Approximately 20% of patients with colorectal cancer have metastatic disease at time of diagnosis, and another 25-35% develop metastases during the course of their disease. Liver, peritoneum, and lungs are the most common sites of metastases. Choroid plexus papilloma is a benign central nervous system tumor that occasionally spreads along the subarachnoid space. The authors report the case of a 49-year-old man who presented with back pain 19 years after resection of a posterior fossa choroid plexus papilloma. The authors studied the mitotic activity of 21 choroidal and ciliary body melanomas treated by external beam radiotherapy (2000 rad in 5 fractions over 5-7 days) before enucleation and 21 case-by-case matched nonirradiated uveal melanomas. We reviewed 6,169 cases in which whole eyes were submitted to the Armed Forces Institute of Pathology (AFIP), Washington, DC, between January 1970 and December 1980. We sought to determine whether there had been any change in the frequency of incorrect diagnoses of malignant melanoma since two previous studies had been conducted at the AFIP. BACKGROUND: Cutaneous melanoma is a primary tumour that rarely produces intraocular metastases. We report on a 47-year-old woman who was referred to our outpatient department because of anisocoria.
METHODS: A cutaneous melanoma of the upper left arm had been excised 21 months ago with subsequent polychemotherapy for lymph node involvement. Visual acuity and intraocular tension were normal. A patient with malignant melanoma of the choroid and ciliary body had a primary cutaneous melanoma and the B-K mole syndrome phenotype. Because of this newly described association, all patients with the B-K mole syndrome (phenotype) should have a complete ocular examination to discover if there is any evidence of ocular melanoma.
