Fractures of the proximal radial head and neck in children with emphasis on those that involve the articular cartilage.

This is a review of 116 children who had a fracture of the proximal radial head or neck over a 15-year period. Of 33 teenagers with closed physes, 17 (52%) had intraarticular involvement. Of 83 younger children with an open proximal radial physis, six (7%) had an intraarticular fracture (Salter-Harris type III or IV). Of the 17 patients with closed physes and intraarticular fracture, 13 had adequate follow-up. There were eight excellent, three good, one fair, and one poor results. Of the six children with open physes and intraarticular fracture, there were one good and five poor results. This

A Rare Case of an Irreducible Patella Dislocation.

Reports of irreducible patellar dislocations are exceedingly sparse throughout the literature. Obvious radiographic or physical exam findings including fracture or inversion of the patella are often present to explain the block to reduction. Not described previously in the literature is the instance of an irreducible patella dislocation in the setting of innocuous appearing injury imaging. We present a case of a healthy thirty-two-year-old female who sustained an irreducible lateral patella dislocation while participating in a dance aerobics class. Closed means of reduction were unsuccessful,

First report of mandibular ramus fracture associated with anterior dislocation of ipsilateral condyle into temporal fossa and locked jaw: A Rarity.

Mandibular ramus fracture is the second least common variant of fracture of mandible. Recently there has been an upsurge in the incidence of ramus fracture due to high velocity road traffic accidents. Ramus fracture is mostly seen in association with other mandibular fracture. This is the first report of mandibular ramus fracture associated with anterior dislocation of ipsilateral condyle into temporal fossa which was treated successfully under local anaesthesia by closed reduction.

[Operative treatment of femoral head fractures through an anterior minimally invasive incision].

Fractures of the femoral head rarely occur and are often combined with dislocation of the hip joint. A separate classification of the femoral head fracture was described in 1957 by Pipkin. The operative treatment depends on the type of injury and requires extensive knowledge of the various operative approaches. We report on a 40-year-old female patient with Pipkin II fracture and operative treatment of the injury through an anterior minimally invasive incision. We present a review of the literature, evaluate the type of fractures, the kind of approaches and treatment and the long-term results.

[Drill wire osteosynthesis in subcapital humerus fractures: percutaneous or open procedure?].

Between 1978 and mid-1990, 135 patients suffering from dislocated, non-luxated fracture of the humerus at the anatomical neck (fractura colli anatomici) were treated by means of open or closed percutaneous drill wire osteosynthesis. Follow-up examination after an average of 9 months did not show any significant differences between the two surgical approaches in 117 patients, independent of the shape of the fracture. However, in about 30% of the cases it was impossible to employ the percutaneous approach due to the presence of an obstacle to reduction, so that open reduction and fixation was th

Anterior dislocation of the shoulder with ipsilateral humerus shaft fracture.

Anterior dislocation of the shoulder joint with ipsilateral humerus shaft fracture is a very rare injury. Until July 2006 only 18 cases have been reported in the literature. Here we report a case of Anterior Shoulder dislocation with greater tuberosity and shaft of humerus fracture. The patient was treated by Open reduction and internal fixation of the shaft fracture followed by closed reduction of the dislocation. A post-operative rehabilitation program was instituted for complete rehabilitation of the shoulder function.

Functional results of unilateral mandibular condylar process fractures after open and closed treatment.

BACKGROUND: This retrospective study compared the functional results of unilateral mandibular condylar process fractures treated either by open reduction or by closed treatment. METHODS: Sixty-six patients with unilateral mandibular condylar process fractures were reviewed. Thirty-six patients received open reduction, and the other 30 underwent closed treatment (intermaxillary fixation only). Each group was further divided into condylar and subcondylar subgroups according to fracture level. The functional outcome was evaluated by posttreatment occlusion status, maximal mouth opening, facial s

Traumatic posterior Atlanto-axial dislocation: case report of an atypical C1-C2 dislocation with an anterior arch fracture of C1.

BackgroundAn atypical case of a traumatic posterior C1-C2 dislocation with an anterior arch fracture of C1 is reported. A novel conservative treatment for this rare lesion is described.Case presentationAn eighty-nine-year-old male fell off a ladder at home and presented with an acute traumatic cervical spine trauma, which we believe involved a distraction mechanism. The patient was neurologically intact; he denied any weakness, numbness or paresthesia. A preoperative CT-scan demonstrated a posterior dislocation with an anterior arch of C1 fracture. Conservative management was elected. Reductio

Effect analysis of different methods on radial neck fracture in children.

To analyze the curative effect of varying treatment types in 75 pediatric cases of radial neck fracture and explore the prognosis-related factors. Clinical data of 75 children with radial neck fractures treated in our hospital from January 2015 to December 2016 were retrospectively collected. The relationship between age, fracture type, treatment method, X-ray examination after reduction, and prognosis was analyzed. Age was related to prognosis. The excellent and good rate of treatment was 89.25% for children with age ≤ 10 and 57.89% for children over 10 years old. The type of fracture was clo

The prevention and treatment of complications from fractures of the distal radius and ulna.

