The treatment of trigeminal autonomic cephalalgias requires very careful attention to clinical aspects. It is important to spend enough time assessing the patient to arrive at an accurate diagnosis. Identifying trigger factors (eg, alcohol), when applicable, is part of the therapy, as behavior modifications may be necessary. Cluster headache treatment should never be delayed; patients should be able to visit the clinic within 48 hours to expedite medication initiation. Abortive therapy typically is best achieved with nasal oxygen, sumatriptan injections, or both. Typically, a steroid taper is …

Peripheral neurostimulation techniques have emerged as promising treatments for patients with medically intractable, highly disabling chronic daily headaches including chronic migraine (CM) and chronic cluster headache (CCH) besides other less common headache syndromes. Encouraging controlled and open label data in medically intractable CM and trigeminal autonomic cephalalgias (TACs) have suggested a meaningful therapeutic role for occipital nerve stimulation (ONS). In view of the frequent occurrence of pain in the first branch of trigeminal nerve, percutaneous supraorbital nerve stimulation a…

OBJECTIVE: We represent primary headaches within the International Headache Classification (ICHD3) in matrix form and show that this representation allows for automated diagnosis as well as additional insights into headache classification.
BACKGROUND: ICHD3 has been the gold standard for clinical trials and research in headache medicine. Given its criteria-based language, ICHD3 can be interpreted as a set of logical statements and as a result can be encoded as the biadjacency matrix of a mathematical graph. This paper implements this interpretation of the classification and explores the clini

Trigeminal autonomic cephalalgias (TACs) are a distinct group of primary headache disorders characterized by unilateral pain and autonomic symptoms such as tearing, nasal congestion, and ptosis. This article provides an in-depth review of the different TAC subtypes, including cluster headache, short-lasting unilateral neuralgiform headache with conjunctival injection and tearing, short-lasting unilateral neuralgiform headache with cranial autonomic features, paroxysmal hemicrania, and hemicrania continua, highlighting their unique clinical features and treatment responses. Early recognition is…

Non-invasive vagus nerve stimulation (nVNS) has been shown to be effective in patients with cluster headache, another trigeminal autonomic cephalalgia (TAC), with cranial parasympathetic autonomic activation during the attacks.

Neurologists can only classify headaches, not patients. Doing that, they are well aware that this is partially arbitrary. But classification is essential, especially for primary headaches (migraine, tension type headache, cluster headache) to achieve epidemiological and physiopathological studies. The criteria of definition for each type of headache must be established with pertinence. Neurologists, general practitioners as well as radiologists can then use these criteria to imagine an algorithm for the diagnostic procedure of headache patients. The aim of this publication is to describe the I…

PURPOSE OF REVIEW: We explore the anatomy of the central and peripheral autonomic pathways involved in primary headache as well as the mechanisms for secondary headache associated with disorders of the autonomic nervous system. The prevalence and clinical presentation of cranial and systemic autonomic symptoms in these conditions will be discussed, with a focus on recent studies.
RECENT FINDINGS: Several small studies have utilized the relationship between headache and the autonomic nervous system to identify potential biomarkers to aid in diagnosis of migraine and cluster headache. Headache …

Headache is a frequent symptom in otorhinolaryngology. Such a secondary headache is typically caused by an organic disease in the head and neck area. Much more frequent is primary headache i.e. that an organic cause cannot be found. The most frequent primary headache types are migraine, tension-type headache, and cluster headache. What has to be done when the otorhinolaryngologist cannot find a cause for a headache in the head and neck area? This continuing education article should enable the otorhinolaryngologist to perform a first differential diagnostic exploration. Often a detailed inquiry…

The relationship between sleep and headache has been known for over a century. Headache and sleeping problems are both some of the most commonly reported problems in clinical practice, and cause considerable social and family problems, as well as socio-economic impact and costs. There is a clear association between headache and sleep disturbances, especially headaches occurring during the night or early morning. The mechanism and causes are complex, multifactorial and poorly understood. Headache disorders like migraine, tension-type headache, cluster headache and hypnic headache all affect or …

A 36-year-old woman experienced an excruciating right retro-orbital paroxysmal headache after a stroke of the anterior two-thirds of the right hemicord at the C1 level (anterior spinal artery territory) and the dorsal medulla (posterior spinal artery territory). A right vertebral artery dissection was demonstrated. This unusual infarct mimicked a cluster headache attack or paroxysmal hemicrania.

Chronic daily headache (CDH) is a heterogeneous group of headaches that includes primary and secondary varieties. Primary CDH is a frequent entity that probably affects 4-5% of the population. It can be subdivided into headaches of short duration (<4 h/attack) like chronic cluster headache, and disorders of long duration (>4 h/attack). Primary CDH of long duration includes transformed migraine, chronic tension-type headache, and new daily persistent headache and hemicrania continua. Analgesics, ergots and triptan overuse are frequent in all types of CDH. We revise recent insights into the epid…

DISCUSSION: Melatonin has been used successfully as a treatment for migraine, cluster headaches and other headaches. There is a rationale for including the pineal gland as a relevant brain structure in the mechanisms of headache pathophysiology, and melatonin as a treatment option in primary headache.

Facial temperature was measured thermographically and pupillary diameter recorded photographically during and between episodes of headache a … d during the active phase of CPH. These findings, which are similar to those previously reported in cluster headache, suggest that CPH is associated with an ocular sympathetic deficit and with overactivity in the greater superficial petrosal nerve.

Temporomandibular joint (TMJ) dysfunction may manifest itself clinically by a variety of presentations ranging from headache, pre-auricular pain or tenderness, otalgia, to mandibular hypomobility. Some symptoms may mimic forms of facial pain such as: temporal arteritis, migraine, cluster headache, trigeminal or glossopharyngeal neuralgias, myofascial pain dysfunction, or muscle contraction (tension) headache. This article will focus on a relatively new diagnostic tool that may be used to examine the TMJ for intracapsular pathology which may be responsible for the presenting patients' symptoms.
