From mice to clinical relevance: humanizing neuroscience with human-based model systems.

Preclinical research in neuroscience has traditionally relied on animal models to investigate disease mechanisms and develop new therapeutic strategies. While these models are valuable to gain mechanistic insights, their translational power remains limited due to interspecific differences and, hence, frequent failures in clinical translation. The uniqueness of the human brain calls for alternative approaches in neuroscientific research that more faithfully capture human physiology and pathology. In recent years, a variety of human-based model systems have emerged, ranging from dissociated neur

Critical evaluation of compositions and clinical relevance of Wharton's jelly-derived biologics.

BACKGROUND: Wharton’s jelly, the mucoid connective tissue that surrounds umbilical cord vessels, contains both acellular and cellular components with recognized relevance to regenerative medicine. The acellular extracellular matrix (ECM) is composed of collagen fibers, hyaluronic acid, proteoglycans, glycosaminoglycans, that provides a scaffold for bioactive molecules such as growth factors and cytokines that support structural integrity and tissue repair. The cellular component of native Wharton’s jelly consists primarily of multipotent mesenchymal stromal cells (MSCs), which are leveraged in

Clinical Relevance of the CALLY Index in Prognostic Stratification of Intrahepatic Cholangiocarcinoma.

BACKGROUND/AIM: Recent investigations have highlighted the prognostic significance of the C-reactive protein-albumin-lymphocyte (CALLY) index across various malignancies. However, its prognostic relevance in patients with intrahepatic cholangiocarcinoma (ICC) treated with surgical resection remains to be fully clarified. This study aimed to evaluate whether the prognostic significance of preoperative CALLY index for long-term outcomes in patients with intrahepatic cholangiocarcinoma undergoing surgical resection. PATIENTS AND METHODS: We retrospectively examined 131 patients who underwent cur

Assessing the Clinical Relevance of BRCA1 RING Domain Variants of Uncertain Significance.

The BRCA1 protein serves an essential function in maintaining genomic integrity, to the extent that up to 80% of women carrying a pathogenic BRCA1 variant develop breast cancer (BC). Most of these carriers would benefit from prophylactic care, but genetic screens that uncover variants of uncertain significance (VUSs) do not provide insight on disease risk or clinical decision-making. In accordance with guidelines established by The American College of Molecular Genetics (ACMG) and Association for Molecular Pathology (AMP), this study produced computational and functional evidence to inform the

Understanding Basic Concepts of Viral Quasispecies: From Evolutionary Dynamics to Clinical Relevance.

This review examines the viral quasispecies concept and its implications for understanding viral evolution, pathogenesis, and the development of effective antiviral therapies. Quasispecies are dynamic populations of closely related but genetically distinct viral genomes, which evolve under mutation and Darwinian selection. Notably, minority variants, often dismissed as "genetic noise", may harbor significant biological differences-such as drug resistance-and become dominant under changing selective pressures. Next-generation sequencing (NGS) has become an indispensable tool for characterizing

Impact of systemic diseases on periodontal remodeling: Mechanisms, mediators, and clinical relevance.

BACKGROUND: Periodontal remodeling is a continuous, adaptive process essential for maintaining the structural and functional integrity of periodontal tissues. While external factors such as bacterial biofilm, excessive forces, and poor oral hygiene are recognized triggers of periodontal pathologies, they do not fully explain their persistent prevalence. OBJECTIVE: This opinion paper emphasizes the impact of systemic diseases on periodontal homeostasis and considers periodontal disease as a manifestation of systemic dysfunction rather than solely a bacterial local disease. METHODS: This paper

Deconditioning in coronary artery disease: an underrecognized problem with contemporary clinical relevance.

