Construction and validation of an interpretable machine learning model for predicting diabetes risk in COPD patients.

To construct and validate a machine learning (ML) model for predicting diabetes risk in COPD patients, enabling early and personalized intervention. Using data from the MIMIC-IV database, 49 variables were screened by LASSO and logistic regression. Six ML algorithms were constructed and internally validated on a 70%/30% split dataset. Model performance was assessed using multiple metrics, followed by external validation. Interpretability was achieved through SHAP analysis. All six ML algorithms demonstrated strong performance across training, testing, and validation sets. The LightGBM model ac

Advances and Challenges in Transitional Urology: Caring for Adolescents and Young Adults with Lifelong Complex Genitourinary Conditions.

PURPOSE OF REVIEW: As long-term survival with complex congenital and pediatric diseases has improved, more patients with congenital genitourinary conditions are living into adulthood. These patients can continue to face lifelong issues related to their conditions, including urinary incontinence, recurrent urinary tract infections, chronic kidney disease, and difficulties with sexual health and function. RECENT FINDINGS: The practice of transitional urology continues to grow nationwide and abroad, and it will be important to develop evidence-based practices for coordinated transition of these

A comprehensive model for gynecologic cancer patients' survivorship care.

OBJECTIVES: The population of gynecologic cancer survivors is rapidly increasing, yet survivorship care remains fragmented and often focused primarily on surveillance for disease recurrence. This review aims to define the key domains shaping survivorship and propose a comprehensive, patient-centered model for gynecologic oncology. METHODS: Our narrative reviews and synthesizes current evidence and clinical perspectives to examine gynecologic cancer survivorship. We organize the analysis around four clinically relevant pillars: menopause management, sexual health and function, psychosocial hea

TLR7 promotes chronic airway disease in RSV-infected mice.

Respiratory syncytial virus (RSV) commonly infects the upper respiratory tract (URT) of humans, manifesting with mild cold or flu-like symptoms. However, in infants and the elderly, severe disease of the lower respiratory tract (LRT) often occurs and can develop into chronic airway disease. A better understanding of how an acute RSV infection transitions to a LRT chronic inflammatory disease is critically important to improve patient care and long-term health outcomes. To model acute and chronic phases of the disease, we infected wild-type C57BL/6 and toll-like receptor 7 knockout (TLR7 KO) mi

A Remote Chronic Disease Management Program to Improve Cardiovascular and Metabolic Outcomes in a Diverse Community-Based Diabetic Population: Study Design of the TAMIS Trial.

INTRODUCTION: Cardiovascular and cerebrovascular diseases are projected to expand in the coming decades as the main cause of death. This study outlines the escalating burden of cardiovascular diseases in low- and middle-income countries (LMICs), particularly in Latin America (LA), which is expected to surpass that of other high-population regions. Despite advances in prevention strategies, adherence to therapeutic goals remains a global challenge, especially in LMICs where control rates are notably low for conditions such as hypertension, type 2 diabetes mellitus (DM2), and dyslipidemia. The health system burden of chronic disease care: an estimation of provider costs of selected chronic diseases in Uganda.

OBJECTIVE: To explore the chronic disease services in Uganda: their level of utilisation, the total service costs and unit costs per visit. METHODS: Full financial and economic cost data were collected from 12 facilities in two districts, from the provider's perspective. A combination of ingredients-based and step-down allocation costing approaches was used. The diseases under study were diabetes, hypertension, chronic obstructive pulmonary disease (COPD), epilepsy and HIV infection. Data were collected through a review of facility records, direct observation and structured interviews with he

Assessing group medical visits for the management of chronic pain in older adults: a systematic review and meta-analysis.

BACKGROUND AND OBJECTIVES: Chronic pain in older adults affects mobility, cognition, and social isolation. Episodic medical visits often struggle to address the complexity of chronic pain. Group medical visits combine clinical care with group education and peer support, addressing limitations of one-on-one care. This systematic review and meta-analysis evaluated group medical visit's effectiveness for chronic pain management in older adults. RESEARCH DESIGN AND METHODS: A systematic literature search was conducted in June 2025 for studies evaluating group medical visits in adults aged ≥60 wit

Core High-Risk Foot Profiles and Surgery-Coded Care-Intensity Indicators Among Hajj Pilgrims Presenting with Foot and Ankle Conditions: A Presentation-Level Analysis.

BACKGROUND/OBJECTIVES: Foot and ankle presentations during Hajj occur in a dense mass-gathering environment where prolonged walking, heat exposure, crowding, variable footwear, and limited self-care can interact with chronic disease and wound vulnerability. Previous Hajj studies have described foot injuries and diabetes-related complications, but less is known about whether simple high-risk foot documentation flags identify presentation records with higher care-pathway intensity. The primary objective was to estimate the presentation-level burden of core high-risk foot profiles among pilgrims

Screening and management of atrial fibrillation in primary care.

Atrial fibrillation is a common chronic disease seen in primary care offices, emergency departments, inpatient hospital services, and many subspecialty practices. Atrial fibrillation care is complicated and multifaceted, and, at various points, clinicians may see it as a consequence and cause of multi-morbidity, as a silent driver of stroke risk, as a bellwether of an acute medical illness, or as a primary rhythm disturbance that requires targeted treatment. Primary care physicians in particular must navigate these priorities, perspectives, and resources to meet the needs of individual patient

"An illustrative socio-biological cascade of the concentric model for health-bound networks: social determinants, social support and biological responses".

INTRODUCTION: Health has been associated with social support. Supported patients with complex health issues have a better chance of living healthier, longer lives and enjoying a better quality of life. Research indicates that physical, mental, and emotional health are connected to both biological and social determinants. METHODS: This paper proposes a conceptual, society-to-biology model, the Concentric Model for Health-Bound Networks, that explores the complexity of social support and integrates the foundational principles of systems biology and systems thinking. This transdisciplinary model

Protocol for a randomized controlled trial of an internet-based prevention intervention for young children at-risk for functional abdominal pain.

BACKGROUND: Chronic pain often clusters in families, where parents and their offspring both experience chronic pain conditions. Young children of parents with irritable bowel syndrome (IBS) represent an at-risk group for the development of abdominal pain, disability, and excess health care visits in later childhood. Parental solicitous responses to children's expressions of discomfort and maternal modeling of their own illness behavior contribute to a greater focus on somatic sensations, leading to illness behaviors in children. This randomized controlled trial will test the effectiveness of a

Improved Disabilities of the Arm, Shoulder and Hand scores after myoelectric arm orthosis use at home in chronic stroke: A retrospective study.

BACKGROUND: Most stroke survivors have persistent upper limb impairments after completing standard clinical care. The resulting impairments can adversely affect their quality of life and ability to complete self-care tasks and remain employed, leading to increased healthcare and societal costs. A myoelectric arm orthosis can be used effectively to support the affected weak arm and increase an individual's use of that arm. OBJECTIVE: The study objective was to retrospectively evaluate the outcomes and clinical benefits provided by the MyoPro® orthosis in individuals 65 years and older with upp

Development of a risk stratification-based model for prediction of acute kidney injury in critically ill patients.

Acute kidney injury (AKI) is a complex syndrome with a variety of possible etiologies and symptoms. It is characterized by high mortality and poor recovery of renal function. The incidence and mortality rates of patients with AKI in intensive care units are extremely high. It is generally accepted that early identification and prompt treatment of AKI are essential to improve outcomes. This study aimed to develop a model based on risk stratification to identify and diagnose early stage AKI for improved prognosis in critically ill patients. This was a single-center, retrospective, observational s
