexus. Frequency, localization and age].

In 30 years, from 1956 through 1985, 155 primary tumours of the choroid plexus were first reported for entry into the National Cancer Registry of the GDR, with 125 of them having been identified as plexus papillomas and 30 as plexus carcinomas. A total of 30.4 per cent of all plexus papillomas was diagnosed in patients in the first decade of life, half of them during the first year of life. A lateral ventricle was the site from which 78.9 per cent of the plexus papillomas in the first decade were recorded. Among all plexus tumours 39.4 per cent were detected only on postmortem investigation. T…