Coronary artery disease (CAD) remains a leading cause of morbidity and mortality worldwide. Beyond recurrent ischemic events, many patients experience a sustained and gradual reduction in physical activity and functional capacity after diagnosis, hospitalization, and treatment. Deconditioning is a complex process that encompasses physiological and behavioral decline driven by inactivity, symptoms, fear-avoidance behaviors, and treatment-related factors. It provides a unifying condition through which these deficits can be understood and targeted. Although exercise-based cardiac rehabilitation i

Exercise as a therapeutic strategy for insomnia: Current mechanisms and clinical relevance.

Insomnia affects 10-30 % of adults, with higher prevalence among older individuals, women, and those with comorbid medical or psychiatric conditions. It disrupts cognitive function, emotional regulation, and physical health, elevating risks for depression, cardiovascular disease, and diminished quality of life. Although pharmacological treatments have offered short-term symptom relief, their use has been constrained by side effects, dependency potential, and limited long-term efficacy, underscoring the need for effective non-pharmacological alternatives. Exercise has emerged as a well-supporte

Disinfectant-induced bacterial resistance and antibiotic cross-resistance-mechanisms and clinical relevance.

The rise of antibiotic-resistant bacteria has become an alarming global health challenge. Disinfectants, such as alcohols, aldehydes, chlorine compounds, phenols, quaternary ammonium compounds (QACs), peroxides, and chlorhexidine, are widely used in healthcare settings as a critical line of defense against infection. Nevertheless, their overuse or misuse, especially at subinhibitory concentrations, can promote the emergence of bacterial resistance, potentially leading to cross-resistance to antibiotics. Several mechanisms, including efflux pump activation, alterations in membrane permeability,

Oxidative Stress in Benign Prostatic Hyperplasia: Mechanisms, Clinical Relevance and Therapeutic Perspectives.

BACKGROUND/OBJECTIVES: Benign prostatic hyperplasia (BPH) is among the most common conditions affecting men as they age, resulting in lower urinary tract symptoms (LUTS) that can profoundly impact quality of life. While historically attributed primarily to androgenic imbalances, current evidence implicates additional factors-particularly oxidative stress (OS) and chronic inflammation-in BPH pathogenesis. This review aims to synthesize research on the interplay between OS, inflammation, and hormonal regulation in BPH, emphasizing their clinical relevance and potential therapeutic implications. …

Clinical relevance and management of atrial high-rate episodes: a narrative review.

Atrial high-rate episodes (AHREs) are atrial tachyarrhythmias detected by cardiac implantable electronic devices when the atrial rate exceeds the device's programmed detection threshold. The frequency and duration of these episodes vary, with those lasting more than 5 min generally considered clinically significant. The occurrence of AHREs has been associated, in multiple clinical trials, with an increased risk of progression to overt atrial fibrillation (AF) and a higher incidence of thromboembolic events, particularly ischemic stroke. Several studies have demonstrated that the strength of th

Clinical Relevance of 'Cap' and 'Track' Development after Recent Small Subcortical Infarct.

OBJECTIVE: After a recent small subcortical infarct (RSSI), some patients develop perilesional or remote hyperintensities ('caps/tracks') to the index infarct on T2/FLAIR MRI. However, their clinical relevance remains unclear. We investigated the clinicoradiological correlates of 'caps/tracks', and their impact on long-term outcomes following RSSI. METHODS: We identified participants with lacunar stroke and MRI-confirmed RSSI from 3 prospective studies. At baseline, we collected risk factors, RSSI characteristics, small vessel disease (SVD) features, and microstructural integrity on diffusion…

Staging for endometrial carcinoma FIGO 2023 and its relevance for clinical practice.

BACKGROUND: International Federation of Gynaecology and Obstetrics (Fédération Internationale de Gynécologie et d'Obstétrique - FIGO) introduced a new staging system for endometrial carcinoma - FIGO 2023 - in June 2023. OBJECTIVE: The new staging system differs significantly from previous versions. The new system represents a significant departure from the traditional staging systems for other gynaecological cancers, as the definition of individual stages includes not only the traditional anatomical extent of the tumour, but also the molecular profile of the tumour and other histopathological
