Severe vitamin K deficiency-associated coagulopathy triggered by Clostridioides difficile infection and antibiotic-associated dysbiosis: A case report and literature review.

PURPOSE: Clostridioides difficile infection (CDI) represents a major healthcare associated infection with potentially life-threatening complications. While gastrointestinal and systemic manifestations are well recognized, severe micronutrient deficiencies, particularly vitamin K deficiency are rarely described. We aimed to report a unique case of CDI-and antibiotic-associated dysbiosis and malabsorption leading to profound vitamin K deficiency and coagulopathy, thereby highlighting the clinical intersection between infection, microbiome disruption, and hemostasis. METHODS: We report the clini…

15-strain live biotherapeutic product or same donor fecal microbiota transplant for recurrent Clostridioides difficile infection: a randomized phase 1b trial.

Fecal microbiota transplant (FMT) is an effective therapy for recurrent Clostridioides difficile infection (rCDI) but has undefined composition and poor scalability. In vitro manufactured live biotherapeutic products (LBPs) enable both scalability and defined strain composition but with higher manufacturing complexity, resulting in few LBP clinical trials. Here we show how an accessible platform to produce human-grade LBPs could accelerate LBP development. We provide regulatory documentation and manufacturing protocols to facilitate translating microbiome advances to human trials. With this pl…

Amino acid-based formula feeding is associated with increased risk of Clostridioides difficile-associated disease in children younger than two years.

Infants and toddlers were previously considered to be exempt from Clostridioides difficile-associated disease (CDAD). However, recent studies reported that Clostridioides difficile (C. difficile) infection can cause severe gastrointestinal symptoms within this age group, yet the influential risk factors related to CDAD in infants remain unknown. Here, we investigated 27 CDAD cases in infants and toddlers under two and found that the amino acid-based formula (AAF) feeding is a notable risk factor closely associated with the development of CDAD in infants. Infants fed with AAF exhibit reduced le…

Central Line-Related Bloodstream Infection by Saccharomyces cerevisiae Following Probiotic Use in a Patient with Clostridioides difficile Colitis: A Case Report.

Clinical infections caused by Saccharomyces cerevisiae develop infrequently. We report a case of central line-related bloodstream infection caused by S. cerevisiae in a patient with Clostridioides difficile colitis and central venous catheter placement. The administration of probiotics in C. difficile colitis was associated with fungemia caused by S. cerevisiae. This case suggests the potential for serious infection caused by S. cerevisiae in immunocompromised patients with C. difficile colitis. Removal of the central venous catheter may be important for a better prognosis in fungemia caused b…

Phenotypic and genotypic antimicrobial resistance profiles of clinical Clostridioides difficile isolates collected from private and public health settings in South Africa.

PURPOSE: Antimicrobial resistance (AMR) is important in the pathogenesis and spread of Clostridioides difficile infection (CDI). However, very little is known about the association of AMR and C. difficile in South Africa. The present study aimed to investigate the phenotypic and genotypic antimicrobial profiles of clinical C. difficile isolates. METHODS: Phenotypic antimicrobial susceptibility testing (AST) and whole genome sequencing (WGS) were used to characterize the isolates. RESULTS: The AST showed that 43 isolates were susceptible to metronidazole and vancomycin. The WGS revealed a Pni…

Tubo-ovarian abscess caused by Clostridioides difficile after eight months of surgery: Case report and review of extraintestinal abdominal abscess cases.

We present a case of tubo-ovarian abscess (TOA) caused by Clostridioides difficile (CD) in a 43-year-old female. Despite lacking a history of sexually transmitted diseases, the patient had undergone paraovarian cystectomy nine months before admission. Transvaginal ultrasonography performed eight months post-surgery revealed left ovarian enlargement, accompanied by subsequent lower abdominal pain and fever exceeding 38 °C. As oral antibiotic treatment was ineffective, the patient was admitted to our hospital. Computed tomography upon admission revealed a massive TOA. Surgical drainage of the ab…

Comparison of Risk Stratification Approaches to Identify Patients with Clostridioides difficile Infection at Risk for Multidrug-Resistant Organism Gut Microbiota Colonization.

INTRODUCTION: Multidrug-resistant organisms (MDRO) commonly colonize the gut microbiota of patients with Clostridioides difficile infection (CDI). This increases the likelihood of systemic infections with these MDROs. To help guide MDRO screening and/or empiric antibiotic therapy, we derived and compared predictive indices for MDRO gut colonization in patients with CDI. METHODS: This was a multicenter, retrospective cohort study of adult patients with CDI from July 2017 to April 2018. Stool samples were screened for MDRO via growth and speciation on selective antibiotic media and confirmed us…

In vivo evaluation of Clostridioides difficile enoyl-ACP reductase II (FabK) Inhibition by phenylimidazole unveils a promising narrow-spectrum antimicrobial strategy.

Clostridioides difficile infection (CDI) is a leading cause of hospital-acquired diarrhea, which often stem from disruption of the gut microbiota by broad-spectrum antibiotics. The increasing prevalence of antibiotic-resistant C. difficile strains, combined with disappointing clinical trials results for recent antibiotic candidates, underscore the urgent need for novel CDI antibiotics. To this end, we investigated C. difficile enoyl ACP reductase (CdFabK), a crucial enzyme in de novo fatty acid synthesis, as a drug target for microbiome-sparing antibiotics. To test this concept, we evaluated t…

CDBN-YGXZ, a Novel Small-Molecule Drug, Shows Efficacy against Clostridioides difficile Infection and Recurrence in Mouse and Hamster Infection Models.