Almost one third of fractures of the distal radius are associated with some complication, either bony or soft tissue. Proper reduction and appropriate stabilization of the fracture are necessary to prevent many of these complications, particularly those involving the articular end of the radius. Although closed treatment is usually satisfactory, an open reduction and internal fixation of the fracture often is indicated in younger patients.

A Protocol for Percutaneous Transarticular Fixation of Sanders Type II and III Calcaneal Fractures With or Without an Added Mini-Open Approach.

Intra-articular fracture of the calcaneus is one of the most displeasing fractures if not properly managed. Open reduction and internal fixation have been associated with a high incidence of postoperative soft tissue complications. Closed reduction and percutaneous fixation have resulted in a greater incidence of postoperative subtalar osteoarthritis with improper reduction of the articular surface. In the present study, a mini-open approach was used in cases of failure of articular surface restoration with closed reduction. A total of 64 feet in 57 consecutive patients with an intra-articular

A neurologic complication following Monteggia fracture.

Posterior interosseous nerve palsy can occur following a Monteggia fracture by a variety of mechanisms. In this case report of a 35-year-old man the nerve became wrapped around an anterolaterally dislocated radial head in the course of an unsuccessful attempt as closed reduction. Cadaveric dissections demonstrated how reduction of the radial head can be blocked by an enwrapped nerve. Obviously open reduction of the fracture with exploration of the nerve is the treatment of choice.

Irreducible fracture of the wrist in a child. Entrapment of the extensor tendons.

A previously undescribed fracture-separation of the distal radial and ulnar physis is presented. A Salter-Harris Type II phys-metaphyseal fracture of both radius and ulna, with volar displacement, was found irreducible by closed means. At operation the extensor tendons were found entrapped between the metaphysis and epiphysis of both radius and ulna, which prevented reduction. Reduction was possible only after this entrapment was released.

Treatment of Fracture-Related Infections with Bone Abscess Formation after K-Wire Fixation of Pediatric Distal Radius Fractures in Adolescents-A Report of Two Clinical Cases.

Closed reduction and K-wire fixation of displaced distal radius fractures in children and adolescents is an established and successful surgical procedure. Fracture-related infections after K-wire fixation are rare but can have significant consequences for the patient. There is a lack of literature on the treatment of K-wire-associated fracture-related infections in children and adolescents. Herein, we report two cases of fracture-related infection after initial closed reduction and Kirschner wire fixation in two adolescents. One 13-year-old boy and one 11-year-old girl were seen for fracture-r

Percutaneous Screw Fixation of Lateral Condylar Humeral Fractures.

Lateral condylar humeral fractures are the second most common elbow injury in children and commonly occur between the ages of 5 and 10 years. There are several systems for classification of this fracture, including those of Milch (fracture line location) and Jakob et al. (displacement). Although nonoperative management is indicated for nondisplaced or minimally displaced fractures (<2 mm), operative fixation is indicated for greater displacement. Traditionally, open reduction and internal fixation has been the method used to ensure adequate reduction. However, closed reduction and internal fix

Tibial pilon fracture repair using Ilizarov external fixation, capsuloligamentotaxis, and early rehabilitation of the ankle.

Open surgical treatment of comminuted tibial pilon fractures is associated with substantial complications in many patients. Indirect reduction and stabilization of fractures by means of distraction using a circular external fixator can be a useful method of achieving satisfactory joint restoration, and employs the principles of closed reduction to realign disrupted bones and joint structures. In this report, we describe the case of a patient with a comminuted tibial pilon fracture treated by means of closed reduction and stabilization using a circular external fixator.

Tibiofibula Transposition in High-Energy Fractures.

We report two cases of failed attempts at closed reduction of high-energy tibial fractures with an associated fibula fracture. The first case was a 39-year-old male involved in high-speed motorbike collision, while the second was a 14-year-old male who injured his leg following a fall of three metres. Emergency medical services at the scenes of the accidents reported a 90-degree valgus deformity of the injured limb and both limbs were realigned on scene and stabilized. Adequate alignment of the tibia could not be achieved by manipulation under sedation or anaesthesia. Open reduction and exposu

Management of Displaced Radial Neck Fractures with Closed Technique using Titanium Elastic Nail and Review of Literature: A Case Report.

INTRODUCTION: Injuries around the elbow are common in pediatric population. Amongst them, most common is fractures of the supracondylar part of humerus, followed by condylar fractures. Fractures of the neck of the radius account for five-ten percent of the total injuries. Various techniques for closed reduction of the fracture have been described in literature. Open reduction and fixation is indicated when closed reduction fails to provide satisfactory results and the angulation remains more than 45-60 degrees. CASE REPORT: We present two cases of severely displaced radial neck fractures trea