Clostridioides difficile infection (CDI) causes severe diarrhea and colitis, leading to significant morbidity, mortality, and high medical costs worldwide. Oral vancomycin, a first-line treatment for CDI, is associated with a high risk of recurrence, necessitating novel therapies for primary and recurrent CDI. A novel small-molecule compound, CDBN-YGXZ, was synthesized by modifying the benzene ring of nitazoxanide with lauric acid. The mechanism of action of CDBN-YGXZ was validated using a pyruvate:ferredoxin/flavodoxin oxidoreductase (PFOR) inhibition assay. The efficacy of CDBN-YGXZ was eval…

Impact of a Clostridioides Difficile Testing Computerized Clinical Decision Support Tool on an Adult Stem Cell Transplantation and Hematologic Malignancies Unit.

Clostridioides difficile infection rates are higher in hospitalized hematopoietic stem cell transplantation (HSCT) recipients and patients with hematologic malignancy (HM) compared with the general population. This is related both to extensive exposure to antibiotics as well as to frequent and often prolonged hospitalization. In this population, with numerous potential causes of diarrhea, a subset of C difficile detected is presumed to represent colonization rather than clinical infection. The use of decision support tools to guide ordering in hospitalized patients has been reported to decreas…

A global to local genomics analysis of Clostridioides difficile ST1/RT027 identifies cryptic transmission events in a northern Arizona healthcare network.

Clostridioides difficile is a ubiquitous, diarrhoeagenic pathogen often associated with healthcare-acquired infections that can cause a range of symptoms from mild, self-limiting disease to toxic megacolon and death. Since the early 2000s, a large proportion of C. difficile cases have been attributed to the ribotype 027 (RT027) lineage, which is associated with sequence type 1 (ST1) in the C. difficile multilocus sequence typing scheme. The spread of ST1 has been attributed, in part, to resistance to fluoroquinolones used to treat unrelated infections, which creates conditions ideal for C. dif…

Secondary Prophylaxis of Recurrent Clostridioides difficile Infections During Systemic Antibiotics with Vancomycin (SPORES-V): study protocol for a randomized controlled trial.

BACKGROUND: Clostridioides difficile infection (CDI) is a prevalent, morbid, and expensive disease that recurs in approximately 25% of patients despite treatment. Re-exposure to systemic antibiotics is one of the strongest risk factors for recurrence. Observational evidence and two small randomized controlled trials suggest that secondary vancomycin prophylaxis during antibiotic re-exposure may prevent recurrences, but the current evidence is not definitive. METHODS: SPORES-V is a Bayesian multicenter phase III double-blind randomized controlled trial. Patients with CDI in the preceding 120 d…

Pharmacovigilance signals and regional burden of antibiotic-associated Clostridioides difficile infection: a pharmacovigilance study and Global Burden of Disease Study 2023.

BACKGROUND: Despite the substantial global burden of Clostridioides difficile infection (CDI) and its close relationship with antibiotic exposure, evidence characterizing drug- and age-specific risk patterns remains limited. OBJECTIVES: This study aimed to characterize antibiotic-associated CDI by assessing drug- and age-specific signal patterns and geographic and temporal variations. METHODS: Using the FDA Adverse Event Reporting System (FAERS), antibiotic-associated CDI reports were identified and evaluated through disproportionality analyses (information component [IC] and reporting odds …

Reliable delineation of Clostridioides difficile and related members of the family Peptostreptococcaceae using phylogenomics and spore coat protein-specific molecular markers.

Traditional bacterial classification relies on phenotypic traits (e.g., morphology and metabolic profiles), but these methods lack resolution for closely related taxa and are biased by culture conditions. While 16S rRNA gene sequencing is a widely used molecular complement, it fails to resolve closely related Peptostreptococcaceae species, including Clostridioides difficile. These limitations have caused family-level taxonomic confusion and ambiguous Clostridioides genus boundaries, hindering clinical identification of pathogenic strains and posing public health risks. To address these limitat…

Simulating interventions to prevent gastro-intestinal transmission of Clostridioides difficile and antimicrobial-resistant bacteria in healthcare settings: a model calibration study.

BACKGROUND: Hospital-acquired infections caused by pathogens such as Clostridioides difficile and antimicrobial-resistant bacteria are a major burden to healthcare systems. Infection prevention and control (IPC) measures are used to limit transmission, but quantifying the impact of individual interventions remains challenging. METHODS: Here, we describe an agent-based model (ABM) that simulates faecal-oral transmission of pathogens under antimicrobial selection pressure in hospital settings. The model was calibrated to a C. difficile outbreak using approximate Bayesian computation. FINDINGS:…

Lactococcus lactis and Bifidobacterium longum attenuate Clostridioides difficile- or Clostridium symbiosum-induced colitis and depression/anxiety-like behavior in male mice.

Clostridioides difficile causes severe colitis, which induces neuroinflammation and psychiatric disorder. In a preliminary study, we isolated Clostridium symbiosum from the stools of patients with ulcerative colitis. Therefore, we first examined whether oral infection with C. difficile or C. symbiosum could induce colitis and depression in male mice. Orally gavaged C. difficile or C. symbiosum caused diarrhea, bodyweight loss, depression/anxiety-like behavior, and tumor necrosis factor (TNF)-α and interleukin (IL)-6 overexpression in the colon and hippocampus in the pseudo-germ-free (PGF) and …
